1 Page 1 of 5 This form is divided into three sections: Section 1) Section 2) Section 3) Supportability for Commercial off the Shelf (COTS) and Modified Commercial off the Shelf (MCOTS) Items for Supplier Repairable Items Supportability for GDLS Designed, Supplier Manufactured Assemblies Instructions If you are a supplier that intends to supply COTS and/or MCOTS Items, please fill out Section 1 and 2 only If you are a supplier that intends to supply manufactured items for GDLS designed assemblies, please fill out Section 3 only Complete and return this form with your proposal. Applicable portions will be expanded upon, and form part of the contract requirements if your product is selected.

2 Page 2 of 5 Section 1 - Supportability for COTS and MCOTS Items The following information shall be submitted with your proposal to allow GDLS to compare your product with others being evaluated on the basis of Total Life Cost. A Total Life Cost is developed using the data requested below in addition to the initial production cost data. The Total Life Cost will be a consideration in the design trade-off study in the selection process. The items requested are not to be developed in support of this proposal. It is assumed that the majority of these items are available in support of the existing product. Place a check mark in the column to indicate what deliverables are included with your response. Failure to provide this information may result in your proposal being deemed non-compliant. Serial Requirement / Description Is your product repairable? If - what is the average time to repair by your company or an authorized / facility? What is the average repair cost? 1.1 Bill of Material or Repair Parts Breakdown If - Please provide an explanation as to why the item is not repairable. Where available, provide a detailed bill of materials, illustrated parts list or detailed service parts breakdown for your product. Supplier format is acceptable. All component down parts must be indicated. Any available service parts overhaul kits or service parts assemblies should be included in your submission Repair Parts Price and Availability Where available provide the price and lead-time for each component referenced in Serial 1.2. Volume discounts are to be included where available. Identify shelf life or storage requirements for components if applicable.

Identify the source of the data used to generate the estimate (i.e. Test results, warranty claim analysis, comparison to similar product(s), engineering estimate, etc.) 1.4 Have you developed FMEA or FMECA data? Provide the sample size used and operating conditions If Please provide a copy of the FMEA and or FMECA Do you have technical manuals for your product? Does your product require Special Tools and Test Equipment to maintain? Do you have Training courses and aids for your product? Do you have military standard packaging data for Packaging, Handling, Storage, and Transportability? If - Provide the available data such as operation, maintenance, parts, troubleshooting and or diagnostics. Indicate what formats are available for both manuals and illustrations. If - Provide a priced (if available) or un-priced list of non-common tools, lifting devices, fixtures, diagnostic equipment, etc. developed specifically for maintaining the product If - Provide details as to the min/max class size, duration, location and any prerequisites required for the training. Also indicate format (classroom, computerbased), and training aids such as wall charts, cutaways, etc. If - Provide detail, if available, such as existing approved MIL-STD packaging, fragility ratings, shelf life, ESD and EI, preservation requirements, radioactivity, magnetivity, HAZMAT/MSDS, phytosanitary standard, and single/multi-packaging info.

4 Page 4 of 5 Section 2 for Supplier Repairable Items If your company is capable of repairing or servicing the parts or components you supply to GDLS-C, the following items must be submitted for each supplier repairable assembly to allow for Total Life Cost development and to assess the feasibility of engaging the supplier (your company) for Repair and Overhaul (R&O) services throughout the life of the component. If a trained end user can also repair the product, please fill out this section. Serial Requirement Description 2.1 Location of Supplier Facility Provide the name and address for each organization that is authorized to perform repairs on the supplier s component. 2.2 Flat Rate Repair Cost 2.3 Estimated Repair Cost 2.4 Turn Around Time 2.5 Condemnation Rate Provide the flat rate repair cost for the repair of this component, if it is available. If repairs are billed on a per job basis rather than flat rate, provide an estimated average repair cost that will be used by GDLS-C to determine Total Life Costs. Provide the average turnaround times for items returned for repair and overhaul of parts. If there are multiple sites, please indicate turnaround time for each site if there are differences Provide an estimated percentage of the items that are returned to your R&O facility that cannot be repaired.

5 Page 5 of 5 Section 3 Supportability Requirements for GDLS Designed, Supplier Manufactured Assemblies If you are a supplier that intends to supply manufactured items for GDLS designed assemblies, please fill out this page. Serial Requirement Description Included with 3.1 Spare/Repair Part Pricing and Provide the price and lead-time for each piece part used in the manufacture Availability of this assembly. Volume discounts are to be included when available. Indicate items that have a shelf life, the length of shelf life and storage criteria. The list should be in supplier format and provided electronically. 3.2 Repair and Overhaul Capability Do you have the capability, capacity or interest in becoming the Repair and Overhaul centre throughout the life of this product? (GDLS cannot estimate annual throughput at this time) If R&O capability is available, provide an estimated flat rate repair cost for the assembly or a flat rate hourly cost. If R&O capability is available, provide an estimated turn around time for the repair of this assembly. 3.3 Cost of Factory Test Equipment Provide an estimated cost for the development and production of factory test equipment to support the repair of this item.
