A separate registration card must be provided for each IRP account and for any intrastate vehicles. c. Submit if applicable to your carrier type. Will affect your eligibility to bypass at relevant sites if not provided. d. You must also provide your carrier s Colorado LVC permit if submitting Colorado overweight vehicle permits. e. KYU License required for vehicles with registered gross vehicle weight of 60,000 lbs. or greater. STEP 3: If applying for PrePass Plus or Tolls Only service, refer to the included Vehicle Reference Chart. Write the Vehicle Reference Number (VRN) on the copy of each vehicle s registration card or provide a truck list that indicates the VRN for each vehicle. STEP 4: If applying for PrePass Ag service, an authorized representative of your company must complete the enclosed PrePass Ag Application. Mail the completed form to the address specified. STEP 5: Submit your paperwork via one of the methods listed at the top of your Account Application. For more information about PrePass safety criteria, please visit Interstate carriers must have paid their current Unified Carrier Registration (UCR) fees in order to bypass in participating states. Vehicles that travel in New Mexico must have a valid New Mexico Weight Distance Tax (WDT) account to be eligible to bypass in that state. No UCR or NM WDT credentials need to be submitted with your application; registration will be validated electronically. You may enroll using transponders previously issued by PrePass for your carrier s inventory, or with compatible transponders issued by another party. Transponders will be shipped for your vehicles unless you provide the transponder serial numbers and vehicle assignments at the time of enrollment. State Vehicle-Type Restrictions Arkansas: Vehicles with a Gross Vehicle Weight (GVW) of 26,000 lbs. or less are not eligible to bypass. California: Only commercial motor vehicles listed in CVC are eligible to bypass. These include: Truck tractors; Motor trucks with three or more axles; A vehicle towing a vehicle, such as a trailer or semi-trailer, with a GVW of more than 10,000 pounds; A combination of a motor truck and a vehicle(s), such as a trailer or semi-trailer, that exceeds 40 feet in length when coupled together; Two-axle interstate vehicles with a gross vehicle weight of 10,001 lbs. or more which are part of an interstate fleet and subject to Federal Motor Carrier Safety Regulations; Two-axle interstate vehicles with a gross vehicle weight less than 10,001 lbs. with a DOT number associated with an interstate carrier that also has motor trucks with 3 or more axles or truck tractors enrolled. 04/17

6 PrePass Seasonal Shut-Off Service (SSOS) Procedure Allows customer to shut off PrePass service for a month at a reduced rate of $4.99 and keep the transponder in their truck(s). SSOS will not be prorated. Any request for PrePass service for a single day constitutes a full month of PrePass Service. Requests to move to SSOS must be made no later than two (2) business days before the end of the month for the next month. Requests to move to SSOS on the last day of the month or later will result in a full month PrePass Service Charge the following month. SSOS can be in place for as many months as the customer wants. Bypassing ability can be restored in 48 hours. A regular PrePass account must be opened in order to sign up for SSOS. A truck will receive red lights at all PrePass weigh stations while in SSOS status and the truck must pull into the station if it is open. When a customer wishes to move some or all of their trucks from PrePass Service to SSOS, the following steps must be followed. Customer must notify Lorraine what trucks they want to put in SSOS. Lorraine will notify PrePass. In order for the customer not to be billed for PrePass service, PrePass must be notified of the switch to SSOS no later than the last day of the previous month. PrePass will move requested truck(s) to the SSOS status and be billed the $4.99/per device until requested to activate. The trucks will be moved to SSOS status on the 1 st day of the month following month. The truck will stay in SSOS status and will not be able to bypass until up to 48 hours after Lorraine notifies PrePass to reactivate device(s). When a customer wishes to move some or all of their trucks from SSOS to PrePass Service, the following steps must be followed. Customer must notify Lorraine what trucks they want to move to PrePass Service. Lorraine will notify PrePass. PrePass will move the truck(s) to PrePass Service within 48 hours. The truck will start receiving green lights within the 48 hours and bypassing according to standard PrePass rules. The customer will be billed the standard PrePass Service charge for the month the service was started. The truck will stay in PrePass Service until the above steps are repeated. PrePass Monthly Service will not be prorated. Any request for PrePass service for a single day constitutes a full month of PrePass Service. Requests to move to PrePass from SSOS on any day of the month will result in a full month PrePass Service Charge for that month.

8 Device Return Instructions To ensure proper credit of all return ed devices to your account; please follo w these steps when shipping devices back to PrePass: Steps: 1. Make a List: Before packaging, make a list of the device identification numbers being returned for your records. Device numbers appear on the device below the barcode, and begin with 0705 on black devices; 210 on gray devices; and 67 on blue devices. 2. Proof of Delivery: Make sure a proof of delivery is obtained for device shipments. The United States Post Office offers several methods for which d elivery notification service can be obtained at the time of shipping. Third party shippers such as UPS or FedEx employ a waybill tracking number that can be entered on their websites to view and print delivery details. 3. Insurance: Insure the package for $100 per device being returned. 4. Ship Devices to: PrePass Device Returns 510 Parkland Drive Sandy, UT Please retain these documents with your permanent business records. The customer is responsible for all devices until they are confirmed as received by PrePass. Device status can be checked our website: click on My PrePass (create a username and password if not pr eviously created). Click on Vehicle and Device Management and click on View Device List. All devices received will be removed from this list, or Contact customer service at option 6. Please allow 2 business days from the date of return d elivery to check for removal of devices from your account. If shipped devices are not received, proof of delivery must be provided, in order to remove devices from customer account at no charge. This is in accordance with the terms of the PrePass License Agreement. Effective - 11/20/2007
