This will be applied twards the revenue share. - Balance Due by Nvember 30, Submissin f vendr lg by Nvember 30, 2018 and permissin t the Italian Assciatin t use lg during festival prmtin. Fr permissin initial here: -Obtain/have all necessary City/State and Maricpa licenses including a Maricpa Cunty issued mbile fd service permit. Fr mre infrmatin visit: -Prvide prf f insurance with the minimum f $1 millin general liability. -Participate t Exhibitrs telecnference meeting that will be called by the Italian Assciatin tw t three weeks befre the Italian Festival, date t be determined. - The Festival will be pen t the public Saturday February 23, 2019 frm 11:00 am t 9:00 pm and Sunday February 24, 2019 frm 11:00 am t 5:00 pm. Vendr s participatin fr the entire duratin f the festival is required unless agreed therwise with the Italian Assciatin (i.e. Yur Vendr Bth Cannt be left unattended during Italian Festival hurs). I will advertise the th ANNUAL ITALIAN FESTIVAL by exhibiting in my Shp and Business the 2019 Italian Festival Pster and by distributing t my custmers the Italian Festival Pstcards, during the mnths f December 2018 thrugh t February 22, I will nt sell any beverages. -I understand n exclusivity is granted t any vendr fr any prduct. I will submit my prducts list and related sales prices t the Italian Assciatin. The Italian Assciatin will review and apprve prducts and prduct pricing f participating vendrs. In ding s it may reasnably restrict the sales f certain cnsumable prducts fr certain vendrs r request price changes based n prduct prtin size. -I understand that any cnsumable prducts nt listed in the abve Participatin Frm will nt be allwed fr sale unless apprved by the Italian Assciatin. -I will cmply with any requirements set by the Italian Assciatin befre the set Italian Festival dates by submitting any required paperwrk. 4

Security will be prvided fr the nights f Friday, February 22 nd and Saturday, February 23 rd. Fd can be mved t the Bth n Saturday and Sunday mrning frm 8:00 AM t 10:00 AM. Fd must be available fr Visitrs by 11:00 AM, Saturday February 23 rd and Sunday February 24 th. -My vendr space will be up t cde fr the health and fire inspectin by 10:00 AM n Saturday February 23 rd. -My vendr space will be pen fr business n Saturday February 23 rd, 2019 frm 11:00 am t 9:00 pm and Sunday February 24 th, 2019 frm 11:00 am t 5:00 pm. Fd will be available thrughut the whle duratin f peratin hurs. - My Display items will nt be taken dwn until the end f the festival n Sunday February 24 th, 2019 at 5:00 pm. -My space will be left empty and in clean cnditin by Sunday February 24 th, 2019 at 8:00 PM. Unclean areas will be assessed a cleaning fee f up t $1, payable t the Italian Assciatin. The fee will be assessed n the basis f the cnditin f the space. -I will cmply with Maricpa Cunty Envirnmental Services regulatins and n actins undertaken by my business shall cnstitute a hazard t the health r general well-being f any ther persn. -I am respnsible fr btaining all necessary State, Cunty and lcal sales tax licenses as well as cllecting and remitting t the prper taxing authrities all sales tax. -My vendr space will cmply with the Scttsdale Fire Department regulatins including, but nt limited t, equipping my space with a fire extinguisher and ensuring all apprpriate display materials are flame retardant accrding t Fire Department Regulatins. -I am respnsible fr prviding and setting up all display inside my space. N displays will extend utside f my 10x20 Bth area. - Althugh n guarantee is made r implied, the Italian Assciatin will make an effrt t prmte the vendrs participating thrugh varius media partners.
