The sum of the concentrations of lead, mercury, cadmium and hexavalent chromium that can be found in any package or packaging components, including inks or labels: < or = 100 ppm (0.01%). For recycled materials, the sum of the concentrations of lead, mercury, cadmium and hexavalent chromium that can be found in any package or packaging components, including inks or labels, shall not exceed 200 ppm (0.02%). As applicable to consumer commodities: 1) Statement of identity. 2) Name and place of business of manufacturer, packer or distributor. Place of business shall include street address, city, state, and zip code; however, the street address may be omitted if it is listed in a readily accessible, widely published, and publicly available resource, including but not limited to a printed directory, electronic database, or Web site. (Effective Dec. 17, 2015.) 3) Net quantity of the contents in terms of weight, measure, or numerical count. If bag thickness is <1 mil, in order to establish compliance (pass/fail), the bag's dimensions need to be considered in conjunction with the labeling specifications of the individual jurisdictions as follows: CALIFORNIA - You must label if: 1) Thickness is an average less than one mil, and 2) Size of opening is larger than 25 square inches or a capacity of more than 125 cubic inches. CHICAGO, IL - requirement is for polyethylene bag, other than one used for food products weighing not more than five pounds, intended for household. MASSACHUSETTS & NEW YORK - You must label if: 1) Thickness is less than one mil, and 2) Size of opening has a diameter of seven inches or more, and 3) Length and width when added together totals 25 inches or more. RHODE ISLAND - You must label if: 1) Thickness is less than one mil, and 2) Size of opening is larger than five inches in diameter. VIRGINIA - You must label dry cleaning bags if: 1) Thickness is less than one mil, and 2) Length totals twenty-five inches or more. CALIFORNIA & RHODE ISLAND - Total length and width of: <30 inches = 10 point type minimum = or > 30 inches and < 40 inches = 14 point type minimum = or > 40 inches and < 60 inches = 18 point type minimum = or > 60 inches = 24 point type minimum. MASSACHUSETTS - Total length and width of: = or > 25 inches and < 40 inches = 14 point type minimum = or > 40 inches and < 60 inches = 18 point type minimum = or > 60 inches = 24 point type minimum. Label shall appear on both sides of bag. If total length and total width is more than 40 inches (when added), label shall be repeated at 20 inch intervals. NEW YORK - Total length and width of: = or > 25 inches and < 30 inches = 10 point type minimum = or > 30 inches and < 40 inches = 14 point type minimum = or > 40 inches and < 60 inches = 18 point type minimum = or > 60 inches = 24 point type minimum. VIRGINIA - 36 point type minimum. Plastic bags in California, Massachusetts, New York, Rhode Island, and Virginia require the following warning: "WARNING: To avoid danger of suffocation, keep this plastic bag away from babies and children. Do not use this bag in cribs, beds, carriages or play pens. This bag is not a toy." If the warning is required based on thickness and opening size, but not present, then the test shall be reported as fail. If a warning is present, report the warning listed on the bag. Point of Purchase documentation shall meet language and clarity requirements. The product shall have current the certification of listing by the appropriate regulatory agency in the country of destination (e.g. UL, ETL, CSA or IAPMO). The product shall have a current certificate of conformance to product standards from a source independent of the manufacturing operations. The presence and authenticity of the UL issued holographic label is verified. Physical Characteristics USB Port 1 Report if sample includes a USB port. The result of this test is reported for reference only; the result is not rated. Quality & Workmanship Enclosure 1 The sample shall have an enclosure to prevent the user from making contact with live electrical and moving parts. Parts Inventory Test UL-TS parts and components required to complete the assembly and to operate the sample as intended must be present. Sharp Edges Test 16 CFR (d) Citation: Federal Hazardous Substances Act Modifications: 1) As-received condition only. 2) Because this test is for items which fall outside the scope and age grade (under 8 years of age) of this section of the CFR, the result of this test is reported for reference only; the result is not rated. Sample is assessed in as-received condition only, and any nonfunctional hazardous sharp edges are noted.

