If you are an existing Brilliant Distinctions member, your program account will be automatically upgraded to include the Brilliant Distinctions Points Program! 10

11 REGISTER EARN REDEEM As a Brilliant Distinctions member, you will receive a monthly enewsletter packed with tips, articles, and information designed with you in mind. It will also contain helpful information about your account, including treatment reminders and exclusive invitations to events in your area. The Brilliant Distinctions Points Program will reward you with points each time you receive a BOTOX Cosmetic (onabotulinumtoxina) or JUVÉDERM XC treatment from a participating physician (as long as it has been three months since your last BOTOX Cosmetic treatment). All physicians will have a BD icon located next to their name when you are using the Find a Doctor tool Visit your physician for a BOTOX Cosmetic (onabotulinumtoxina) or JUVÉDERM XC treatment Your physician will issue your voucher points after your treatment Note: BOTOX Cosmetic treatments must be at least three months apart for points to be deposited into your account. 13 REGISTER EARN REDEEM 3) REDEEM When you have made an appointment with a participating physician, follow the steps below to redeem your points (qualified patients only). Log on at and go to Redeem Points to get dollars off select ALLERGAN products and services, including: BOTOX Cosmetic (onabotulinumtoxina) JUVÉDERM XC LATISSE (bimatoprost ophthalmic solution) 0.03% VIVITÉ products Click on Go to Create Coupon or Redeem Points You will see a drop-down menu that will show available coupon amounts based on how many points you have accumulated. Alternate coupon amounts will be shown in gray to illustrate what you can get when you save up more points Click Create Coupon and a printable coupon will be available for you to bring to your physician at your next visit. Coupons can be created for a maximum dollar amount of $75 Note: Once a coupon is created, it will expire in 60 days. If your treatment plans have changed and you will not be using your coupon before the expiration date, call the call center ( , 9AM 9PM EST, M F) and have the points redeposited. You can check with your physician to see if this applies to where you live When creating a coupon for redeeming on LATISSE, you have the option to create a pharmacy-specific coupon to be presented at time of purchase Note: Pharmacy coupons for LATISSE are available in amounts of $25 and $50. You also have the option to donate your points to the following charities: Make-A-Wish Foundation Dress for Success You can choose the charity you would like to donate your program points to and select the amount of points to be redeemed. Then, select the checkbox to acknowledge that the donation is being made by Allergan, Inc. on your behalf. 15 Track Your Rewards Another great feature of the Brilliant Distinctions Points Program is the ability to view all of your past transactions online in the My Rewards History section of the program site. You can view a list of your transaction history by date and also view your points earned, points redeemed, and the status of each of your recent transactions. Refer A Friend You have the chance to let your friends begin earning rewards and saving money by joining the Brilliant Distinctions Points Program! There is a Refer a Friend section on your program homepage, where you can click on the Refer a Friend link and it opens an information box where you can enter the following information: Friend(s ) Address First and last name is optional An will be sent to your friend (from you, as the sender) and if they decide to join the program, they will receive bonus points for completing activation and registration. How do I sign up for this program? ANSWER Presently, the only treatments included in the program for earning points are BOTOX Cosmetic and JUVÉDERM XC treatments. 19 QUESTION What if I have registered but am not showing up in my physician s system? How do I access my Brilliant Distinctions Program account? How do I earn points? ANSWER The Brilliant Distinctions Program has two parts: Membership to receive ALLERGAN product information, exclusive s, and event information Access to the Brilliant Distinctions Points Program Note: You must specifically be signed up for the Points Program to begin earning program rewards. Once you activate into the program, you can log on at or There, you can access specific account information, deposit points from BOTOX Cosmetic (onabotulinumtoxina) and JUVÉDERM XC treatments, and redeem coupons for dollars off ALLERGAN products and services (qualified patients only). You will also have the option to donate your points to select charities. Each time you receive a BOTOX Cosmetic or JUVÉDERM XC treatment, you will receive voucher points from your physician. Each treatment is worth the following number of points: BOTOX Cosmetic treatment 200 points JUVÉDERM XC treatment 1 syringe 200 points 2 syringes 400 points An will be automatically sent to you (and can also be printed at your physician s office) confirming that the voucher points have been generated. You will then log in to your program homepage and click on Deposit to bank the points into your account. Please note that BOTOX Cosmetic treatments must be at least three months apart for points to be deposited into your account. Additionally, the BOTOX Cosmetic treatment must total $200 or more and the JUVÉDERM XC treatment must total $350 or more to be eligible for a voucher. 