If you were not referred, click on and a member number will be entered for you. Be sure the first radio button is selected, or you may not be added to Team Vitality. 2 Membership Information: Enter your personal information: Name, address, (to opt in to receive s), and phone number. Sign-In Information: Create a username, password (minimum 8 characters, one upper case, & one number), and 4 digit pin. WRITE THESE DOWN SO YOU WILL BE ABLE TO ACCESS YOUR ACCOUNT INFORMATION IN THE FUTURE. Tax Information: Select whether you are enrolling as an individual or a business and enter your social security or tax id number. Terms & Conditions: Read through the documents and check the box to the left. 2
