2 Welcome to the RAC Benefits Plus Co-Worker Program! This guide will help you remember and understand how to best use the program. To start using the program, simply print the Co-Worker Program and Pet Rx Cards you receive via . Carry the ID cards with you to identify yourself as a RAC Benefits Plus Co-Worker Program participant. If you have any questions, please call (888) Monday through Friday from 8AM to 8PM, Eastern time and a Member Services representative will be happy to assist you. We hope that you are as excited about your program as we are to serve you! The following information may be found in this member guide, which constitutes a part of the membership agreement. Range of Discounts / Other Benefits / Member Assistance & Plan Information Disclosures: This discount medical, health, and drug plan is NOT insurance, a health insurance policy, a Medicare Prescription Drug Plan, or a qualified health plan under the Affordable Care Act. The Plan provides discounts at certain health care providers for medical services or products. The range of discounts for medical, pharmacy or ancillary services offered under The Plan will vary depending on the type of provider and products or services. The Plan does not make and is prohibited from making members payments to providers for products or services received under The Plan. The plan member is required and obligated to pay for all medical or ancillary services, but will receive a discount from those providers that have contracted with the discount medical plan organization. The Discount Medical Plan Organization is Alliance HealthCard of Florida, Inc., P.O. Box , Dallas, TX You may call for more information or visit for a list of providers. The Plan will make available upon request, a list of program providers and the providers city, state and specialty, located in the member s service area. Note to MA consumers: The discount health benefit plan is not insurance coverage and does not meet the minimum creditable coverage requirements under M.G.L. c. 111M and 956 CMR 5.00.

3 Pharmacy Network You will now save an average of 31% off the retail price of generic drugs and 15% off the retail price on brand name drugs at retail pharmacies. Simply take your prescription into any of the more than 65,000 participating pharmacies nationwide. Present your Membership ID card along with your prescription to the pharmacist. You will receive the network negotiated price or the pharmacy s price, whichever is lower. Simply pay when you pick up your prescriptions! The retail pharmacy network includes more than 65,000 national and regional chain pharmacies such as CVS/pharmacy, RiteAid, Walgreens, Albertsons, Kroger, Walmart and more! To determine if your pharmacy is part of the network, call Member Services or use the provider locator at Mail Order Direct 1. Ask your doctor to write a prescription for a 90-day supply, with as many as three refills. 2. Complete the Mail Order Direct order form. Download the form at: or call Member Services at to have one mailed to you. 3. Mail your order form, along with your prescription and payment, to Caremark SAT STD, PO Box , San Antonio, TX For your convenience, we accept VISA, MasterCard, Discover or American Express. You can also pay by check or money order. Please do not send cash. 4. Allow 10 to 14 days for delivery of your medication from the day you mail your order. Standard shipping is free! Overnight or second-day shipping is available for an additional charge. Expedited shipping only affects shipping time, not processing time of your order. On average, with expedited shipping, you will receive your medication within 7 to 12 days from the day you mail your order. THIS IS NOT A HEALTH INSURANCE POLICY

4 Vision Network Savings Savings on eye examinations, eye glasses, contact lenses, lens options and accessories, LASIK and PRK laser vision procedure and frames from Luxottica, the world s leading frame manufacturer of such brands as Anne Klein, Brooks Brothers, Persol and others. Access to a national network of over 45,000 vision providers in 21,000 locations, including LensCrafters, Pearle Vision, Sears Optical, Target Optical, and JCPenney Optical. Members enjoy their choice of participating independent optometrists, ophthalmologists and opticians located throughout the country. How to use the program 1. To locate a participating vision care provider, call Monday through Friday from 8 a.m. to 8 p.m. Eastern Standard Time or visit the web at 2. Call the provider for an appointment. Verify that the provider participates in the program offered through EyeMed Vision Care. If your provider does not recognize your card or has questions, please call Member Services. 3. When arriving at your provider s location, present your ID card and pay the provider at the time of service. This program does not make payments to providers. You are obligated to pay the provider or make payment arrangements at the time of service. 4. Members are obligated to pay for all health care services but will receive a discount from those health care providers who have contracted with the discount plan organization. 5. If the provider has any questions, please ask them to call Member Services.