20 QUESTION How often can I earn points for BOTOX Cosmetic (onabotulinumtoxina) treatments? JUVÉDERM XC treatments? If I receive a treatment for both BOTOX Cosmetic and JUVÉDERM XC on the same day, will I receive multiple vouchers? How do I receive my voucher points? What if I did not receive my voucher confirmation ? ANSWER Since Injection intervals of BOTOX Cosmetic should occur no more than every three months, patients coming in for a BOTOX Cosmetic treatment less than three months from their last treatment should not be given a voucher for treatment. There are no patient restrictions on the frequency of JUVÉDERM XC treatments. Only one voucher will be issued by the physician that displays an itemized list of the points received for each treatment. Once a physician creates a voucher for a treatment, the points will show up as undeposited voucher points on your program homepage. You will log in, and with the click of a button, the points are deposited into your program account. The voucher is not necessary to deposit points into your account, but check to make sure your address is correct on the myinfo tab. Note: You will need to add BDPoints@BrilliantDistinctionsProgram.com as a safe sender to ensure these s are being delivered. Do I have to go back to the same physician that I activated my account with to receive my vouchers? 21 QUESTION Do vouchers expire? Can I give my voucher points to someone else to add to their account? Do coupons expire? Do points ever expire? Can points be reinstated after they expire? What if I accidentally create a coupon? Can I give my coupon to someone else to use for a treatment? ANSWER Yes, you will need to deposit your voucher points within 60 days from the voucher issue date. Point vouchers are non-transferable. Yes, when a coupon is created, it expires 60 days from the coupon creation date. Points will remain active as long as they have been deposited or redeemed within the last two years. If an account is closed due to inactivity, all points will be forfeited. Points cannot be reinstated after they expire, but it is easy to keep points active by depositing or redeeming them. There is an option to cancel a coupon once it s been created. Additionally, you can go to My Rewards History on the program site and click on the cancel icon that corresponds to the coupon you want to cancel. Program reward coupons toward treatments are non-transferable. FAQs 21

22 QUESTION What happens if a coupon is lost or stolen? How do I redeem my points? If I make a donation to a charity, will any of my information be shared? ANSWER Cancel the coupon on the program site by going to My Rewards History and click on the cancel icon that corresponds to the coupon you want to cancel. If additional assistance is needed, contact the call center at (9AM 9PM EST, M F) to speak to a representative who can help issue a new coupon. Once you sign up for the Brilliant Distinctions Points Program, you will have access to the program homepage. The page provides specific account information, the ability to deposit points from BOTOX Cosmetic (onabotulinumtoxina) and JUVÉDERM XC treatments, and redeem points for dollars off future treatments or select ALLERGAN products (qualified patients only). You can also donate points to select charities. No patient information will ever be shared when donating to a charity. You will select the amount of points to be redeemed and select the checkbox to acknowledge that the donation is being made by Allergan, Inc. on your behalf. 24 QUESTION How can I redeem points for LATISSE (bimatoprost ophthalmic solution) 0.03% at the pharmacy? What is the process for redeeming a LATISSE coupon at the pharmacy? How can I find a participating physician? Can I choose a physician to store in my Brilliant Distinctions Program profile? How can I verify which physician is on my profile? Do I need to specify a physician when requesting/printing a reward coupon? ANSWER When creating a coupon, select the product you will be redeeming points for. If you choose LATISSE, you will need to indicate if you are going to redeem your coupon at a physician s office or pharmacy. By choosing pharmacy, a pharmacy-specific coupon will be generated that can be used at your local pharmacy. But don t worry, if you change your mind and decide to use your coupon at a physician s office it will work there as well. Currently, there are some states that only dispense LATISSE via the pharmacy. Check with your physician to find out if this applies to where you live. The name of your preferred physician will be displayed. No, there is no need to specify a physician. As long as the physician is registered for the program, then they will be able to earn the credit from the redemption. 24

25 QUESTION If I go to two different physicians for BOTOX Cosmetic (onabotulinumtoxina) and JUVÉDERM XC treatments, can I have two accounts for the program? How does Refer a Friend work? Can I participate in this program without Internet access? How do I change my name or other information, like an address? ANSWER Each person can have only one account, but can earn and redeem points at any participating physician. For example, if a patient lives in another city for part of the year, as long as the physicians at both locations are participating physicians, you can earn points for all BOTOX Cosmetic or JUVÉDERM XC treatments received. Please note that BOTOX Cosmetic treatments must be at least three months apart for points to be deposited into your account. There is a Refer a Friend section on the program website where you can enter friends addresses (entering the first and last name will be optional). An will be sent to your friend (from you, as the sender) and if they decide to join the program, they will receive bonus points for completing the activation and registration. You can refer up to 10 friends at any one time. Only the friend will receive bonus points. The program is only accessible through the Internet. This includes program registration, depositing points, and redeeming points. If you don t personally have Internet access, we suggest that you visit a local public library to use the computers and Internet service there. These services are usually provided free of charge to anyone with a library card. Log in to your account on click on myinfo, and update any pertinent information. 