On behalf of the Illinois Physical Therapy Association s Board of Directors and staff, we are pleased to invite you to participate in our 2016 REVITALIZE Conference & Expo. This two-day event will be held at the Q Center in St. Charles, Ill. on March 18-19, REVITALIZE 2015 exceeded all expectations bringing together over 300 physical therapy (PT) professionals, students, and educators from across Illinois for two powerpacked days of education, networking and governance. Our exhibit hall was a huge draw for attendees and exhibitor space was sold out for the event! Attendees look forward to REVITALIZE Conference & Expo not only for the educational programming but also for the exposure to the latest products and service innovations in PT. Many attendees own their own practice and seek product literature and demonstrations that will help them to make informed purchases and offer progressive treatment recommendations to their patients. Other professional attendees advise clinic decision-makers of potential products and services to add to their clinical mix. Some professionals, along with student members, hope to gather information about career opportunities and/or advancement in their chosen field. REVITALIZE is the only yearly event that gives you the opportunity to get in front of this many Illinois PT professionals and students in one place. 5 BECOME AN EXHIBITOR! Exhibit hall space and sponsorship opportunities are limited, so don t delay! EXHIBITOR BOOTHS: $1,000 reserve by December 15, 2015 and pay only $750! The Exhibit Hall will be open to conference attendees from 11:00 am 2:00 pm on Saturday, March 19, Exhibit spaces are available to products/service providers, employers and recruiters. All exhibitors must have products or services relevant to physical therapy. Exhibitors receive: 10 x 10 exhibit space One 6 table and two chairs Electrical hook-up if needed (must be requested in advance) Lunch for two exhibitors (additional lunches may be ordered in advance - $20 each) Logo placement in , web promotions, and event signage Wifi in Exhibit Hall NEW FOR 2016! Welcome Bags: All exhibitors may provide one(1) promotional item or marketing literature for inclusion in attendees welcome bags. All items must be shipped to the IPTA Chapter Office (905 N. Main Street, Naperville, IL 60563) for arrival prior to February 15, Product Showcase: Encourage attendees to visit your booth! A table will be set up during attendee registration on March 19th (7:30 am - 9:30 am) that will provide a sneak peek into the exhibit hall. All exhibitors may display one(1) product with literature that will entice attendees to visit their booth. Display items must be dropped off/set-up prior to registration open at 7:30 am on March 19th. Product must be able to be placed on a tabletop and be no greater than 2 long and 2 wide. PRODUCT DEMONSTRATION: $100 FOR 15 MIN Only four 15-minute time slots available! Gain exclusive visibility in front of a captive audience! A corner of the exhibit hall will be reserved for exhibitor product demonstrations. Reserve your time slot and demonstrate to all interested attendees simultaneously. Demos are restricted to 15 minutes (you cannot reserve more than one slot) and are scheduled for the following times: 11:30 am (students), 12:15 pm (mixed), 12:45 pm (mixed) and 1:15 pm (PT/PTAs). Reservations will be accepted on a first come basis. Following REVITALIZE, the IPTA will make available to all sponsors/exhibitors an excel file of our attendees mailing addresses upon request. APTA policy prohibits us from sharing our members /attendees addresses, so don t forget to gather names and addresses from those who stop by your booth. 9 EXHIBITOR TERMS & CONDITIONS EXHIBITORS Assembly and Dismantling of Exhibits Exhibitors can check in at the conference registration table as early as 7 am but no later than 10 am on Saturday, March 19, Exhibits must be set up by 11 am and remain open through 2 pm. All exhibits must be fully dismantled and removed by 4 pm. The Q Center provides material handling, inclusive of the following: Receiving items on the dock and tagging them for storage Moving items from storage to the vendor booth area in Expo Storing empty containers during Expo hours Returning empty containers to vendor booths after Expo closes Moving items from vendor booth to the dock for shipping Insurance IPTA and the Q Center do not maintain insurance covering the exhibitor s property. It is the sole responsibility of the exhibitor to obtain the appropriate amount of insurance to cover their property, agents or employees from theft, damage by fire, accident or other cause. Liability The exhibitor agrees that IPTA and Q Center and any of their officers, staff members, agents or employees are not responsible for and are released from all liability as to any injury, loss or damage that may occur to the exhibitor, the exhibitor s agents or employees, or to any other persons, or to the exhibitor s property prior, during or subsequent to the holding of the 2016 IPTA REVITALIZE Conference and Expo. Security Each exhibitor is required to have their booth area staffed at all officially open exhibit times. IPTA will not guarantee exhibitors against, nor shall it be responsible for loss of any kind. It is the exhibitor s responsibility to pack and remove all items of value prior to leaving their exhibit unattended. Safety Regulations All construction shall be substantial and fixed in position for the duration of the meeting. No exhibit that violates any municipal or state laws, rules and regulations, including safety codes, will be permitted. No combustible decorations shall be used at any time. All decorations, drapes, acoustical materials, table coverings and other decoration material shall be flame-resistant to the satisfaction of the local fire department. The use of any heater or heat producing device as part of the exhibit is forbidden unless a special permit is first obtained from the local fire department and a copy of the permit is in IPTA files. Approval must be obtained from the local fire department for the use, operation or presence of an electrical, mechanical or chemical device which, in the opinion of the local fire department might be hazardous in a public place. EXHIBITORS AND SPONSORS Cancellation In the event of fire, strikes, riots, civil commotion, acts of God, war and other unavoidable circumstances rendering it impossible or impractical for any reason for the IPTA to perform such contract, the performance under such contract shall be excused. Cancellation of exhibitor space must be made in writing on or before February 15, Cancellation of exhibit space prior to this date will receive a full refund minus a $ processing fee. Cancellations received after February 15, 2016 will not receive a refund. Event sponsorships cannot be canceled once payment is received by IPTA.

10 EXHIBITOR TERMS & CONDITIONS Cancellation of the 2016 IPTA REVITALIZE Conference & Expo at any time will not subject IPTA to any damages or other liability of any kind other than a refund of the original booth or sponsor fee. Agreement The exhibitor/sponsor hereby agrees to be bound by the Rules and Regulations outlined here. The exhibitor further agrees to adhere to and be bound by the following: All applicable fire, utility, building codes and regulations. Any rules or regulations of the exposition facility. Raffles or drawings are permitted. Noisy equipment is prohibited in booths. IPTA reserves the right to expel, decline, or prohibit any sponsor or exhibitor or part of an exhibit or proposed exhibit which is not suitable or in accordance with the ethics of IPTA or which does not comply with the rules established in this prospectus. IPTA reserves the right to change location assignments of exhibits at any time as deemed necessary. The exhibitor will be immediately informed of any such changes. Cooperation of Exhibitors and Sponsors The foregoing regulations have been formulated in the best interest of all parties so your cooperation is therefore requested. These regulations are a part of the agreement between the sponsor/exhibitor and IPTA. Payment Payment must be received at the time of registration either through the web site or mailed with the registration form. QUESTIONS?
