3 I. OBJECTIVES: The CONSULTANT shall provide all necessary services to designate and locate existing surface and subsurface Utilities to support the design of complete Right of Way and construction plans on a district wide basis on projects selected by the Florida Department of Transportation (FDOT or Department). In addition to these services, the Department may authorize additional services to include but not limited to, Utility location and verification during construction, identification of existing/proposed utility facilities, resolution of conflicts between utility facilities and proposed construction, documentation of utility company activities for the project s special provisions, plans and notes, securing of executed legal agreements to clear a project for letting as stipulated by state and federal guidelines, analyze and certify utility work schedules for compatibility to FDOT construction schedule, and provide for Right of Way staking for advanced utility relocation as required. The term "designate" in this Scope of Services includes 2-dimensional collection of existing utilities and selected 3-dimensional verification as needed for designation. The term "locate" includes non-destructive excavation to determine size, type and location of existing utility, as necessary for final 3-dimensional verification. Survey includes collection of data on points as needed for designates and locates. Survey data may also be collected using traditional (static) methods or manned/unmanned (drone) mobile remote sensing vehicles. Survey includes analysis and processing of all field collected data, and delivery of all appropriate electronic files. A Professional Land Surveyor, registered in the State of Florida, shall sign and seal the data provided and included in the FDOT Verified Utility Locate Plan Sheets. All information shall be provided in the format requested by the DEPARTMENT. Any data provided by the CONSULTANT is not a substitute for full compliance with all applicable underground utility damage prevention laws. In the execution of its services, the CONSULTANT will work principally with and coordinate its activities with the Department's District Utility Office (DUO) and Professional Services Unit. II. SERVICES TO BE PROVIDED: It is the intent of this Scope of Services that the CONSULTANT, employing qualified, competent and experienced personnel, shall perform the services set forth herein providing services equal to the practice prevalent by CONSULTANT S practicing within the subject area of work and commensurate with the magnitude and intricacy of the work under consideration. In addition, the CONSULTANT shall provide, operate and maintain any appropriate equipment as necessary to perform the services as set forth herein and commensurate with industry standards. These services shall be so complete that it will not be necessary for the Department to supplement any of the operation by its own personnel, except as noted hereinafter. The Department, however, reserves the right to make such reviews from time to time as it may deem necessary or desirable and to maintain proper liaison. The items that follow are not intended to be comprehensive or exclusive but set forth in a general outline of the work that is expected. Stage II SOS A-1 FPID No. TBD

4 This section describes various work items that may be required of the CONSULTANT. All of the work items will not necessarily be required in a single assignment. The CONSULTANT shall be prepared to perform all work included in the work areas covered by the contract. Work areas are defined as all work items included in Section II of this Scope of Services. When work items are referenced to existing specifications and or standards all work must strictly adhere to those specifications and/or standards unless the District Utility Administrator (DUA) approves alternate methods in advance. The CONSULTANT shall be responsible for obtaining all manuals, guides, standards and procedures referenced in the following sections. The most recent editions of any publications shall be used. The CONSULTANT S services shall include, but not be limited to, providing qualified representation to assist the Department's DUO when authorized in the following work areas: A. Subsurface Utility Designating Service The CONSULTANT shall: (1) Obtain all necessary permits from city, county, municipality or other jurisdiction to allow the CONSULTANT to work in existing street, roads, etc. for the purpose of marking, measuring and recording the location of existing subsurface utilities. (2) Designate, record and mark the approximate horizontal location of the existing utilities and their major laterals to existing buildings by a method approved by the Department. No storm sewers are to be designated unless authorized by the Department. All survey work required, including the retracing of the survey centerline of baseline, will be determined and performed by the