1 Exhibit G: Proposer Response Form Please answer the questions enumerated below. If an answer requires diagrams or other large documents, abbreviate the response here, and indicate exactly where the documents can be found in the proposal document, i.e. "Appendix #," "Tab #, Page #. Lead us through the process from purchase order receipt, delivery, and receipt of payment. What locations do you ship from for each of the Universities (BTAA member institutions) identified in this solicitation? In addition, specify your firm s normal business hours for receiving orders and for providing customer service. Include a list of all company holidays. 17. Does your firm have a Customer Advisory Board? If so, please respond with information on the members and their roles. General 18. Will your firm make the Consortium Master Agreement available to affiliates of member institutions? (Exhibit B in the RFP document) 19. Describe all steps and processes regarding the creation and distribution of the price files. How will pricing updates be submitted for review by the Big Ten Academic Alliance and then by the participating Universities? 20. Are the price files audited before sending to the customer? 21. How does your firm handle billing clients that have multiple locations and/or different departments? 22. What methods of payment are accepted? (Credit cards?)

3 23. Does your firm utilize the services of subcontractors? If so, identify the methods used, your firm s quality control procedures for subcontractors, and if the subcontractors are certified small businesses or minority owned? 24. How do you measure customer satisfaction, and how would your current customers describe your customer service rating? 25. Explain what differentiates you from your competitors. 26. Provide a copy of your Service Level Agreement (SLA). Include service levels and maximum response time for service, from receipt of the issue to initial response to the customer. Identify how requests for service are made, logged, tracked and escalated by your company. 27. Provide the name, address, telephone number, fax number and for your support services. 28. Provide standard hours of operation for your support services. If standard hours are not 24/7, what support do you provide for off hours? 29. Does your firm agree and comply with the Consortium Master Agreement Terms & Conditions (Exhibit D in the RFP document)? If not, please provide redlines. 30. Is your company in agreement with the Deliveries expectation in RFP Sections 3:3.8? Reporting - General 31. Your firm should provide detailed quarterly savings reports to participating Universities, comparing their spend to what they would pay without the contract (Net Discount off List). This would be for each University using the contract and for the overall savings of the Consortium. Please respond if your firm can accommodate this request. 32. Your firm should provide quarterly electronic reports to the Consortium, detailing total dollar expenditures and relevant Administrative Fees generated by each Participant for goods or services provided under the Agreement. Please respond if you can accommodate this request. 33. For Universities with multiple campuses, can your firm provide quarterly electronic reports to the Consortium detailing their spend by category? Technology 34. Describe the web-based system you provide and how it would integrate with other systems. Give details and describe technologies and techniques used to integrate with other systems.

4 35. Is your e-commerce system mobile friendly and/or do you utilize apps? If so, describe what components are mobile friendly, including the candidate interface and the administration/client side. 36. Can you comply with the following: Proposer shall comply with the Americans with Disabilities Act (ADA) by supporting assistive software or devices such as large print interfaces, text-to-speech output, voice-activated input, refreshable braille displays, and alternate keyboard or pointer interfaces, in a manner consistent with the Web Accessibility Initiative Web Content Accessibility Guidelines 2.0 AA ( Proposer shall ensure that product maintenance and upgrades are implemented in a manner that does not compromise product accessibility. Proposer shall provide to Licensee a current, accurate completed Voluntary Product Accessibility Template (VPAT) to demonstrate compliance with the federal Section 508 standards ( Proposer warrants the accuracy of the information contained in such VPAT and in any other documentation furnished to Consortium with respect to the accessibility of the product. If the product does not comply, the Proposer shall adapt the product in a timely manner and at no cost to Consortium in order to comply with applicable law." 37. Provide explanation of any planned system changes and any proposed effect on order processing under your proposal. 38. Describe the different user security levels supported within your system. 39. Describe how your proposed system supports event logging, with respect to authentication or attempts to access unauthorized data or functions. 40. Describe your development practices in place to prevent security vulnerabilities. This could include programming standards, employee training, code walkthroughs, and testing. 41. Provide the name and contact information for a resource on your information security team. 42. Has your system ever been compromised through hacking attempts or security vulnerability? Describe. Implementation 43. Provide the contract implementation process and a sample timeline for an individual institution to on-board. 44. Describe company's steps to ensure successful implementation of a Consortium member account.

5 45. Describe the training plan for implementation and any provision for institutions with multiple locations. 46. Does implementation include marketing? 47. Is training available for new users after implementation? If so, what is the charge for this training?
