1 1 Log on to the English Language Proficiency Assessments for California (ELPAC) Test Operations Management System (TOMS) and select the [Orders] tab from the menu on the left side of the Web page. 2 Select the [Place Suppl. Orders] tab. Released January 2018 Page 1

2 3 Select [Select Organization] under Select LEA (i.e., local educational agency [LEA]). Type at least five characters of the organization s name (including spaces) or county/ district/school (CDS) code to retrieve selectable values. Select a test administration, testing program, and test form type from the respective drop-down lists. 4 Select the [CONTINUE] button. 5 Confirm the LEA s primary shipping address in the confirmation pop-up message. Selecting [EDIT] opens the [Edit] tab in the View and Edit Organizations Web form, where you can edit the primary shipping address. If you choose this option, you will have to repeat step 2 to continue with your supplemental order; or, select the [CONFIRM] button to continue. Released January 2018 Page 2

3 6 Select the specific quantity for the grade(s) and type of test form that you want to order. You can order the exact number of Test Books and Answer Books as the total will not round up or include any overage. Note: Kindergarten, grade one, and grade two use a combined Test Book and Answer Book. Grades three through twelve use a separate Test Book and Answer Book. When placing a materials order for grades three through twelve, an Answer Book should be ordered for each Test Book. 4 7 Select [CONTINUE] and review your order. Take these steps if you need to change your order for materials prior to selecting the [Submit Order] button: 1. Select the [EDIT ORDER] button. 2. Make your updates. 3. Select the [CONTINUE] button. 4. Review your order. Released January 2018 Page 4

5 8 Once you have reviewed your order, select the [SUBMIT ORDER] button. You will receive a pop-up message confirmation that your order has been placed. Important Note: Included in the Site Coordinator Kit are Return Instructions, the Group Identification Sheet (GIS), paper bands to wrap around answer documents, and tape for sealing boxes to be returned to ETS. 6 Not included in the Site or LEA Coordinator Kit are white cartons for returning scannable materials and United Parcel Service labels for scannable and non-scannable materials return. The ELPAC Customer Support Center will create an e-label (UPS electronic label) for the LEA. The label will be printed and shipped with the carton(s) to the LEA ELPAC Coordinator s attention. The brown cartons from the original test materials shipment are to be retained for the return shipment of the non-scannable materials.
