13 HMO & PPO Referrals Currently receiving treatment with a specialist that was referred under the 2017 plan? Please notify your PCP and inform them of your new Anthem Blue Cross Health Plan. Remind your PCP that you are currently seeking treatment at a specialist and request another approved referral under the new Blue Cross HMO. PPO plans don t require a referral from their Primary Care Physician, they can choose any doctor without a referral even if the doctor is In or Out of Network. 13

15 HSA at a glance + + Health savings account (HSA) You ll have an account with PNC, our HSA partner. And you ll get a debit card in the mail to help pay for doctor visits and prescription drugs with money from your HSA. account Annual deductible You pay a yearly deductible before we start sharing the cost of covered health care you receive. You can use the money in an HSA to help meet your deductible. Major medical benefits (your percentage of the costs) After you meet your annual deductible, you pay a percentage of the costs for covered care. If you still have money in your HSA after you've met your annual deductible, you can use the funds to pay your percentage of the costs. Once you pay the annual maximum for your percentage of the costs, the plan pays 100% for everything else that s covered the rest of the year.