CONSULTANT. The horizontal designating of the underground utility line shall be within of either side of the underground utility line as shown on the plan sheets. (3) Determine and provide the Department with the approximate depth of all existing utilities as determined by the designator when the readings appear valid. This depth indication is understood by both the CONSULTANT and the Department to be approximate only and is not intended to be used in designing the right-of-way and construction plans. (4) Provide all signs and other safety devices and equipment, including all necessary flagmen needed to control traffic in accordance with Department policy and MUTCD, labor and equipment. (5) Secure all "As Built" plans, plats and other data as supplied by utility companies and FDOT Operations Center Permit Office as are necessary. (6) Provide all necessary support personnel to secure the data outlined in this section. Stage II SOS A-2 FPID No. TBD

5 (7) All data will be developed and furnished to the FDOT utilizing Computer Aided Drafting and Design (CADD) systems. The FDOT makes available software to help assure quality and conformance with policy and procedures regarding CADD. It is the responsibility of the CONSULTANT to meet the requirement in the Department s CADD MANUAL (Topic No ). The CONSULTANT will submit final documents and files as described therein. Additional related information is found in the FDOT Design Manual (Topic No ). In addition to this a hard copy shall also be provided. Text or table formatted information shall be provided to FDOT in Word and/or Excel formats. (8) Provide a weekly summary of work completed and underway. B. Subsurface Utility Locating (Test Hole) Service The CONSULTANT shall: (1) Obtain and review plans at location designated by the Department and obtain utility company's records as required. (2) Obtain all necessary permits from city, county municipality or other jurisdiction to allow the CONSULTANT to work in existing streets, roads, etc. for the purpose of locating (digging), measuring and recording the location of existing subsurface utilities. (3) Coordinate with utility agency/owners as required by this Agreement and as required by law. (4) Neatly cut and remove existing paving. a. Cut area not to exceed 225 square inches. b. Excavate using a method enabling vertical as well as horizontal exploration through this cut. (5) Excavate test holes in such a manner as to prevent any damage to wrappings, coatings, or other protective coverings of existing utilities. (6) Be responsible for any damage to the utility during excavation of same. (7) Backfill and compact with select material around utility structure. (8) Furnish and install a color-coded permanent above ground marker (i.e., P.K. nail, peg, steel pin, or hub), directly above centerline of the structure and record the elevation of the marker. (9) Provide a permanent restoration of the pavement within the limits of the original cut at the time of backfill. 6 than the roadway pavement, the area disturbed shall be restored to the condition prior to the excavation and per Department Standards and Specifications. (10) Provide all signs and other safety devices and equipment, including all necessary flagmen needed to control traffic in accordance with Department policy and MUTCD. (11) Provide complete clean-up of work site to equal or better condition than before excavation. (12) Tie all vertical controls to furnished datum. If turns or additional benchmarks are required to obtain the tie, the CONSULTANT shall do this. The accuracy of these turns or additional benchmarks shall be in accordance with established surveying practices. (13) Close-out permits are required. (14) All data will be developed and furnished to the FDOT utilizing Computer Aided Drafting and Design (CADD) systems. The FDOT makes available software to help assure quality and conformance with policy and procedures regarding CADD. It is the responsibility of the CONSULTANT to meet the requirement in the Department s CADD MANUAL (Topic No ). The CONSULTANT will submit final documents and files as described therein. (15) Provide the following test-hole information: a. Elevation of top and/or bottom of utility tied to the datum furnished. b. Horizontal location using the distance right or left of indicated center line stationing when available. All survey work required, including the retracing of the survey centerline or baseline, will be determined and performed by the CONSULTANT. c. Outside diameter of pipe or width of duct banks and configuration of nonencased multi-conduit systems. d. Utility structure material composition, when possible. e. Lid diameter and depth as well as box size as applicable. f. Identify benchmarks used to determine elevations. Stage II SOS A-4 FPID No. TBD

