2 FIVE MAJOR INCIDENT CAUSES Failure to stop equipment Failure to disconnect from power source Failure to dissipate residual energy Accidental restart Failure to clear work areas before restarting APPLICATION Standard applies to the control of energy during servicing and/or maintenance 2

3 SERVICING AND/OR MAINTENANCE Constructing Installing Setting up Adjusting Inspecting Modifying NORMAL PRODUCTION OPERATIONS: The utilization of a machine or equipment for its intended function 3

4 SCOPE Covers servicing and maintenance of machines and equipment when UNEXPECTED start-up or release of stored energy could cause injury PURPOSE To protect employees from injury Establish written program Develop machine/equipment specific procedures Training Periodic Inspections 4

5 EMPLOYEE CATEGORIES Authorized Affected Other ENERGIZED Machines and equipment are energized when: connected to an energy source contain stored energy 5

6 ENERGY ISOLATING DEVICE A mechanical device that physically prevents the transmission or release of energy, such as: manual circuit breaker disconnect switch line valve block ENERGY ISOLATING DEVICE Push buttons, selector switches, and other control circuit type devices are NOT energy isolating devices 6

7 ENERGY CONTROL PROCEDURES Equipment specific developed, documented, and utilized Must include: scope purpose authorization rules techniques means to enforce ENERGY CONTROL PROCEDURES Statement of intended use Specific steps for: shutting down isolating blocking securing 7

8 ENERGY CONTROL PROCEDURES Specific steps include: placement removal transfer responsibility Verification and/or testing to ensure energy control ISOLATION OF ALL ENERGY SOURCES Physically locate all energy isolating devices Operate all devices to isolate the machine or equipment from the energy source(s) 8

9 MULTIPLE ENERGY SOURCES Multiple energy sources - same type and magnitude of energy Multiple energy sources - same type, different magnitude Various types and magnitude RELEASE OF STORED ENERGY Residual energy - relieved, disconnected, restrained i.e. rendered safe 9

10 APPLICATION OF LOCKOUT/TAGOUT DEVICES Affix lockout/tagout devices to each energy isolating device by authorized employee Devices in the safe position GENERAL REQUIREMENTS If an energy isolating device is capable of being locked out, the employer shall utilize lockout If lockout is not possible - must demonstrate tagout will provide full employee protection 10

11 HARDWARE Provided by the employer for isolating, securing or blocking The only device(s) used for energy control; not used for other purposes STANDARDIZED & IDENTIFIABLE Standardized by: Color; shape; or size; print format Indicate the identity of the employee applying the device(s) 11

12 GROUP LOCKOUT/TAGOUT Authorized employees equivalent protection under group system Each authorized employee affixes lock to lockout box SHIFT/PERSONNEL CHANGES Procedures to maintain lockout continuity Transfer protection from offgoing to oncoming personnel 12

13 EMPLOYER REMOVAL OF A LOCK Verification Contact and inform Ensure knowledge before resuming work PERIODIC INSPECTION Conduct periodic inspection of energy control procedure at least annually Ensure procedure and standard are being followed 13

14 PERIODIC INSPECTION Performed by authorized employee other than the one(s) utilizing the energy control procedure being inspected Conducted to correct any deviations or inadequacies identified PERIODIC INSPECTION Include review, between the inspector and each authorized employee, of employee's responsibilities Hmmmm refresher training? 14

15 EMPLOYEE TRAINING Training to ensure the purpose and function of the program are understood Knowledge and skills required are acquired by employees RETRAINING Retraining for authorized and affected whenever: Change in their job assignments, Change in machines, equipment, or Change in the energy control procedures Periodic inspection reveals or employer believes there are deviations or inadequacies in knowledge or skills 15

16 CERTIFICATION OF TRAINING The employer shall certify training has been accomplished and kept up to date Certification shall contain each employee's name and dates of training Authorized CONTRACTORS Contractor ensures their employees understand and comply with the host employer's energy control program Lockout Procedure Coordinate as necessary 16
