1 CITY OF KENOSHA REQUEST FOR PROPOSALS BARRICADES, SHEETING AND TRAFFIC CONES PROPOSAL NO ISSUED : January 10, 2017 NOTICE AND CONDITIONS 1. Proposals for barricades, barricade sheeting and traffic cones as described in the attached specifications, will be accepted by the City of Kenosha, Wisconsin, in the Department of Finance, Municipal Office Building, nd Street, Room 208, Kenosha, Wisconsin until Thursday January 26, 2017 at 4:00 P.M. 2. Proposals must be submitted on the attached form and returned in a sealed envelope. Proposal number and title must be identified on the sealed envelope. Sealed proposals received after the scheduled date and deadline will not be considered. 3. All items furnished shall conform to the applicable requirements of the latest edition of the FHWA, Wisconsin Manual on Uniform Traffic Control Devices and/or I.T.E., and Wisconsin Department of Transportation Specifications. 4. Indicate the delivery schedule in the space provided on the proposal form. 5. Show the manufacturer and model numbers in the spaces provided on proposal form. 6. F.O.B. Destination, prepaid and allowed with no charges for packing or cartage. The destination shall be the City of Kenosha Street Division Facility, th Avenue, Kenosha, Wisconsin The City of Kenosha reserves the right to accept or reject any proposal, and to accept any proposal (s) considered to be the most advantageous to the City. 8. The City of Kenosha is exempt from Federal Excise Tax and State Sales Tax, proposals should be made exclusive of these taxes. Tax Exemption Certificate and/or Tax Exemption Registry Number will be furnished to the successful vendor. 9. Award: A sample of the proposed item(s) maybe required prior to the City accepting an item(s). The award within thirty (30) days after the submission deadline, to the lowest responsible vendor complying with all requirements of this request. The City reserves the right to reject, within one week of delivery, any item which does not comply with the description or proposal conditions stated 1

2 herein, or is found to be damaged at the time of delivery. Replacement of any item so rejected shall be made promptly and at no expense to the City. 10.Successful Vendor agrees to hold the unit price per commodity item constant for a period of one (1) year from the date of award. 11. 3 CITY OF KENOSHA REQUEST FOR PROPOSALS BARRICADES, SHEETING AND TRAFFIC CONES PROPOSAL NO BARRICADES, SHEETING AND TRAFFIC CONE SPECIFICATIONS 1.0 TYPE I FOLDING BARRICADES (Quantity- 36): shall be twenty-four inches (24") wide, folding, wood/metal, and be NCHRP-350 compliant. All metal and wood shall be painted complete (double dipped) with opaque exterior, white enamel paint. All hardware shall be attached with steel hex head cap screws, washers and nuts. RIVETED Construction is NOT ACCEPTABLE. All wood boards shall be one-half inch (0.5") thick plywood. Plastic boards are NOT ACCEPTABLE. The twelve-inch (12") wood top boards shall be faced with full-width, High- Intensity Prismatic reflective bands. One (1) side with LEFT diagonal reflective sheeting and opposite side with RIGHT diagonal reflective sheeting. 2.0 TYPE I VERTICAL PANEL BARRICADES (Quantity- 12): shall be forty inches (40") to forty-five inches (45") high and be NCHRP-350 compliant. Barricade to be constructed of low-density polyethylene orange in color. Barricade shall have a molded hole for flasher or traffic sign mount. Each barricade shall be eight inches (8") wide by thirty-six inches (36") in height with orange and white alternating, permanently attached, High-Intensity Prismatic reflective bands. One (1) side with LEFT diagonal reflective sheeting and opposite side with RIGHT diagonal reflective sheeting. Reflective bands shall be factory applied with high tack adhesive. Barricades shall include thirty (30lb.) pound rubber base (forty-four inch (44") Gemstone or other City-approved alternate.) INCH TRAFFIC CONES WITH FLASHER MOUNT (Quantity- 40): Forty-two inch (42") high, high-visibility orange, low-density weighted traffic cones with molded hole for flasher mount. Cones shall have a large loop handle for ease of handling and which will allow caution tape to be strung through the loop. Cones shall be break-away design, NCHRP-350 compliant, with capability of being stacked. Each cone shall include four (4), four-inch (4") wide, orange and white alternating, permanently attached, High-Intensity Prismatic, reflective bands and sixteen (16 lb.) pound rubber collar base. The reflective bands shall be factory applied with a high tack adhesive. Acceptable product includes the Navigator Channelizer cone or other City-approved alternate) INCH STANDARD TRAFFIC CONES (Quantity - 24): 18" high, three (3 lb.) pound minimum, in fluorescent orange INCH PVC TRAFFIC CONES (Quantity - 12): 18" high, three (3 lb.) pound minimum, solid color, fluorescent orange with one (1), six-inch (6") 3M High-Intensity Prismatic reflective bands. The reflective band shall be factory applied with a high tack adhesive 3

4 6.0 LEFT STRIPE HIGH-INTENSITY PRISMATIC GRADE REFLECTIVE BARRICADE SHEETING (Quantity 3 rolls of 900 '): Alternating orange and white four-inch (4 ) stripes with pressure sensitive adhesive tape. 7.0 RIGHT STRIPE HIGH-INTENSITY PRISMATIC GRADE REFLECTIVE BARRICADE SHEETING (Quantity 3 rolls of 900') : Alternating orange and white four- inch (4 ) stripes, pressure sensitive adhesive tape. 4
