Poag Cancelling First Revised Sheet 13 Director Effective: July 25, 2006 EXPLANATION OF TERMS DISCONTINUANCE OF SERVICE - An arrangement made at the request of the subscriber, or initiated by the Company for violation of tariff regulations by the subscriber, for a permanent interruption of telephone service. Once effected, telephone equipment would be removed, if appropriate, from the subscriber s premises and a final bill would be rendered showing monies owed to the Company as of the date service was disconnected. DORMITORY STATION - A station furnished to a college or university and located in the living quarters of students, faculty members, or employees. DROP WIRE - Wires used to connect the circuits of open wire, aerial, buried, or underground distribution facilities to a point of entrance to the building where connection is made with the inside wiring. DUPLEX SERVICE - Service which provides for simultaneous transmission in both directions. EMBARQ LOCAL OPERATING COMPANY (a.k.a. Embarq LOC) - The term used to describe Embarq Corporation s Incumbent Local Exchange Carrier (ILEC). (N) (N) EQUALIZATION - The treatment applied to a telecommunications channel so that the component frequencies of the program material transmitted have about the same relationship at the two ends of the channel. ENTERPRISE SERVICE - A service plan by which a subscriber can offer his/her customers in selected areas the privilege of calling him/her without payment of toll charges and without having to request that charged be reversed.

SPECIAL PROMOTIONS (Cont'd) GENERAL REGULATIONS Area of Promotion Service Charges Waived Period Existing Business Customers with five or more lines or trunks (including T1 based voice channel services such as ISDN BRI, Custom Access Solution, and Digital Trunking Service). Addition of ISDN BRI, Frame Relay, ATM, Digilink, Translink, Lightlink, OptiPoint or SONET Ring to their account. The customer will receive a bill credit for the first month s service for each qualified service added to the customer s account. There is no limit to the number of qualified services that a customer can add to their account nor is there a limit to the number of times a customer can qualify for the bill credit during the promotional period. 6/19/06 thru 9/16/06 (T) (T) If a customer discontinues service for which credit was issued prior to the end of required service period, credits issued under this promotion will not be rescinded, and the customer is subject only to the applicable minimum service period or termination liability charges that are normally applicable for that service.

General MISCELLANEOUS SERVICE ARRANGEMENTS a. Talking Call Waiting allows customers to know who is calling while they are on the telephone with another party. Talking Call Waiting enhances Call Waiting by allowing the customer to hear who is calling after hearing the call waiting tone while the customer is on the line. Any and all names will be voiced to the subscriber. This includes "private" or "unavailable" to be voiced in the appropriate situation. Talking Call Waiting Service subscribers will hear both a call waiting tone plus the name of the calling party on an incoming call. The customer then presses the switch hook or flash button to place the current call on hold and talk to the call waiting party. 2. Terms and Conditions a. Talking Call Waiting is provided subject to availability of facilities. b. Talking Call Waiting is available on a monthly subscription basis. This service requires no additional adjunct or telephone display equipment. c. Talking Call Waiting is available to single-line business and residence customers. d. Talking Call Waiting is not offered in conjunction with Centrex, DID, PBX, ISDN, Pay Telephone Service or Coin Line Service. e. A Call Waiting Custom Calling Feature, including but not limited to, Call Waiting Service, Enhanced Call Waiting, Call Waiting Options or any package containing the Call Waiting feature, is required in order to subscribe to Talking Call Waiting Service. 3. Rates and Charges a. Service charges and monthly rates for exchange access lines and other services with which this service is associated apply, as appropriate. Monthly Rate Per Residence Line $2.95 Per Business Line $2.95 b. Talking Call Waiting is available as an add-on to Package 5, Package 7, Ideal Solution, Sure Solution II, Home II Solution and Progressive Plan. Once effected, telephone equipment would be removed, if appropriate, from the subscriber s premises and a final bill would be rendered showing monies owed to the Company as of the date service was disconnected. DORMITORY STATION - A station furnished to a college or university and located in the living quarters of students, faculty members, or employees. DROP WIRE - Wires used to connect the circuits of open wire, aerial, buried, or underground distribution facilities to a point of entrance to the building where connection is made with the inside wiring. DUPLEX SERVICE - Service which provides for simultaneous transmission in both directions. EMBARQ LOCAL OPERATING COMPANY (a.k.a. Embarq LOC) - The term used to describe Embarq Corporation s Incumbent Local Exchange Carrier (ILEC). EQUALIZATION - The treatment applied to a telecommunications channel so that the component frequencies of the program material transmitted have about the same relationship at the two ends of the channel. ENTERPRISE SERVICE - A service plan by which a subscriber can offer his/her customers in selected areas the privilege of calling him/her without payment of toll charges and without having to request that charged be reversed.

General MISCELLANEOUS SERVICE ARRANGEMENTS a. Talking Call Waiting allows customers to know who is calling while they are on the telephone with another party. Talking Call Waiting enhances Call Waiting by allowing the customer to hear who is calling after hearing the call waiting tone while the customer is on the line. Any and all names will be voiced to the subscriber. This includes "private" or "unavailable" to be voiced in the appropriate situation. Talking Call Waiting Service subscribers will hear both a call waiting tone plus the name of the calling party on an incoming call. The customer then presses the switch hook or flash button to place the current call on hold and talk to the call waiting party. 2. Terms and Conditions a. Talking Call Waiting is provided subject to availability of facilities. b. Talking Call Waiting is available on a monthly subscription basis. This service requires no additional adjunct or telephone display equipment. c. Talking Call Waiting is available to single-line business and residence customers. d. Talking Call Waiting is not offered in conjunction with Centrex, DID, PBX, ISDN, Pay Telephone Service or Coin Line Service. e. A Call Waiting Custom Calling Feature, including but not limited to, Call Waiting Service, Enhanced Call Waiting, Call Waiting Options or any package containing the Call Waiting feature, is required in order to subscribe to Talking Call Waiting Service. 3.
