1 DRIVE CLEAN ACCREDITATION CHECKLIST How to Become a Light Duty Test Only, Test & Repair, Repair Only, Heavy Duty Non-Diesel, or Heavy Duty Diesel (Mobile or Non-Mobile) Drive Clean Facility Important Complete each step in order. Do not begin the next step until the preceding step is completed. If possible set aside some time (approximately 1/2 1 hour) in order to complete. Use this document as a guide to help with preparing the information you will need to provide. Do not invest in or obtain emissions testing equipment or training of staff until your Application has been reviewed and Approved by the Drive Clean Office (Step 5). The Drive Clean Office does not allow the use of the words Drive Clean in facility Legal and or Business Names. Drive Clean is an official mark of the Province of Ontario. This application process is for the New Modernized Drive Clean Program that uses the new OBD II and/or TSI, and Heavy Duty test equipment. Do not post signage indicating affiliation with the Drive Clean Program or emissions testing until a Facility Audit has been completed and Accreditation has been granted (Step 8). Failure to adhere to this may affect accreditation approval. In order to become a Drive Clean Facility you must first log on to and create a unique log on and user profile of your company. The Drive Clean Program is now modernized and has provided an easy and convenient online application process. It is very important to be complete and accurate when filling out the online forms. This information will be used in all of the Drive Clean Information Systems and in future correspondence to you. It is strongly recommended that you have these important documents and/or pieces of information before you start the application process on line; 1. Articles of Incorporation (if applicable if your business is incorporated) 2. Business Name Registration and/or Master Business Licence 3. Copy of current business insurance (the limits and additional insured do not matter at this point). 4. Photographs of your proposed Drive Clean Facility (for your reference, samples are provided in the Drive Clean Facility Layout document). Hard copy or electronic copy (faxed) photos are acceptable but must be legible, digitally uploaded photos are recommended. 5. Simplified floor plan and site diagram that detail your facility layout (for your reference, samples are provided in the Drive Clean Facility Layout document). These items will need to be scanned and uploaded later on in the process. If you do not have the ability to scan and upload documents you will be able to , fax, or mail them. 2 Step 1 Create user profile and unique log on to access Drive Clean Start by logging into 1. Once web page has been displayed, select create new user, 2. You will be prompted to enter your first and last name and a valid address. When you have submitted the required information a confirmation will be sent to the address you submitted The confirmation will identify your user ID and a temporary one time user password, along with a web link that will take you back to the website: When you click that link you will be asked for your user ID and password, once entered you are required to change the temporary password, Successfully entering the new password following the rules outlined will then take you to the Drive Clean Facility Web Portal page where you can select the option of Applying to be Drive Clean Facility Owner or you may exit and return at a later date. Step 2 Create Owner Information If you are continuing immediately from step 1 above, or if you have signed back in from previously enrolling you will be prompted to provide some more specific owner information. You are required to fill in information such as a telephone number, cell phone number etc. Please do not leave any information fields blank. Please be advised that the new online application process shows your progress at each and every step, it will not allow you to continue until all required fields are filled out, or allow you to skip any steps. Step 3 Provide Facility Information This part of the online application process will require the entry of information regarding your specific facility. Please follow all screen instructions and make sure that the entered information is accurate and complete. There will be an automated mapping application that will help you to confirm the address was entered correctly. This step will not allow you to proceed with missing information; you will have to ensure all required information is entered. It is strongly recommended that you use the information contained on your Master Business License to help you fill out the online form. You will also be required to provide your banking information as this is required to set up the Pre-Authorized Debits needed for the tests fees as well as the monthly maintenance fees. There is also a supporting document section that will require you to upload electronic copies of documents, photos and diagrams described on page 1. Step 4 Declarations The declarations step will require the user to read 6 statements and agree to the statements by clicking in the check box. 1. You will be required to confirm and declare information submitted, 2. The insurance requirements are detailed and you are asked to agree to maintaining the insurance requirements, 3. Then you are asked to declare that your taxes are up to date and consent to a verification check, a. Click the tax declaration link to open, this will need to be electronically signed by you, 4. The next declaration will ask you to verify that you selected the correct type of facility that matches the services you provide. You will then be directed to electronically sign the Performance Contract and Application. a. By clicking on the link(s) the person with signing authority will electronically sign the Performance Contract(s) and/or agreement(s), Accreditation Checklist - March 2015 Page 2 of 5

3 5. Another declaration statement will need to be checked off regarding the term of the Performance Contract 6. The last statement is informing you of the accreditation fee if your application is approved. Once the declaration steps are complete you will be required to click the submit button. All of the information that you entered will now be submitted. The application process will be temporarily stopped until the application review is complete. The individual(s) signing (or electronically signing) these documents must be authorized signing officer(s) of the business applying for accreditation. Step 5 Application Review Status The Application Review stage is where Parsons Canada Ltd. first previews the submitted information for completeness and then accepts submission for the Drive Clean Office. The Drive Clean Office then performs the necessary verification checks. Tax compliance, Company and/or Business information, verification of correct facility type and Company/Business/Personnel backgrounds will be checked. The status of these verification steps will be shown on the Status screen. If the overall result of this step is