1 1st Revised Page 466 Cancels Original Page 466 SECTION 5 - SPECIAL PROMOTIONS, CONTRACT SERVICE ARRANGEMENTS 5.1 Special Promotions Prepaid Calling Service A. Special Promotions Carrier may, from time to time, waive or vary the rates and charges associated with certain services for promotional, market research or other similar business purposes. All promotional offers to Customers shall be the same as those stated in Carrier's Tariff No. 1 on file with the Federal Communications Commission. In no case shall the varying rates and charges exceed the rates and charges listed in the tariff for the same services. Carrier will provide a complimentary prepaid calling card to individuals who respond to, or are targeted by, marketing, advertising and retention programs sponsored by affiliates of Carrier. The prepaid calling card allows users to originate outbound, direct dial long distance calls via a toll free access number. All calls are rounded to the next higher full minute or unit. The prepaid calling card account shall expire on the date specified on the card or 180 days after first use, or in the absence of a physical card, on the marketing material accompanying the prepaid calling service offer. These offerings may be limited to certain dates, times and locations. The following specific rates, terms and conditions applicable to each promotional offering are described below. Promotional minutes awarded do not, under any circumstance, apply to INMARSAT calls. Global Private Line and Discount Waiver Global Private Line Service is hereby cancelled and withdrawn. 6 Original Page Contract Service Arrangements Carrier will offer Individual Service Arrangements to meet the diverse communications needs of Carrier's Customers. All terms and conditions as specified in this Rate Schedule will apply unless otherwise specified in the contract between Carrier and Customer. Contract Service Arrangement I Services - Large Business Voice Switched Access Service for Customer's location in California. All domestic and international usage will count toward the minimum annual commitment. Term - The term of this option is 1 year from commencement of service. Minimum annual revenue commitment of $24,000 per year is required. Rates and Charges - The following rates apply: Per Minute Rate China $0.807 Korea $0.570 Indonesia $0.608 Japan $0.390 Singapore $0.400 Rates to all other countries are detailed in this Rate Schedule in Section 7.2. No other discounts or promotions are applicable. Monthly Recurring Charges, Service Charges, and Surcharges will apply as defined in this Rate Schedule for Large Business Voice Service. Calling Card and Operator Assisted calls are subject to the prevailing charges for Large Business Voice Switched Access Service in applicable tariffs. 7 Original Page Contract Service Arrangements (Continued) Contract Service Arrangement II Services - Large Business Dedicated and Switched Access Voice Service to be provided to Customer with locations in Illinois and New Jersey. All domestic inbound and outbound usage, including Directory Assistance, Operator Assisted calls and surcharges, Calling Card usage and surcharges will count toward the Minimum Annual Commitment. Outbound International usage and Monthly Recurring Charges will also contribute to the Minimum Annual Commitment as defined in this Rate Schedule for Large Business Voice Service. Term - The term of this option is 2 years from commencement of service. Minimum annual revenue commitment is $1,500,000 per year. Rates and Charges - The following rates and charges apply to all Dedicated Outbound International calls for specified countries. Dedicated Outbound International Voice Service Country Per Minute Rate Panama $.69 Venezuela.43 Colombia.58 Spain.27 Ecuador.80 Portugal.37 India.77 Dominican Republic.50 Pakistan 1.01 Bangladesh 1.26 Peru.58 Nigeria.82 Senegal 1.29 China.85 No other discounts or promotions are applicable. Calling Card, Operator-Assisted, Directory Assistance, International calls, other than specified above, are subject to the then prevailing charges for Large Business Voice Service in this Rate Schedule. Unless otherwise stated above, all terms and conditions of Large Business Voice Service as defined in this Rate Schedule apply to this Customer. Term - The term of this agreement is one year from commencement of service. Minimum annual commitment of $10,000 per year is required. Rates and Charges - The following per minute rate will apply to all direct dialed calls, calling card calls and operator assisted calls. Surcharges will apply as defined in Carrier's standard offering. Customer will be charged $.12 per minute for international outbound calls. All domestic calls are at tariffed intrastate or Carrier's posted standard interstate rates. Availability CSA V is no longer available to new customers.
