5 Enter your login credentials and click Login to continue. Username: Eastern Password: Eastern Next, select the site from which this order will be placed. Click Continue to proceed

6 Next, select which department to which the order applies. Click Continue to proceed The hospital and department selections help you to determine which cost centre is applicable for billing purposes and which consignment locations are available.

7 The order form consists of two main sections: The Order Details section, & the Equipment Selection table.

8 Order Details Section First, select the applicable cost centre from the dropdown list Then, complete the clients details including their contact person or next of kin. Note that fields marked with an (*) are mandatory Now complete your contact details. Selecting a contact preference will allow our staff to contact you by your preferred method. If equipment is taken from consignment, please select a consignment location. Note the comments box is used to communicate order specific details or requirements. For example: Please adjust chair height to 55cm Notes form a valuable part of the communication process. An alternate delivery address can be entered if the client is not at their listed residential address.

9 Equipment Selection Table Equipment selection is specific to the department selected at the beginning of the form process You can select equipment for either hire or purchase based on the requirement. The equipment quantity can be selected if multiple items are required. The equipment asset number must be entered if equipment is taken from a consignment location. This process ensures up-to-date and accurate stock levels are maintained at all times.

10 Once completed, select the Next button at the top of the form. A new page will open allowing you to review your order details and equipment selected. Changes? Select Back to return to the order page. Confirmed? Select Send Order Request A receipt confirmation screen will appear, while a copy is sent to the address provided From your PDF print a copy