approval, a notification is sent to you. Step 6 Readiness Status If you have been electronically notified and received the you will be required to sign back into the web portal with your user ID and password. When you have re-entered, the portal will show your notification and link you right back in the application process to the Readiness step. The first 4 items shown are automatically updated by the system once it is known that your facility has these items, for instance, an accredited Drive Clean Inspector, an accredited Drive Clean Repair Technician, Drive Clean Orientation training and the purchase of the approved Ontario Test Unit. If these items have not been obtained by your facility, there will be an electronic link beside the item required that will take you to the appropriate section(s) in order to update or add the corresponding item(s). The details of the Training and the Ontario Test Unit requirements are found when selecting the links. Obtain Emission Testing and/or Repair Only Equipment Purchase testing equipment. Staff Training Following Application Approval (Step 5), complete the Training Application forms online as soon as possible and enroll your staff in required Drive Clean training courses. There are three types of training courses: Orientation Training, Inspector Training, and Repair Technician Training. All DCFs Customer Service Representatives (CSRs) or other personnel who interact with the public on Drive Clean business are required to have Orientation certification. Light Duty Repair Only DCFs are only required to have a Certified Repair Technician. Test Only and Test & Repair DCFs At least one certified Inspector must be available onsite to conduct emission inspections during all hours the facility is open to the public as posted on the front door or window of the DCF. Repair Only and Test & Repair DCFs In order to provide customer service to the public, all Repair Only and Test & Repair DCFs are required to: (i) Post the facility s normal business hours; Accreditation Checklist - March 2015 Page 3 of 5

4 (ii) Post the hours that their certified Repair Technician is present; and (iii) Have at least one certified Repair Technician working at least 40 hours per week, within the facility s posted normal business hours. If the facility s normal business hours are less than 40 hours per week, the Certified Repair Technician must be available on site for these hours. (iv) A Repair Technician that is on an on-call basis is not considered to be available onsite at the facility. (v) A Repair Technician employed at different DCFs cannot be shared between DCFs if that technician is the only Repair Technician at either DCF. Obtain an Internet Service Provider Make sure that you meet the internet service provider requirements outlined in the Drive Clean Facility Requirements document included with your application package (Section 3). The other items shown in the Readiness section are items you must declare that you have or your facility meets. The descriptions of these items are shown and are based upon facility type. These are the requirements that your facility must meet throughout the term of the program. These items will be verified during Readiness. If it is found that you do not meet these requirements the Accreditation Audit will not take place and it will delay the process until the situation is corrected. 1. Facilities must comply with all Federal, Provincial, and Municipal laws and regulations, 2. Facilities must meet insurance requirements as set out in the Standard Operating Procedures (SOP) 3. Facilities must be able to meet the Environmental Controls of their facility as set out in the SOP 4. Facilities must meet the minimum requirements for the test bay as set out in the SOP 5. Repair Only and Test and Repair facilities must meet the Diagnostic and Repair equipment requirements as set out in the SOP 6. Repair Only and Test and Repair facilities must meet the Diagnostic reference material requirements stated in the SOP 7. Facilities must meet the communication requirements for their Ontario Test Units and Repair Only Units as set out in the SOP 8. Facilities must meet the requirements for customer waiting areas as described in the SOP 9. All facilities must maintain a file/record storage area as set out in the SOP 10. Facilities must have/maintain all Program Documents and brochures as set out in the SOP. Step 7 Accreditation Status Once Parsons Canada Ltd. has verified that your facility has met all of the requirements of the Readiness items listed above, your facility will be contacted in order to arrange an onsite Equipment Installation/Initial Accreditation Audit. It is at this point that the Pre-Authorized Debt for payment of the Vehicle Emissions Test Certificates is set up and your facility is prepared and ready for the site visit. The accreditation audit fee will also be required prior to the Equipment Installation/Initial Accreditation Audit. For Test & Repair, Test Only Facilities, Heavy Duty Mobile or Heavy Duty Non-Mobile the following payment is required: payment of the initial Accreditation fee, please include a certified cheque/money order in the amount of $ for each proposed Ontario Test Unit, payable to the Parsons Canada Ltd., and for a Repair Only Facility, only one payment is required: As payment of the initial Accreditation fee, please include a certified cheque/money order in the amount of $100, payable to the Parsons Canada Ltd. 5 A successful Equipment Installation/Initial Accreditation Audit will automatically notify the Director of the Drive Clean Office that your facility has passed and is requiring final approval. Once signed off your facility is considered open, and from that point onward allow you to provide Drive Clean services to customers. The Auditor will ensure that all of the required Program Materials, signs, and posters are dropped off. If the Equipment Installation/Initial Accreditation Audit is unsuccessful the Auditor will explain and provide documentation that states the reason(s). Once the deficiency(s) has been corrected you will be directed to contact Parsons Canada Ltd. in order to arrange a revisit in order for the Auditor to return. Accreditation When you pass the initial audit, Drive Clean auditors will provide you with official signage. The location of your accredited facility will be posted on the Drive Clean Website and will be available through the public Call Centre. You may then begin advertising in accordance with the Directive on use of the Drive Clean Official Marks. Full accreditation will be complete when at least one of your staff for the type of facility selected has performed the Hands On Training (ie. Test and Repair OBD facility at least1 inspector and 1 repair tech needs to perform Hands On Training), and the Director of the Drive Clean Office has signed off on the final approval. Step 8 - Begin Operating as an Accredited Drive Clean Facility Congratulations! You may begin providing Drive Clean emissions services and help improve the quality of the air we breathe.