7 g. Elevations provided by the CONSULTANT shall be within an accuracy of +0.05' based on the benchmarks shown on the CONSULTANT S certification forms. (16) Provide a weekly summary of work completed. C. Surface Utility Locating Service Surface location services shall be conducted in accordance with the Department's Surveying and Mapping Handbook. (1) Unmanned Aircraft System (UAS) and Unmanned Aerial Vehicle (UAV) All survey measurements collected using manned or unmanned (drone) mobile remote sensing vehicles, require the Surveying and Mapping Consultant be prequalified in Photogrammetric Mapping. The use of any Unmanned Aircraft System (UAS), Unmanned Aerial Vehicle (UAV), drone, or similar system to accomplish contract activities must comply with federal, state, and local laws and regulations. D. Utility Location and Verification Service during Construction The CONSULTANT shall be available for these services during construction if required. Procedures are general for all projects; one or more of the steps may not be necessary depending on the scope of the task work order. E. Utility Coordination The CONSULTANT shall identify utility facilities and secure agreements, utility work schedules, and plans from the Utility Agency Owners (UAO) ensuring all conflicts that exist between utility facilities and the DEPARTMENT s construction project are addressed. The CONSULTANT shall certify all utility negotiations have been completed and that arrangements have been made for utility work to be undertaken. (1) Utility Kickoff Meeting Before any contact with the UAO(s), the CONSULTANT shall meet with the District Utility Office (DUO) to receive guidance, as may be required, to assure that all necessary coordination will be accomplished in accordance with DEPARTMENT procedures. CONSULTANT shall bring a copy of the design project work schedule reflecting utility activities. (2) Identify Existing Utility Agency Owner (s) The Consultant shall identify all utilities within and adjacent to the project limits that may Stage II SOS A-5 FPID No. TBD

8 be impacted by the project. (3) Make Utility Contacts Preliminary Concept Report Notify each utility owner of the upcoming project. Send appropriate letters, straight line diagrams and scope of work to each utility identified within the Design Ticket (and other known utility owners within limits of project). Request utility involvement and contact person for each utility located within project limits. First Contact (Phase I) The CONSULTANT shall prepare and transmit an initial statutory contact package to all utility companies/agencies that may have existing facilities within the project limits. This package shall include two sets of plans (hard copy, disk or electronic files) with the statutory letter. Includes contact by phone for meeting coordination. Request type, size, location, easements, and cost for relocation if reimbursement is claimed. Request the voltage level for power lines in the project area. Send UAO requests for reimbursement to FDOT for a legal opinion. Include the meeting schedule (if applicable) and the design schedule. Include typical meeting agenda. If scheduling a meeting, give approximately 4 to 6 weeks advance notice. Second Contact (Revised Phase II) At a minimum of 4 weeks prior to the meeting, the CONSULTANT shall transmit the second Statutory contact letter with the necessary agreements, and documents to each utility company/agency as required. Two complete sets of plans (hard copy, disk or electronic files) and the utility conflict information (when applicable and in the format requested by the DEPARTMENT) shall be furnished to each UAO having facilities located within the project limits. One plan set will be color coded by the utility company showing proposed relocation and returned to the CONSULTANT with the utility work schedules and agreements as appropriate to be transmitted to the DUA or designee. Third Contact (Revised Phase III) The CONSULTANT shall transmit the third Statutory contact letter to each utility company/agency as required. Two complete sets of plans, including all component sets, (hard copy, disk or electronic files), the utility conflict Information (when applicable and in the format requested by the DEPARTMENT) and List of Plan Changes shall be furnished to each UAO having facilities located within the project limits. Revised plans will be marked by the utility company and returned to the CONSULTANT with revised utility work schedules to be transmitted to the DUA or designee. Stage II SOS A-6 FPID No. TBD