5 Pet Prescription Savings With this benefit you can get your pet s needed and trusted prescriptions and other everyday general pet needs at a discount. Approximately 50% of all pet prescriptions are actually human drugs that can be filled at your local pharmacy. Your Pet Prescription Savings card is widely accepted at over 54,000 participating pharmacies across the United States, including national and regional chains, pharmacy associations and local community pharmacies. How to use the program 1. To locate a participating pharmacy provider, call Monday through Friday from 8 a.m. to 8 p.m. Eastern Standard Time or visit the web at 2. Enter your Member ID and select Find My Benefits. 3. Click on the Search for Provider icon next to Paramount Pet RX. 4. The pharmacy look up is located on the Retail Pet Med tab. 5. Take your written prescription(s) from your vet to the participating provider of your choice to receive a discount.

6 Savings on Diabetic Supplies As a member you can save 40% to 60% on most diabetic supplies. This program provides discounted prices on packages of diabetic supplies (including monitor, lancets, test strips, and carrying case), medications, and special programs. There are no health restrictions, free shipping on all orders, and you can use this service an unlimited number of times a year. This benefit applies to you and your immediate family. How to use the program 1. Visit 2. Click on the tab that reads Diabetic Supplies. 3. Review the information on what is available. 4. Click on Enroll Now to start the process of saving on your diabetic supplies. 5. Select how many times a day you test. 6. Enter your personal information to continue ordering your supplies. 7. Or members can call and give the representative GroupID AXS1000.

7 Vitamins & Supplements Members receive 15% off at check out from an online company that sells top-ofthe-line natural vitamins, nutritional supplements, and bath and personal care products. You can choose from 3,500 products on the site and most products ship within hours. Shop by product (Healthy Lifestyle, Vitamins & Minerals, Bath & Personal Care, and Healthy Pets), shop by condition (Eye Health Support, Blood Sugar Support, Mood Support, and more), or shop by brand name. How to use the program 1. Visit 2. Enter Coupon Code: access1 at check out for immediate savings.

8 Dining & Shopping Discounts As a member, you will enjoy savings up to 50% off thousands of merchants nationwide. Members can save when visiting some of their favorite restaurants, car washes, dry cleaners, hair salons and retailers they visit every day. Online Registration 1. Go to and choose your state. 2. Click on the Dining & Shopping Discounts link at the top of the page. 3. Select a language and enter the membership number to register your membership and create a user name and password. The member ID# is found on the front of your membership card. 4. This username and password will be used for all future visits to the site. 5. Enter your ZIP code to see what savings are in your area. You can also shop by category or by company name by selecting the appropriate tab at the top of the page. 6. Print the offer or coupon to present to your local retailer or place an order online using the provided discount code. Mobile Registration 1. From your mobile device download the My Deals app from the itunes App Store or Android Market. (Currently available for the iphone, ipad, ipod Touch, and Android) 2. After opening the mobile app, you will be prompted to create a profile. Enter your registration code in the Mobile Password field. NOTE: Your Registration Code is 2135-Your Member ID# (your member ID# is located on the front of your membership card) 3. Select a category of savings and scroll through all the available deals. You can also look for specific merchants within the category by entering the company name in the search field. 4. Click on the offer for details about the savings, then click redeem for access to the coupon. You will need to show your mobile device to the merchant to receive the savings. 9 Grocery Coupon Savings You can save on the groceries you plan on buying when you order coupons through this coupon savings plan. These savings can total up to $1,000 at stores that honor double coupon values. You choose from thousands of coupons for items such as cereals, coffees, juices, soaps, detergents, aspirin, snacks, frozen foods, baby products and much more. How to use the program 1. Go to 2. Enter the Member ID# located on your membership card, your address, and then click submit. 3. Complete the registration process. 4. Select a category to review and make your coupon selections. You can choose the coupons you want (up to $20 worth of savings) by entering the number of coupons you want and clicking add to cart. You cannot order more than $20 worth of coupons, but are not required to order this amount. 5. Once you have selected the coupons that you want click checkout to finalize the order. 6. You will need to follow the instructions to complete the order online. You will pay $.75 for shipping and handling plus 10% of the value of the coupons. For example, if you order $10 in coupons then you will pay $1.75. The fee submitted for shipping and handling is offset in additional coupons. This amount can be charged to a credit or debit card. If you do not have a credit or debit card you will need to print out the online order form and mail it to the specified address listed on the website with a check or money order and a self-addressed, stamped envelope for processing. All mailed orders must be post marked within 7 days. 7. Limit of 4 orders per month.

