For every goal in the list below, we will ask you to enter a target number: 1. Come to weigh-ins. (Remember that there are 3 weigh-ins remaining) 2. Attend David Barton Gym times during the free month membership. 3. Attend events of the LiveSmart program including Total Body Blast, Cardio Bollywood or one of the two nutrition seminars. This is entirely voluntary: you can choose not to set a goal. If you do not want to set a goal, please select 0 for all these options and you will receive the $20 irrespective of your performance in this challenge. You can also set a single goal or choose multiple goals. However, whatever you decide, your decision will be strictly enforced. Example: Suppose that you enter 2 in goal 1. and 0 for the other two options. Then you will only receive the gift card if you came to at least 2 weigh-ins during the rest of the challenge. In order to verify whether your goal(s) was (were) attained, your attendance will be recorded with the collaboration of David Barton Gym and LiveSmart. Now please select your answers. Part 3: Reimbursement of your meals As mentioned in the introduction, 10 participants in this study will be randomly drawn to receive reimbursement of all their lunch meals over the 4 week period starting on April 1st and ending on April 28th. You will be reimbursed for a maximum of 20 meals and for up to $15 per meal ($300 value). In order to be reimbursed, you will need to save all your receipts and bring them to the last weigh-in on Tuesday, April 29th. Important: You will need to bring detailed receipts of your order with your name on it so please purchase your lunch with a credit card. The winners will be announced at the award ceremony one week later, on Tuesday, May 6th. The reimbursement will cover one, two or all three of the following lunch categories, depending on your choices below and on chance: Green Category G : all salads (including regular, kale and quinoa salads) or soups (including veggie and noodle soups) natural fruits or low-fat yogurt Page 4 of 11

5 water (spring or sparkling) Orange Category O : all sandwiches (including bagels, wraps, baguette, club and hot sandwiches) cereal bars, fruit bars or trail mix fruit juice Red Category R : all burgers, pizzas or fried foods (including French fries, Chicken wings and barbecue) all pastries (including cookies, cakes, muffins, donuts and croissants) soda We now would like to know which of these lunch categories you would like to be reimbursed for. You can be potentially reimbursed for one, two or all three of the lunch categories described above. - For instance, you may prefer to restrict the reimbursement to cover only one lunch category. In this case, you would not be reimbursed for an option belonging to any of the other two categories. These are the reimbursement options G, O and R. - Alternatively, you may prefer a reimbursement that covers all three categories. This is the reimbursement option GOR. - As an intermediate case, you may prefer to partially restrict the reimbursement to cover only two lunch categories. These are the reimbursement options GO, GR and OR. Therefore, there are 7 reimbursement options in total. The only difference between all 7 reimbursement options is the range of lunch items covered by the reimbursement. Otherwise, the reimbursement conditions are exactly the same: you will need to bring detailed receipts showing your name and the items you ordered at the last weigh-in and you will be reimbursed in cash on May, 6th in case you won. Notice however that being reimbursed only for a certain category of foods may give you an incentive to eat more of these foods and less of the foods not covered by the reimbursement. We now ask you to rank all 7 possibilities according to your preference. On your screen, all reimbursement options will appear listed in a random order. You will be asked to first select your favorite option(s), that is, the option(s) you wish to assign rank 1. You can select multiple options at a time if you are indifferent between these options. Once you have made your selection, you will be asked to rank the remaining options in the list, starting with your rank 2 option(s). Again, you can give rank 2 to multiple options if you are indifferent between them. The process stops once the list of options is empty and you assigned a rank to all of them. Simply click on the continue button once your first empty screen appears. The reimbursement option that will be assigned to you at the end of this survey will be randomly determined. However, the odds that you are assigned a given reimbursement option will depend on the ranking you provided. You will have a higher chance to obtain the options you ranked higher. If you are indifferent between several options, they will be selected with the same percentage chance. If you want to know more about the selection procedure, please click on the button at the bottom of this page once you have read the reminders below. You will be informed of the reimbursement option you were assigned after you finished completing this first survey. 6 At this point, if you have any question, please feel free to contact Severine Toussaert at Learn more about the selection procedure Selection Procedure To determine the reimbursement option you will receive, you will be entered in a lottery composed of 100 lottery tickets, each ticket giving right to one of the 7 reimbursement options. The number of lottery tickets corresponding to a given reimbursement option will depend on its rank: the higher the rank, the larger the number of lottery tickets. As a result, you will have a higher chance to receive the option you ranked first than the option you ranked second and so on. The number of lottery tickets corresponding to each rank is given in the table below: Rank Number of lottery tickets Total 100 For instance, you will receive the reimbursement option you ranked 3rd with a 20% chance and you will receive one of your Top 3 preferred options with an 85% chance. On the other hand, there is no chance that you will receive your least preferred option. Notice that since the chances of being assigned a given reimbursement option will depend on its rank, it is in your interest to assign a higher rank to the options you like better. What if you assigned the same rank to several options? For instance, suppose that you assigned rank 1 to 3 options and then assigned rank 2, 3, 4 and 5 to the remaining options. Then the system will pick at random among your top 3 options the option which is assigned 35, 30 and 20 lottery tickets respectively. If you are selected, the gift card will be sent to you electronically either today or in the coming weeks. On each such night, you and a companion will get the best table and an unlimited budget for food and drink at a restaurant of your choosing. There will be no cost to you: all payments, including gratuities, come as part of the prize. The certificates are available for immediate use, starting tonight, and there is an absolute guarantee that they will be honored by any restaurant you select if they are used within a two-year window. If they are not used up within this two-year period, however, any that remain will be valueless. The questions below concern how many of the certificates you would ideally like to use in each year, how tempted you would be to depart from this ideal, and what you expect you would do in practice: (a) From your current perspective, how many of the 10 certificates would you ideally like to use in year 1 as opposed to year 2?