9 Final Contact (Phase IV) Send one set of Phase IV plans (hard copy, disk or electronic files) to each of the involved UAO(s). The UAO(s) should confirm that their facilities are accurately depicted within 5 to 10 days from receipt of the Phase IV Plans. All utility conflict Information must be resolved. Not all projects will have all contacts as described above. (4) Exception Processing The CONSULTANT shall coordinate the processing of design exceptions involving utilities with the UAO and the Department. Coordinate and process per the UAM. The CONSULTANT shall be responsible for transmitting/coordinating the appropriate design reports including, but not limited to, the Resurfacing, Restoration and Rehabilitation (RRR) report, Preliminary Engineering Report, Project Scope and/or the Concept Report (if applicable) to each UAO to identify any condition that may require a Utility Exception. The CONSULTANT shall identify and communicate to the UAO any facilities in conflict with their location or project schedule. The CONSULTANT shall assist with the processing of design exceptions involving Utilities with the UAO and the DEPARTMENT. Assist with processing per the UAM. (5) Preliminary Utility Meeting/Preliminary Drainage Meeting The CONSULTANT shall schedule (time and place), notify participants, and conduct a preliminary utility meeting with all UAO(s) having facilities located within the project limits for the purpose of presenting the proposed drainage for the project, review the current design schedule, evaluate the utility information collected, provide follow-up information on compensable property rights from the FDOT Legal Office, discuss the utility work by highway contractor option with each utility, and discuss any future design issues that may impact utilities. This is also an opportunity for the UAO(s) to present proposed facilities. The CONSULTANT shall keep accurate minutes and distribute a copy to all attendees. (6) Individual/Field Meetings The CONSULTANT shall meet with each UAO as necessary, separately or together, throughout the project design duration to provide guidance in the interpretation of plans, review changes to the plans and schedules, optional clearing and grubbing work, and assist in the development of the UAO(s) plans and work schedules. The CONSULTANT is responsible for motivating the UAO to complete and return the necessary documents after each Utility Contact or Meeting. (7) Collect and Review Plans and Data from UAO(s) The CONSULTANT shall review utility marked plans and data individually as they Stage II SOS A-7 FPID No. TBD

10 are received for content. Ensure information from the UAO (utility type, material and size) is sent to the designer for inclusion in the plans. Forward all requests for utility reimbursement and supporting documentation to the DUO. (8) Subordination of Easements Coordination The District Right-of-Way Office will handle processing of all subordinations of easements. The Consultant shall refer all UAOs to the DUO to address subordinations of easements when they arise. (9) Utility Design Meeting The CONSULTANT shall schedule (time and place), notify participants, and conduct a Utility meeting with all affected UAO(s). The CONSULTANT shall be prepared to discuss impacts to existing trees/landscaping and proposed landscaping, drainage, traffic signalization, maintenance of traffic (construction phasing), review the current design schedule and letting date, evaluate the utility information collected, provide follow-up information on compensable property rights from FDOT Legal Office, discuss with each UAO the utility work by highway contractor option, discuss any future design issues that may impact utilities, etc., to the extent that they may have an effect on existing or proposed utility facilities with particular emphasis on drainage and maintenance of traffic with each UAO. The intent of this meeting shall be to assist the UAOs in identifying and resolving conflicts between utilities and proposed construction before completion of the plans, including utility adjustment details. Also, to work with the UAOs to recommend potential resolution between known utility conflicts with proposed construction plans as may be deemed practical by the UAO. The CONSULTANT shall keep accurate minutes of all meetings and distribute a copy to all attendees within 3 days. (10) Review Utility Markups & Work Schedules and Processing of Schedules & Agreements The CONSULTANT shall review utility marked up plans and work schedules as they are received for content and coordinate review with the designer. Send color markups and schedules to the appropriate DEPARTMENT office(s) such as construction, survey, geotechnical, drainage, structures, lighting, roadway, signals, utilities, landscape architecture, municipalities, maintaining agency, and District Traffic Operations for review and comment if required by the District. Coordinate with the District for execution. Distribute Executed Final Documents. Prepare Work Order for UAO(s). The CONSULTANT shall coordinate with the DUO the programming of necessary Work Program funds. (11) Utility Coordination/Follow-up The CONSULTANT shall provide utility coordination and follow up. This includes follow-up, interpreting plans, and assisting the UAOs with completion of their work schedules and agreements. Includes phone calls, face-to-face meetings, etc., to motivate and ensure the UAO(s) complete and return the required documents in accordance with the project schedule. Ensure the resolution of all known conflicts. The CONSULTANT Stage II SOS A-8 FPID No. TBD

