3 Rules and Regulations governing Usage of Lift for Delivery / Transportation of Bulky Equipment / Items 1. Usage of Lift for the respective block for the delivery / transportation of equipment / items is restricted to the following hours: Mondays to Fridays : 9.00 am pm Saturdays : 9.00 am pm Sundays and Public Holidays (PH) : Strictly no delivery / Transportation activity allowed 2. Refer to the Management Office for dimensions of Fire Lifts Interiors 3. Booking of lift for exclusive use is NOT allowed. 4. Any request for the purpose of delivery / transportation of equipment / Items shall be made in the Application shall be made on the prescribed forms, namely, Form 8 - Application for Delivery / House-Moving and Form 9 - Application for Use of Lift and submitted to the Management for approval at least three (3) working days in advance by the Contractors. 5. The maximum time allowed for request to use the lift for purpose of delivery / transportation of equipment / items shall be two (2) hours per day per unit. 6. All requests shall be on first-come-first-served basis. 7. The contractor shall ensure that there is no obstruction to the staircases, walkway, lift lobby and / or any other areas while delivering / loading / unloading. 8. The contractor shall reinstate all damages caused to the building and / or Common Property during the course of delivery / loading / unloading. 9. The contractors shall only use designated lift (lift with Perspex, canvas or plywood protection) to transport their bulky equipment / items. 10. In the event if the protection is dismantled, the contractor shall provide his own material to protect the lift components adequately and to the satisfactory of the Management. 11. The contractor shall NOT halt the lift by inserting a stopper between the lift doors. 12. The contractors shall ensure that the lift is not overloaded when transporting their bulky equipment / items. 4 FORM 10 JOINT INDEMNITY FORM FOR SUBSIDIARY PROPRIETOR AND CONTRACTOR FOR DELIVERY / HOUSE-MOVING : To : MCST Plan No Bishan Street 15 #B1-01 Singapore Dear Sir / Madam I wish to inform you that we would like to carry out Delivery and House-Moving (Moving-In And Moving-Out) in the above premises. Enclosed herewith is my cheque of S$1,000 being the refundable security deposit required to be placed with the Management. I understand that this deposit will be refunded to me without interest upon the completion of my provided always that the Management is entitled to deduct any amount in accordance with the Rules and Regulations contained in the Resident s Guide. I shall ensure that my contractors comply with the following: 1) To adhere the Rules and Regulations governing Delivery / House-Moving of the Residents Guide; 2) Appropriate measures must also be taken to ensure safety of the Delivery / House- Moving of the Residents Guide; 3) To ensure the cleanliness of the Common Property and to remove and cart away waste materials and debris on a daily basis until the completion of works, and/ or as and when directed by the Management; 4) To protect the lobby floors, wall finishes carpets and lifts against damages when transporting materials and/ or debris; I undertake to indemnify the Owner Developer, MCST Plan No. 4291, the appointed Managing Agent and the Management Corporation (upon its constitution) against all loss and damages to the above premises, the building and all property herein caused by my contractors and/ or their workmen. I further undertake to reimburse MCST Plan No. 4291and/ or the appointed Managing Agent and/ or the Management Corporation (upon its constitution) for all costs involved in removing waste materials and debris arising from my works if they are not removed by my contractors, failing which the costs involved is to be offset from my deposit. I understand that it is my sole responsibility to consult, liaise directly and/ or obtain the necessary approvals from the relevant authorities and/ or my own Qualified Person before submitting my application to the Management.
