1 EXERCISE #1 Target Audience: Forklift operators Exercise Objective: Evaluate understanding of safe forklift operation and pre-use inspection criteria Instructions: Answer the questions below. Review your workplace s forklift safety program and discuss any incidents or near misses that have occurred while operating a forklift. 1. Describe how to properly load a forklift truck. 2. How often should the forks of forklift trucks be inspected? Obtain a copy of your workplace s inspection checklist, if available, to fill out. 3. What should an operator avoid when operating a forklift truck?
2 ANSWERS AND GUIDANCE (OSHA.gov) 1. Do not exceed the recommended load limit of your lift truck. Each lift truck has a maximum load limit. The load limit is shown on the data plate of the lift truck. Position the load according to the recommended load center. The load limit of the lift truck decreases as the load center is raised. Do not add extra weight to counterbalance an overload. Keep loads close to the front wheels to keep lift truck stable. 2. Under normal operating conditions, forks should be inspected: Daily: Operators should make visual inspection of forks during the pre-start-up check, with special attention to permanent distortions and cracks. Every six months: A thorough inspection of forks should be done every 6 months, preferably by a trained individual, to check for any cracks and distortions. Forks may need inspections more often, depending on the use of the equipment.
3 3. The operator should avoid: Trying to move or adjust any part of the load, the forklift, or the surroundings when on the forklift. Lifting a load that extends above the load backrest unless no part of the load can possibly slide back toward the operator. Allowing anyone but the operator to ride on the forklift. Using pallets elevated by forklifts as an improvised working platform. Allowing anyone to stand or walk under the elevated part of any forklift, whether loaded or unloaded. Inspection Checklist Guidance The forklift must be checked for defects daily usually by the operator before beginning a shift. Even if you operate a forklift safely, a defect can cause or contribute to a serious accident. Some things to look for are: Is the horn working? (The horn should be used at intersections and wherever vision is obstructed.) Are there leaks? such as of fuel, hydraulic oil, engine oil, or radiator coolant. These can cause slipping hazards or lead to hydraulic failure. Are tires at proper pressure and free of damage? A tire with low pressure or a tire failure can cause a forklift to tip or fall when a load is high. Are the forks, top clip retaining pin, and heel in safe operating condition? Is the load backrest securely attached? Visually check hydraulic hoses, mast chains, cables and stops. Are they in safe operating condition? Are fuel connections tight and battery terminals covered? Dropping a piece of metal across battery terminals can cause an explosion. Is there a lot of lint, grease, oil, or other material on the forklift that could catch on fire? Do all controls, such as lift, lower, and tilt, work smoothly? Are they labeled? Is steering responsive? A lot of play or hard steering will reduce your control. Do brakes stop smoothly and reliably? Sudden stops can cause tipping. Does the parking brake hold the forklift on an incline? Is the load capacity plate readable?
4 SAMPLE GENERIC CHECKLIST FOR POWERED INDUSTRIAL TRUCKS Overhead Guard Are there broken welds, missing bolts, or damaged areas? Hydraulic Cylinders Is there leakage or damage on the lift, tilt, and attachment functions of the cylinders? Mast Assembly Are there broken welds, cracked or bent areas, and worn or missing stops? Lift Chains and Rollers Is there wear or damage or kinks, signs of rust, or any sign that lubrication is required? Is there squeaking? Forks Are they cracked or bent, worn, or mismatched? Is there excessive oil or water on the forks? Tires What do the tires look like? Are there large cuts that go around the circumference of the tire? Are there large pieces of rubber missing or separated from the rim? Are there missing lugs? Is there bond separation that may cause slippage? Battery Check Are the cell caps and terminal covers in place? Are the cables missing insulation? Hydraulic Fluid Check level? Gauges Are they all properly working? Steering Is there excessive free play? If power steering, is the pump working? Brakes If pedal goes all the way to the floor when you apply the service brake, that is the first indicator that the brakes are bad. Brakes should work in reverse, also. Does the parking brake work? The truck should not be capable of movement when the parking brake is engaged. Lights If equipped with lights, are they working properly?
5 Horn Does the horn work? Safety Seat If the truck is equipped with a safety seat, is it working? Load Handling Attachments Is there hesitation when hoisting or lowering the forks, when using the forward or backward tilt, or the lateral travel on the side shift? Is there excessive oil on the cylinders? Propane Tank Is the tank guard bracket properly positioned and locked down? Propane Hose Is it damaged? It should not be frayed, pinched, kinked, or bound in any way. Is the connector threaded on squarely and tightly? Propane Odor If you detect the presence of propane gas odor, turn off the tank valve and report the problem. Engine Oil Check levels. Engine Coolant Visually check the level. Note: Never remove the radiator cap to check the coolant level when the engine is running or while the engine is hot. Stand to the side and turn your face away. Always use a glove or rag to protect your hand. Transmission Fluid Check levels? Windshield Wipers Do they work properly? Seat Belts Do they work? Safety Door (found on stand-up rider models) Is it in place? Safety Switch (found on stand-up riding tow tractors) Is it working? Hand guards (found on stand-up riding tow tractors, walking pallet trucks, walking transtackers) Are they in place? Tow Hook Does it engage and release smoothly? Does the safety catch work properly? Control Lever Does the lever operate properly? Safety Interlock (found on order pickers) If the gate is open, does the vehicle run? Gripper Jaws (found on order pickers) Do the jaws open and close quickly and smoothly? Work Platform (found on order pickers) Does the platform raise and lower smoothly? (OSHA.gov)