11 shall keep accurate minutes of all meetings and distribute a copy to all attendees. This task can be applied to all phases of the project. (12) Utility Constructability Review The CONSULTANT shall review utility schedules against construction contract time, and phasing for compatibility. Coordinate with and obtain written concurrence from the construction office. See Task 4.9 (Cross Section Design Files) for utility conflict identification and adjustments. (13) Additional Utility Services The CONSULTANT shall provide additional utility services. Additional services will be determined when the services are required and requested. This item is not usually included in the scope at the time of negotiation. It is normally added as a supplemental agreement when the need is identified. (14) Processing Utility Work by Highway Contractor (UWHC) This includes coordination of utility design effort between the DEPARTMENT and the UAO(s). The CONSULTANT shall conduct additional coordination meetings, prepare and process the agreements, review tabulation of quantities, perform UWHC constructability and biddability review, review pay items, cost estimates and Technical Special Provisions (TSP) prepared by the UAO. This does not include the utility design effort (15)Contract Plans to UAO(s) If requested by the District, the CONSULTANT shall transmit the contract plans as processed for letting to the UAO(s). Transmittals to UAO(s) may be by certified mail, return receipt requested. (16) Certification/Close-Out This includes hours for transmitting utility files to the DUO and preparation of the Utility Certification Letter. The CONSULTANT shall certify to the appropriate DEPARTMENT representative the following: All utility negotiations (Full execution of each agreement, approved Utility Work Schedules, technical special provisions written, etc.) have been completed with arrangements made for utility work to be undertaken and completed as required for proper coordination with the physical construction schedule. OR An on-site inspection was made and no utility work will be involved. OR Stage II SOS A-9 FPID No. TBD

12 Plans were sent to the Utility Companies/Agencies and no utility work is required. (17) Other Utilities The CONSULTANT shall provide other utility services. This includes all efforts for a utility task not covered by an existing defined task. Required work will be defined in the scope and negotiated on a case-by-case basis. F. Scheduling of Activities The DEPARTMENT will establish appropriate deadlines for each phase or activity on an individual project basis. The CONSULTANT shall be responsible for completing each phase or activity within the established time frames. G. Review of Work A monthly Project Management Report (Status Sheet) will be submitted to the DUO on projects assigned to the CONSULTANT. The status sheet shall be updated as each utility s information is received and reflect a date that each phase or activity is expected to be finished. H. Review and Acceptance The CONSULTANT shall be responsible for making all necessary reviews and acceptance of utility related materials. I. General - Final Utility Negotiation Material Packet At the time of certification, the CONSULTANT shall submit to the DUA all correspondence, plans, work schedules, agreements, etc. and on a CD/DVD. The CONSULTANT shall maintain a filing system on each project in a format similar to the DUO and be made available for inspection by the DUA. J. Staking of Right of Way (1) Use the Proposed Construction Plans and the associated Right of Way Maps to recover sufficient project control to check the plan alignment. (2) Using the plan control points and the respective Right of Way Maps, stake the Right of Way at intervals necessary to assure an accurate location of the line and to facilitate the Utility Location/Re-Location work. All surveying activities shall be accomplished in accordance with the Surveying and Right of Way Mapping Handbooks, and any special instructions contained in the Task Work Order Scope of Services. 13 III. COMMENCEMENT OF WORK AND TIME OF COMPLETION: A. Beginning and Length of Services Services shall commence upon execution of this Agreement. Task work orders shall be assigned for a period of Sixty (60) months from the date of this agreement or until the total Maximum Limiting Amount is reached. B. Task Work Order The Department shall furnish Individual Task Work Orders which shall identify the assigned project and/or Task Work Orders to the CONSULTANT which shall specify the services to be performed and state the compensation for the services. The Task Work Orders shall be executed by the Department s DUA or Project Manager. C. Time to Complete Work All work under this contract shall be completed and delivered to the Department as specified by the Department's Project