10 Savings on Roadside Assistance Should your car breakdown or you need any type of automotive help, this benefit is available 24 hours a day, seven days a week. You will pay commercially negotiated discounted rates at the time service is performed. Discounted service applies to towing, flat tire repair, fuel/fluid delivery, jump starts, lock-out assistance and any auto malfunction. NOTE: Delays may occur depending on weather and your location. How to use the benefit 1. In the event you need automotive roadside service, call Listen for the prompt for Roadside Assistance and press the correct key and wait to speak with a representative. 3. Identify yourself as a Benefits Plus Member. 4. Report your trouble and location. 5. The appropriate servicer will be dispatched to you. 6. Payment for roadside services performed must be made at the time the service is rendered. Cash, Visa and MasterCard are accepted.

11 Discounted Hotel Lodging Members receive 15% off the published lodging rates at participating Choice Hotels. Select the participating hotel you wish to stay at including Comfort Inn, Comfort Suites, Quality, Sleep Inn, Clarion, MainStay Suites, Econo Lodge, and Roadway Inn hotels. You will save 15% off their published lodging rates. This discount may not be combined with any other Choice hotel discounts or promotions. How to use the benefit 1. Visit 2. Choose your destination, dates, rooms and number of guests. 3. Under the Select Rate heading, click the drop-down arrow next to Best Available Rate. 4. Then choose Special Rate/Corp ID*. 5. Enter the Identification code of in the box provided, then click submit button. 6. Click the Yes, Confirm button. 7. Click the Find Hotels button and the website will pull up the discounted rates for the destination you chose. 8. Select the hotel of your choice and book your room at the discounted rate. 9. You may also call to use the hotel savings benefit. 10. Listen for the prompt for Choice Hotels and press the correct key and wait to speak with a representative. 11. Provide the reservation specialist with the discount code The specialist will assist you in making your reservation at the discounted rate. 13. PLEASE NOTE: You must call or go online to book using the discounted hotel benefit. Discounts will not be honored for walk-ins.

12 Automotive Care Savings Coworkers will receive savings on most of their car care needs. Whether it is an oil change, body work or work on your transmission, you can save money when you visit any of the following participating locations: Jiffy Lube- Receive a special fleet rate on Jiffy Lube s Signature Service which includes oil (up to five quarts), the installation of a new oil filter, checking the air filter, checking the wiper blades, vacuuming the interior, washing the exterior windows, and checking the brake fluid level. Meineke- Present your membership card at any of the 800+ participating Meineke Car Care Centers. You will receive a 5% discount on tires and batteries and a 11% discount on all automotive services. These savings are not valid with any current sale or promotional pricing. MAACO- Coworkers receive a 10% discount on all regular priced costs for paint and body labor. This discount does not apply on any special or sale pricing. AAMCO- when visiting a participating AAMCO, you will receive a 10% discount up to $100 on all regular priced costs of transmission service performed. This discount does not apply to any special or sale pricing. How to use the program 1. When having one of the services performed as listed above at the participating locations, present your RAC Co-Worker Program Membership Card. 2. At Meineke and MAACO you will need to present the front of the card prior to service being rendered in order to receive the discount. 3. AAMCO and Jiffy Lube locations will need your Account Number listed on the back of your card. 4. You must have your card with you at the time service is rendered in order to receive the savings.

13 Rental Car Savings Employees can save money whenever you need to rent a car from AVIS, Hertz, National, Alamo or Budget. Simply call the specific number and provide the special Discount Code the next time you make your rental car reservation. You will save between 5% - 25%. How to use the program 1. Choose from AVIS, Hertz, National, Alamo and Budget and you will receive a discount on your rental. -For AVIS call When you make your car rental reservation, mention discount code (AWD)-T to save from 5% to 10%. - For Hertz call When you make your car rental reservation, mention discount code to save from 5% - 20%. - For National call When you make your car rental reservation, mention discount code to save up to 20%. -For Alamo call When you make your car rental reservation, mention discount code to save up to 15%. -For Budget call When you make your car rental reservation, mention discount code BCD# - Z to save up to 25%.

15 Movie & Amusement Park Tickets Here s another way members can save money! Use the Discount Ticket form to order tickets at substantial savings for some of your favorite amusement parks and movie theaters. How to use the benefit 1. To order amusement park or movie theater tickets, print an order form from your RAC store. 2. Go to the RACINET homepage and click on Vendor Links under Quick Links. 3. Click on the Benefits Plus link for your line of business (RTO, California RTO, Puerto Rico RTO, Canada, or RAC Acceptance) 4. Click on Discount Ticket Order form and print. 5. Use the provided websites and discount codes to order tickets online or return the form to the indicated address with a cashier s check or money order for any of the tickets that have a charge. 6.