26 QUESTION Can I de-activate from the Brilliant Distinctions Program? If I de-activate my account, can my points ever be reinstated if the account is re-activated? Does the program provide my name or any other information about my treatments to anyone? If two members of the same household both get BOTOX Cosmetic (onabotulinumtoxina) or JUVÉDERM XC treatments, can they have a joint account? ANSWER We d like to know if this program doesn t meet your needs, but any account can be closed online by logging on to and clicking on the mypreferences tab or by calling the call center at (9AM 9PM EST, M F). If an account is closed by request or inactivity, all points will be forfeited. Names of program members will not be sold or shared with anyone. No information will ever be provided to anyone about treatments, with the exception of participating physicians and call center support personnel in order to help you resolve any issues or answer any questions you have. Federal government (HIPAA) regulations prevent us from joining accounts. ANSWER This program is designed to allow all communications with members to take place via the Internet and . Nothing will ever be sent to your home address without your permission. 29 BOTOX Cosmetic (onabotulinumtoxina) Important Information Indication BOTOX Cosmetic is a prescription medicine that is injected into muscles and used to improve the look of moderate to severe frown lines between the eyebrows (glabellar lines) in people 18 to 65 years of age for a short period of time (temporary). IMPORTANT SAFETY INFORMATION BOTOX Cosmetic may cause serious side effects that can be life threatening. Call your doctor or get medical help right away if you have any of these problems any time (hours to weeks) after injection of BOTOX Cosmetic: Problems swallowing, speaking, or breathing, due to weakening of associated muscles, can be severe and result in loss of life. You are at the highest risk if these problems are pre-existing before injection. Swallowing problems may last for several months. Spread of toxin effects. The effect of botulinum toxin may affect areas away from the injection site and cause serious symptoms including: loss of strength and all-over muscle weakness, double vision, blurred vision and drooping eyelids, hoarseness or change or loss of voice (dysphonia), trouble saying words clearly (dysarthria), loss of bladder control, trouble breathing, trouble swallowing. There has not been a confirmed serious case of spread of toxin effect when BOTOX Cosmetic has been used at the recommended dose to treat frown lines. The dose of BOTOX Cosmetic is not the same as, or comparable to, another botulinum toxin product. Serious and or immediate allergic reactions have been reported. These reactions include itchy rash, swelling, and shortness of breath. Tell your doctor or get medical help right away if you experience any such symptoms, further injection of BOTOX Cosmetic should be discontinued. Do not take BOTOX Cosmetic if you: are allergic to any of the ingredients in BOTOX Cosmetic (see Medication Guide for ingredients); had an allergic reaction to any other botulinum toxin product such as Myobloc or Dysport ; have a skin infection at the planned injection site. Tell your doctor about all your muscle or nerve conditions such as amyotrophic lateral sclerosis [ALS or Lou Gehrig s disease], myasthenia gravis or Lambert-Eaton syndrome as you may be at increased risk of serious side effects including severe dysphagia (difficulty swallowing) and respiratory compromise (difficulty breathing) from typical doses of BOTOX Cosmetic. Tell your doctor about all your medical conditions, including if you have: plans to have surgery; had surgery on your face; weakness of forehead muscles, such as trouble raising your eyebrows; drooping eyelids; any other abnormal facial change; are pregnant or plan to become pregnant (It is not known if BOTOX Cosmetic can harm your unborn baby); are breast-feeding or plan to breastfeed (It is not known if BOTOX Cosmetic passes into breast milk). 30 BOTOX Cosmetic (onabotulinumtoxina) IMPORTANT SAFETY INFORMATION CONTINUED Tell your doctor about all the medicines you take, including prescription and nonprescription medicines, vitamins and herbal products. BOTOX Cosmetic may cause loss of strength or general muscle weakness, or vision problems. If this happens, do not drive a car, operate machinery, or do other dangerous activities. Other side effects of BOTOX Cosmetic include: dry mouth, discomfort or pain at the injection site, tiredness, headache, neck pain, and eye problems: double vision, blurred vision, decreased eyesight, drooping eyelids, swelling of your eyelids, and dry eyes. For more information refer to the Medication Guide or talk with your doctor. You are encouraged to report negative side effects of prescription drugs to the FDA. Visit or call FDA JUVÉDERM Important Safety Information JUVÉDERM injectable gel (including JUVÉDERM Ultra, JUVÉDERM Ultra Plus, JUVÉDERM Ultra XC, and JUVÉDERM Ultra Plus XC) is indicated for correction of moderate to severe facial wrinkles and folds (such as nasolabial folds). Important JUVÉDERM Treatment Considerations Side effects are usually mild to moderate, lasting 7 days or less, and include temporary injection-site reactions such as redness, pain, firmness, swelling, and bumps. JUVÉDERM is not for people with severe allergies.