Manager and agreed upon by both parties. IV. ERRORS: The CONSULTANT shall check the designating and/or locating services for accuracy before submission of data. The CONSULTANT will be required, without additional compensation, to correct any errors, including but not limited to omissions, discrepancies and ambiguities in the work, which may be discovered before or during construction. Acceptance of the right-of-way staking, designating or locating service by the Department shall not relieve the CONSULTANT of the responsibility of subsequent correction of errors. V. MAINTENANCE OF RECORDS: The CONSULTANT shall maintain all books, documents, papers, accounting records and other evidence supporting the cost incurred and shall make such materials available at its offices at all reasonable times during the Agreement period and for three (5) years from the date of final payment under the Agreement for inspection by the Department. VI. CONSULTANT STAFF: The control and supervision of all phases of the work performed for this Agreement by the CONSULTANT shall be under the direction of an Engineer, underground utility locating or designating specialist employed by the CONSULTANT who has not had less than three (3) years experience on the type of work herein described and shall be assigned to the project until all work has been completed or until the Department agrees in writing that he may be replaced or removed. A staff of competent engineers or specialists adequate in number and experience to perform the described work in the prescribed time shall be assigned to perform work under this Agreement. All work shall be performed by or under the supervision of a surveyor or professional engineer who is licensed in Florida. Stage II SOS A-11 FPID No. TBD

14 The CONSULTANT S work shall be performed by the key personnel identified in the CONSULTANT S Letter of Response. Any change in the indicated personnel or the CONSULTANT S office in charge of work as identified in the CONSULTANT S Letter of Response shall be subject to review and approval by the DEPARTMENT. The CONSULTANT is responsible to certify that all necessary arrangements for utility work on a project have been made and will not conflict with the physical construction schedule. The CONSULTANT should coordinate with DEPARTMENT personnel to coordinate transmittals to Utility Companies and meet production schedules. The CONSULTANT shall ensure FDOT standards, policies, procedures, practices, and design criteria are followed concerning utility coordination. The CONSULTANT may employ more than one individual or utility engineering CONSULTANT to provide utility coordination and engineering design expertise. The CONSULTANT shall identify a dedicated person responsible for managing all utility coordination activities. This person shall be contractually referred to as the Utility Coordination Manager and shall be identified in the CONSULTANT proposal. The Utility Coordination Manager shall be required to satisfactorily demonstrate to the FDOT District Utility Administrator that they have the following knowledge, skills, and expertise: A minimum of 4 years of experience performing utility coordination in accordance with FDOT, Federal Highway Administration (FHWA), and American Association of State Highway and Transportation Officials (AASHTO) standards, policies, and procedures. A thorough knowledge of the FDOT plans production process and District utility coordination process. A thorough knowledge of FDOT agreements, standards, policies, and procedures. The Utility Coordination Manager shall be responsible for managing all utility coordination, including the following: Assuring that Utility Coordination and accommodation is in accordance to the FDOT, FHWA, and AASHTO standards, policies, procedures, and design criteria. Assisting the engineer of record in identifying all existing utilities and coordinating any new installations. Assisting the Engineer of Record with resolving utility conflicts. Scheduling and performing utility coordination meetings, keeping and distribution of minutes/action items of all utility meetings, and ensuring expedient follow-up on all unresolved issues. Distributing all plans, conflict matrixes and changes to affected utility owners and making sure this information is properly coordinated and documented. Identifying and coordinating the completion of any FDOT or utility owner agreement Stage II SOS A-12 FPID No. TBD

