The Firm or the Group of Firms shall demonstrate that it meets the applicable qualification criteria by completing this Qualification Questionnaire and by providing the requested documents and references. 3 EDMS No.: INFORMATION ABOUT THE FIRM OR THE GROUP OF FIRMS Names and Addresses Firms interested in participating in the forthcoming Invitation to Tender procedure as a member of a Group of Firms 1 shall return A SINGLE questionnaire for the Group of Firms, to be completed by the Lead Firm and signed by each of its members. The number of firms in the Group of Firms shall not exceed three. 4 2 EDMS No GENERAL QUALIFICATION CRITERIA CERN reserves the right not to consider a Firm or Group of Firms for the forthcoming invitation-totender procedure in the event that: It has replied yes to at least one question in 2.1; It has made a false declaration in the present Qualification Questionnaire; Notwithstanding 3.1.2, CERN reasonably concludes that the Firm or Group of Firms will be unable to complete or fulfil its obligations under the contract due to the Firm s or Group of Firm s financial situation. Only firms that have replied to the market survey and are qualified on the basis of the following criteria shall be invited to tender: The technical, financial and other qualification criteria defined in this document; CERN s previous experience with the firm, if any; The number of firms to be selected from each Member State 2, as specified in 2.3 hereunder. Administrative Situation Is the Firm or any member of the Group of Firms involved in bankruptcy proceedings, prosecution for debt, sequestration or any analogous situation arising from a similar procedure provided for in law? If yes in the case of a Group of Firms, please indicate which member(s):. Subcontracting 3 Subcontracting will be permitted for the execution of the contract. However, the following parts of the Supply cannot be subcontracted or performed by another firm than the Firm or one firm of the Group of Firms: Dielectric, magnetic and mechanical design of the transformer; Manufacturing of active parts (such as magnetic core, windings), current leads, insulation system (such as all resin parts, resin impregnation process) and clamping structures; Assembly of all the parts which constitute the transformer. 2 except where stipulated otherwise, the term Member State(s) shall mean full member state(s) of CERN, associate member state(s) of CERN and associate member state(s) in the pre-stage of accession to membership of CERN. 3 The term subcontract(s) shall mean any arrangement whereby the Contractor contracts with another legal entity (the subcontractor), except legal entities under its direct control, for the provision of a major part of the Supply (including sub-assemblies and components or any part of the services forming part of the Supply). The term subcontract(s) shall include sub-subcontract(s).

5 EDMS No.: Subcontracting is subject to CERN s prior written approval of each proposed subcontractor. CERN reserves the right to refuse the participation of a subcontractor and to make checks about the proposed subcontractor(s). Does the Firm or Group of Firms intend to subcontract any part of the Supply? If yes, please complete the table in 2.3. Country of Origin The term country of origin shall mean the country(ies) of manufacture or last major transformation of the Supply (including sub-assemblies and components) by the contractor or its subcontractor(s). The Firm, or each member of the Group of Firms, shall be established within one of the CERN Member States. The Supply must originate in the Member States. However, CERN reserves the right not to invite a Firm or a Group of Firms to tender if: It proposes a Supply originating in an associate member state or an associate member state in the pre-stage of accession to membership; and The combined annual financial value of orders, contracts and appointments already committed for that country: Exceeds the amount of the country s financial contribution to CERN for the financial year concerned; or Would exceed the financial contribution if the Firm, or a Group of Firms, were awarded the contract. In addition, with a view to improving the distribution of purchases among its Member States, and in order to decide on the number of potential bidders to be selected from each Member State, CERN shall take into account the Member State s contribution, its industrial return coefficient and its estimated share of the contract (refer to the Appendix: Selection and Adjudication Criteria for Supply Contracts ). 13 EDMS No.: TECHNICAL QUALIFICATION CRITERIA Competence and Experience The Firm or the members of the Group of Firms collectively shall prove relevant competence and experience by providing the references specified hereunder Design competence and experience The Firm or the Lead Firm (in the case of a Group of Firms) shall have a proven competence and experience at least in the field of design of high voltage cast-resin dry-type power transformers that are necessary to meet the requirements of the scope, complexity and volume specified in the Technical Description. Is this criterion fulfilled for the above-mentioned field? If not, please explain why not: Manufacturing competence and experience The Firm or the members of the Group of Firms collectively shall have a proven competence and experience in the field of manufacturing of high voltage cast-resin dry-type power transformers that are necessary to meet the requirements of the scope, complexity and volume specified in the Technical Description. Is this criterion fulfilled for the above-mentioned field? If not, please explain why not: References The Firm or the members of the Group of Firms collectively shall provide at least five relevant references corresponding to the above-mentioned competence and experience in the last five years that are similar to the future contract in terms of scope, complexity and volume. Is this criterion fulfilled for all the above-mentioned fields? Please use the reference form annexed and duplicate as necessary. CERN reserves the right to contact the references. If not, please explain why not:

