1 Dear Supplier: The following guide has been created to help address any questions regarding our requirements for Certificates of Conformance/Compliance and Materials Certificates. Ideally, either document will include the following: Supplier letterhead A title at the top of the document indicating Certificate of Conformance, Materials Certificate, or both if combined Plexus part # Part # revision Drawing #, if applicable Plexus PO# Lot code or date code Ship date Quantity shipped Signature of supplier representative A Certificate of Conformance certifies that the product being delivered conforms to the following: The drawings and specifications provided by Plexus The requirements of the specific PO A Materials Certificate certifies that the product was made using: The materials required by the drawings and specifications provided by Plexus. If work is subcontracted, the certificate may also indicate that the supplier has relied upon the certifications obtained from its suppliers to verify raw materials and sub-assemblies. (See the final paragraph in this document for an example) Where possible, Plexus prefers that copies of the actual materials certificates from subcontractors be provided instead of a generic Materials Certificate. Below are some examples of acceptable Certificate of Conformance and Materials Certification documents. 2 Both of the paragraphs below would serve as a combined Certificate of Conformance and Material Certificate: The processes and materials used during the manufacturing of the parts indicated below conform to the specifications and drawings provided with the purchase order below. All material and workmanship has been qualified to the necessary documentation, where applicable, and can be made available upon request. The undersigned hereby certifies that the above listed assemblies have been manufactured using specified materials, inspected and conform to specifications as stated on the purchase order, applicable drawings and supplied requirements. One hundred percent continuity and short testing conducted per manufacturers and/or customers specification. The paragraph below would serve as a Materials Certificate, as well as an Electrical Test Certificate: It is hereby certified that the assembly noted above has been 100% electrical tested for continuity, opens and shorts. All correct material per the drawing has been used in this assembly. The paragraph below would serve as a RoHS Certification: This document certifies that the part numbers manufactured by [Supplier] are in compliance with Directive 2002/95/EC of the European Parliament and of the council of 27 January 2003 on the restriction and use of certain hazardous substances in electrical and electronics equipment (RoHS Directives). The stated components are deemed as compliant as according to definitions given in the directives. (With respect to products or materials purchased from another company, the undersigned has relied solely upon certifications obtained from such other companies without any independent testing or certification by the undersigned.) Conclusion: Thank you for your diligence in adhering to Plexus documentation requirements. Complete and correct documentation is essential in order for our team to quickly receive and process incoming shipments.