15 that is required for reimbursement, or accommodation of the utility facilities associated with the project. Review and certify to the District Utilities Administrator that all Utility Work Schedules are correct and in accordance with the Department s standards, policies, and procedures. Prepare, review and process all utility related reimbursable paperwork inclusive of betterment and salvage determination. The CONSULTANT S utility coordination work shall be performed and directed by the identified Utility Coordination Manager that was identified and approved by the DUA. Any proposed change, of the approved Utility Coordination Manager, shall be subject to review and approval by the DUA prior to any change being made in the contract. The CONSULTANT firm shall have the ability to provide the DUO with on-site support personnel on an as-needed basis. Provide or assist the DUA in training utility staff and utility agency owners (UAO s) as needed; inclusive of on-site and off-site training. VII. CONFERENCES AND MEETINGS: The initial conference shall be called by the Department at a place and time selected by the Department for the purpose of reviewing the CONSULTANT S work program, schedules, procedures, methods and the clarification of any ambiguities that may then exist and shall be attended by the principal CONSULTANT or a partner of the CONSULTANT and the CONSULTANT S Project Engineer. Conferences will be called no more frequently than one each month for the duration of this Agreement at a place and time fixed by the Department. Such periodic conferences will be attended by representatives of the Department, the CONSULTANT S Project Engineer and such other project personnel of the CONSULTANT as may be required for the purpose of reviewing the CONSULTANT S progress and such details of the project that may appear to require examination. VIII. EXPERT TECHNICAL WITNESS: In the event that the testimony of the CONSULTANT is required in any legal proceeding in which the CONSULTANT is not a party in connection with claims brought against or prosecuted by the Department, the CONSULTANT agrees to appear as a witness on behalf of the Department. A fee that is considered fair and reasonable will be negotiated at the time services are requested. IX. SUBCONTRACTING: Firms selected for subcontracts must be approved and qualified by the Department prior to the CONSULTANT authorizing any such work. 16 the satisfactory performance of all subcontracted work. X. DEPARTMENT RESPONSIBILITIES: The Florida Department of Transportation will furnish any or all of the following items as appropriate, for performance of the required services: A. Provide highway plans showing the alignment, profile and benchmark data and project limits of the selected projects, if available. B. Provide highway plans with the locations of the required test holes indicated. C. All previously completed field surveys as required. D. All available roadway plans, bridge plans, right-of-way maps, studies and other available information pertinent to the project. E. Right-of-Way maps and legal descriptions that are not part of the CONSULTANT S work effort, when required. F. When survey services are required, the Department will furnish numbered standard survey books for survey data. G. Pavement Design where necessary. H. Coordination and processing of all permit applications. I. The DEPARTMENT s DUO shall make available all necessary agreements, utility work schedules and other documents required to clear utility negotiations on each project. J. Projects plans, which shall include, but not be limited to, initial review set, multiple utility sets and plans required for preparation of special agreements, if available. K. The DEPARTMENT s DUA or his designee (Project Manager) shall be the single point of contact for the Department of Transportation for this Contract. L. While it is expected that the CONSULTANT shall need to receive advice from various state, regional and local agencies, the final direction on all matters of a technical nature will remain with the DUA. However, all matters pertaining to contract administration lie with the District Professional Services Administrator. M. All correspondence on file that is determined by the DEPARTMENT to be relevant to the utility negotiations. XI. GENERAL: A. The CONSULTANT S submittal shall be accurate, legible and in accordance with the Department s acceptable standards. 17 CONSULTANT shall refer to the current editions of the DEPARTMENT s Plan Preparation Manual, Standard Specifications for Road and Bridge Construction, Utility Accommodation Manual (UAM) and other applicable manuals as determined by the DEPARTMENT. B. It shall be the CONSULTANT S responsibility to acquire and utilize the necessary DEPARTMENT manuals that apply to the completion of utility negotiations on each project. C. The CONSULTANT shall also prepare and distribute minutes of all meetings the CONSULTANT attends relative to the project, including the contract kick off meeting. D. The CONSULTANT shall submit a staff-hour and fee estimate proposal for each Task Work Order within three (3) working days of receipt of our request for same. XII. QUALITY ASSURANCE: In order to assure that the Department will be provided with accurate, high quality designating and locating results and reports, the CONSULTANT is required to establish a Quality Control process for reviewing all work efforts prior to submittal to the Department.