14 12 EDMS No The categories of transformers, as specified in the Technical Description, shall be part of a standard range products of the Firm or of the members of the Group of Firms collectively, designed and manufactured since at least five years. Is this criterion fulfilled for all the above-mentioned fields? If not, please explain why not: Special Design References Additionally, the Firm or the members of the Group of Firms collectively shall provide at least five relevant references corresponding to design and manufacturing projects developed in the last five years, relevant to high voltage cast-resin dry-type power transformers with special design (such as power converter transformers, three-phase transformers composed by three single-phase units, and with special design characteristics such as copper windings, core material with low specific iron losses). Is this criterion fulfilled for all the above-mentioned fields? If not, please explain why not: CERN reserves the right to check the references provided. In addition, the Firm or each member of the Group of Firms shall arrange a visit by CERN to any of its references if so requested. Please use the annexed Reference form and duplicate it as necessary. CERN reserves the right to contact the references. Equipment and facilities Equipment The Firm or the members of the Group of Firms collectively shall have equipment, general and specific tools, adequate working spaces and other necessary resources to supply the quantities required within the schedule indicated respectively in section 2 and 4 of the Technical Description. Is this criterion fulfilled? If not, please explain why not:

15 EDMS No.: Manufacturing Facilities The Firm or the members of the Group of Firms collectively shall be capable of manufacturing castresin dry-type power transformers as specified in the Technical Description. The facilities for the production of such transformers shall be existing and operative at the time of this market survey. Is this criterion fulfilled? If not, please explain why not: Testing Facilities The Firm or the members of the Group of Firms collectively shall have the qualified staff and the necessary equipment to fully test (both type and routine tests) and prove the performance of the required equipment according to IEC standards. The test facilities shall be existing, operative and made available for inspection at the time of tendering. Is this criterion fulfilled? If not, please explain why not: Testing references The Firm or the members of the Group of Firms collectively shall have type test certificates for each one of the five different categories of cast-resin dry-type power transformers specified in the Technical Description. The Firm or the members of the Group of Firms collectively shall provide a list of all type test certificates for cast-resin dry-type power transformers it has in its portfolio. Please use the annexed Testing reference form. CERN reserves the rights to request any original type test certificate provided in the list. Warranty The Firm or the Lead Firm (in the case of a Group of Firms) shall provide a minimum warranty period of two years, extendable up to five years after acceptance, as an option. Corrective Maintenance The Firm or the members of the Group of Firms collectively, in case of any transformer failure, shall perform fault analysis with relevant inspections and corrective maintenance actions during the warranty period, within a period of four weeks from receipt of the written notification from CERN.

16 14 EDMS No The repaired unit(s) shall be returned to CERN within 12 weeks from receipt of the written notification from CERN, excluding transport time. Is this criterion fulfilled? If not, please explain why not: The Firm or the members of the Group of Firms collectively, shall provide a corrective maintenance service for all the delivered transformers, for a period of up to five years after expiration of the warranty. Is this criterion fulfilled? Moreover, the Firm or the members of the Group of Firms collectively shall provide at least three relevant references corresponding to fault analysis and repair activities performed in the last five years, relevant to high voltage cast-resin dry-type power transformers with electrical characteristics similar or equal to the ones specified in the Technical Description. Please use the annexed Fault analysis reference form. Safety The Firm or each member of the Group of Firms (in this case the requested information must be provided for EACH member) shall provide evidence of its commitment to ensuring Safety (safety, health, working conditions and environmental protection) by providing a description of its individual policies and actions in these fields. Please attach a description of the applicable policy and actions undertaken by the firm(s) in the field of Safety. Schedule The Firm or the Lead Firm (in the case of a Group of Firms) shall be able to comply with the provisional delivery schedule stated in section 4 of the Technical Description. In particular: The Firm or the Lead Firm (in the case of a Group of Firms) shall be able to deliver the Supply to CERN within 12 weeks of the notification of the corresponding release order from CERN; In the case of a first order of any transformer type, the Firm or the Lead Firm (in the case of a Group of Firms) shall be able to deliver the Supply to CERN within 16 weeks of the notification of the corresponding release order from CERN; The Firm or the Lead Firm (in the case of a Group of Firms) shall be able to deliver individual release orders of the Supply up to a maximum of 4 units within one calendar week; The Firm or the Lead Firm (in the case of a Group of Firms) shall be able to deliver a supply of maximum 16 transformer units to CERN within 12 weeks starting from October Are these criteria fulfilled?

17 EDMS No.: If not, please explain why not: Quality Assurance Certification The Firm or each member of the Group of Firms (in this case the requested information must be provided for EACH member) shall have ISO 9001:2015 certification in the field of transformers design and manufacturing, or be able to demonstrate compliance with certification or shall be willing to establish an equivalent quality assurance procedure. Is this criterion fulfilled? If yes, please attach a copy of the relevant certificate, which must clearly show the certification authority and date of certification.
