For services offered on a detariffed basis, see also the AT&T Residential Service Agreement and the AT&T Business Services Agreement located at Customers can find Service Descriptions, Terms, Conditions, and Interstate Rates in the Company s Interstate Service Guides located at The Service Descriptions and Terms and Conditions in the Company's Interstate Service Guides also apply to intrastate services in Iowa. Under certain circumstances, funds provided under the American Recovery and Reinvestment Act of 2009 ( ARRA ) may be subject to certain restrictions, requirements and reporting obligations. AT&T may be subject to some of these restrictions, requirements and reporting obligations when Services and Service Components are purchased with ARRA funds. In order to comply with the restrictions, requirements and reporting obligations associated with the use of ARRA funds (if any), AT&T must be apprised of them before provisioning the Services or Service Components. Accordingly, the Services and Service Components provided under this Guidebook shall not be used to support the performance of any portion of a project or program which has been funded in whole or in part with grants, loans or payments made pursuant to the ARRA, without the prior written agreement of AT&T and Customer regarding any specifically applicable terms, conditions and requirements. Customer shall provide AT&T with prior written notice before placing any order that may be funded in whole or in part with ARRA funds. If Customer fails to provide such prior written notice of ARRA funding; or if the parties cannot agree on the terms and conditions (if any) applicable to an ARRA funded order; or if any terms, conditions or requirements (other than those to which AT&T specifically agrees in such separate writing) are found to be applicable, then AT&T may, in its sole discretion, reject such order or immediately terminate provision of any affected Service or Service Component without further liability or obligation.

4 d/b/a AT&T Long Distance Service Original Page 4 Effective: August 30, 2010 SECTION 4 RATES AND CHARGES 4.1 General The services in this Section of the pricing and service guide are intended for Residential and Business Customers. Alternatively, Business customers may subscribe to Complex Voice services found in Section 6 of this pricing and service guide, in lieu of services found in this Section 4. Customers are billed based on their usage of the Company's services. Rates may vary by service type, time of day, day of week, distance, and calling volume as indicated in the individual plan description. Customers will be charged for each individual call placed through the Company based upon the specific rate plan in this section subscribed to by the Customer. Call duration is determined as described in Section 3 of this pricing and service guide. For distance or time of day sensitive offerings, charges are determined based on mileage calculations and applicable rate periods found in Section 3 unless otherwise specified in this pricing and service guide. BellSouth Dial Direct service is available from originating locations within the state. BellSouth Toll Free service is available to Customers served from locations within the state. Operator Services are available from locations within the state where Customers have the ability to dial directly to the Company's network. Calls may be placed to locations within the state. The services found in this Section 4 cannot be combined with any services or plans found in Sections 6, 7, 8 and/or 9 for the same Customer on the same billing account.

5 d/b/a AT&T Long Distance Service 3rd Revised Page 5 Effective: June 12, 2014 SECTION 4 RATES AND CHARGES 4.2 Travel Service 1 Travel Service offers Residential Customers the ability to place calls using a Company-provided travel card when away from the home or office. Customers reach the Company s Travel Service network by dialing a Company-provided toll-free access code or number. In addition, the service provides Customers with optional operator assistance, alternative billing arrangements, and directory assistance using the same access code or number. Time-of-day and holiday discounts do not apply. Service is provided at rates and charges listed below: Application of Rates Initial Billing Increment: One Minute Additional Billing Increment: One Minute Recurring Charges: $0.00 Non-Recurring Charges: $0.00 Minimum Monthly Commitment: None Term Plan Available: No Directory Assistance Charges: $1.20 Per Call Directory Assistance Call Completion: $0.45 Per Call Intrastate Usage Rates: $0.35 Per Minute Intrastate Per Call Service Charges: One of the following per call service charges applies to each live or automated operator assisted call placed using the Company's Travel Service. Per call charges vary by type of call and type of billing method selected by the Customer. 6 d/b/a AT&T Long Distance Service 1st Revised Page 6 Effective: June 30, 2012 SECTION 4 RATES AND CHARGES 4.3 Reserved for Future Use 4.6 Residential and Non-Complex Business Pay Telephone Surcharge An undiscountable surcharge, as defined below, shall apply to each call originating from an instrument the Company identifies as a domestic Pay Telephone. Services for which a Pay Telephone Surcharge applies are defined in Section of this pricing and service guide. This charge applies to all applicable services defined in Section 4.0 of this pricing and service guide. Per Call Surcharge: $0.65

7 d/b/a AT&T Long Distance Service Original Page 7 Effective: August 30, 2010 SECTION 5.0 PROMOTIONS 5.1 Promotions - General From time to time, the Company shall, at its option, promote subscription or stimulate network usage by offering to waive some or all of the nonrecurring or recurring charges for the Customer (if eligible) of target services for a limited duration.