Sample is assessed in as-received condition only, and any nonfunctional hazardous sharp points are noted. Workmanship Sample shall be free of major cosmetic defects which may impact customer satisfaction with the product. Sample is checked for workmanship in "as received" condition. UL 1642: NRTL Certification Battery Certification UL 1642 If product uses Lithium batteries, the batteries must be certified for compliance to UL Certification must have been done by a Nationally Recognized Testing Laboratory (NRTL) for this standard. NOTE: Test report shall be provided. UL 2054: NRTL Certification Battery Pack Certification UL 2054 Battery pack must be certified for compliance to UL Certification must have been done by a Nationally Recognized Testing Laboratory (NRTL) for this standard. NOTE: Test report shall be provided. Batteries: Additional Testing as Required Battery Insulation 1 Batteries and their terminals shall be adequately insulated, and arranged to prevent the risk of electric shock and short-circuiting. States Public Law , 110 Stat & State Legislation Written Confirmation Citation: 47 CFR 15.19, 47 CFR & 47 CFR 18 1) For 47 CFR Part 15 devices in which the highest frequency generated and the highest frequency used are greater than 1.705MHz shall submit objective evidence in the form of the following: a) FCC Declaration of Conformity mark or FCC ID on the product for CB receiver, Superregenerative receiver, TV interface device, Cable system terminal device, Class B personal computers and peripherals, CPU boards and internal power supplies used with Class B personal computers, or Class B personal computers assembled using authorized CPU boards or power supplies. The DoC label must be designed to last the expected lifetime of the equipment in the environment in which the equipment may be operated and must not be readily detachable. NOTE: Devices that digital devices in which both the highest frequency generated and the highest frequency used are less than MHz and which do not operate from the AC power lines or contain provisions for operation while connected to the AC power lines are exempt for the technical requirements for CFR47 Part 15. 2) For 47 CFR Part 18 devices that operate at or above 9kHz and are designed to generate and use locally RF energy for industrial, scientific, medical, domestic or similar purposes, excluding applications in the field of telecommunication shall submit objective evidence in the form of the following: a) Consumer ISM equipment shall be marked with the FCC Declaration of Conformity mark or FCC ID. The DoC label must be designed to last the expected lifetime of the equipment in the environment in which the equipment may be operated and must not be readily detachable. If the FCC ID is affixed to the label then the FCC ID number shall be verified at b) Equipment authorized under the Declaration of Conformity shall include the following compliance information (Note: The compliance information may be placed in the instruction manual, on a separate sheet, or on the packaging. There is no specific format for this information). i) Identification of the product, e.g., name and model number. ii) A statement similar to the following: This device complies with Part 18 of the FCC Rules. iii) The name and address of the responsible party. This party must be located within the United States. c) For all Equipment information on the following matters shall be provided to the user in the instruction manual or on the packaging if an instruction manual is not provided for any type of ISM equipment: i) The interference potential of the device or system. ii) Maintenance of the system iii) Simple measures that can be taken by the user to correct interference. iv) Manufacturers of RF lighting devices must provide an advisory statement, either on the product packaging or with other user documentation, similar to the following: This product may cause interference to radio equipment and should not be installed near maritime safety communications equipment or other critical navigation or communication equipment operating between MHz. Variations of this language are permitted provided all the points of the statement are addressed and may be presented in any legible font or text style. NOTE: Client shall submit test report or certificate of conformity. Regulated batteries and rechargeable consumer products without easily removable batteries shall be labeled in accordance with the regulation. Samples with Case: Additional Testing as Required Fit Test: Functional 1 The item shall fit as intended. Note: Sample shall meet, at a minimum, Grade Colorfastness rating of colored textiles shall meet the criteria as specified. After testing, a comparison with the AATCC Gray Scales for Color Change and Staining is performed and a rated is assigned to the sample.
