Accident Report Forms Old (Do not use) New

Waste Disposal Make sure nothing spills on the outside of the bottle Stockroom can bring down a funnel if needed Make sure waste bottles are closed between uses This includes the drum funnels Do not put anything but water in the sinks All chemicals are to be disposed of in waste containers **CHM 102/114 and 105 must monitor barrels, do not let students overflow them! Call stockroom before they are full so a replacement barrel can be ordered!

Waste Disposal Put all waste into proper waste container -- If lab requires more than one waste bottle, educate the students on each bottle s contents --If non-compatible substances are mixed in a waste bottle, take the cap off, close the fume hood and call stockroom for help! **CHM 226 TAs must constantly remind students to dispose waste in proper containers, many chemicals are incompatible

Waste Labels & Containment Waste labels must be kept neat Make sure bottles have legible hazardous waste labels If the label gets ruined, call the stockroom for a replacement Chemical formulas may not be used on a hazardous waste label Do not date waste bottles Do not transfer waste between labs Make sure you have enough waste containers If not, call the stockroom to bring down a new container before the old one is full Do not overfill waste bottles. Do not put trash in the drum spill pallet or secondary waste containment All waste bottles must be in secondary containment Do not store incompatible wastes in the same containment tray.

Hazardous Waste Satellite Accumulation Area (SAA)-Label all bottles at first use!!! Contact the Stockroom when bottles are nearly full to acquire a new bottle. Keep spill clean-up supplies on hand. 1. The waste area must be identified by a sign. 2. The waste area must be at or near the point of generation. 3. Store waste away from chemical stock storage areas. Keep bottle closed. When not adding waste, REMOVE FUNNEL AND CLOSE Do not overfill containers. Leave head space. Correct Hazardous Waste Labels Use appropriate PPE NEVER PUT IN USE CHEMICALS IN THE WASTE PAN Place waste in secondary containment tray to prevent uncontrolled spills. Containers must be in good condition.

Hazardous Waste Labeling 1.The label must say hazardous waste. 2.Use full chemical name, no abbreviations 3.Check the appropriate hazard. 4. If a chemical is missing from the label, notify the Stockroom for a replacement label.

Lab Maintenance Burets Balances LabQuests Hot Plates ph Probes Etc. When you enter the lab: Check the chemicals, lab equipment, waste bottles, paper towels and any other communal items When you leave the lab: Turn off lab equipment and computers, check to make sure waste isn't overflowing, make sure lab benches are clean, check that all drawers are locked and lock doors

LabQuest Systems Temperature Probes: Please be sure to tell your students to use caution in tapping the screen. If they press too hard the screen can break. Graph Data Table Rate/Time/Mode

LabQuest Systems Spectrophotometer: ** For help with the LabQuest systems you can contact the stockroom

Chemical Hazards Please be conscious of chemical hazards. Make sure students realize the severity of hazards when working with chemicals SDS information will be available in the lab Familiarize yourself with chemicals and the associated SDS you will use in the lab For chemical injuries, give EMS the multi page SDS Be familiar with NFPA Ratings & be able to explain them to students

Global Harmonization System (GHS) Health/physical/environmental hazards

Chemical Spills Isolate & contain spill! If a small spill occurs, use a spill pad to absorb it If you need more than 2 spill pads to fully clean up the area, call the stockroom to report the spill and get some help. Certain materials will require more help, such as concentrated acids, bases etc. If the spill is very large, use the spill kit if necessary. Isolate the area Call the stockroom for help Dispose of all spill pads & spill response materials in the appropriate waste container. *Note: Spill cleanup materials are also considered hazardous waste This includes contaminated clothing!!!

Chemicals and Equipment Get to lab early to check on prep Call the stockroom immediately if you need something so they can bring it down before you start the experiment. Call early, so items can be delivered promptly Do not send students to retrieve chemicals at the stockroom only send them down if they need equipment Also, do NOT have students bring broken glassware down to show us Assist the student in disposing of the item in the correct containers in lab Make sure you put the item in the online billing system

Student Attendance Account for all students! Take attendance using the computer attendance program Instead of computers there are Chrome boxes setup under the front counters They work like a computer but the monitor is the TV

Student Attendance If the students name is Red- they cannot be in lab, they have not completed the online safety training quiz and won t be allowed in lab Yellow- they have a problem with their compliance form Orange- means they have a bill with the stockroom that must be resolved by the end of the semester

What if my student is not on the Roster? Make sure the student is in the right room. If student is waitlisted for your section have them complete the safety training and the TA should clearly write waitlist on the top of their compliance form Have them fill out compliance & medical forms. Clearly write waitlist on the top of the form Write what section they are supposed to be in if it s a make up Have the students not on the roster leave the lab before checking into lab drawers

Safety Training Makeups

Compliance Forms Each student must fill one out with their names written legibly! Forms are a permanent record that are kept for 7 years Students must use a blue or black ink pen Must be proper and COMPLETE Plagiarism policy must be signed and dated PLEASE check all compliance forms before the students leave lab

Common Compliance Form Errors Student didn t write their name on plagiarism policy Student checked boxes instead of initialing Student used pencil instead of pen TA didn t write TA name on compliance form Student didn t initial billing bullet TA wrote the wrong section TA didn t sign and date the compliance form Didn t mark if student was waitlisted Students do not understand the terms of the compliance and plagiarism forms

Incorrect Compliance Forms Failure to complete properly results in student being unable to be marked present in subsequent labs If a students name is red or yellow it requires immediate attention If its red have them take the online safety training module in the hallway If its yellow send them to the stockroom to fix their compliance form If a students name is colored at all remind them what the color means

Note: Unpaid bills will result in a hold on e-campus and reminders w week!

This is the department copy of the plagiarism form. It is on the back of the compliance form. Make sure the form is brought to the stockroom after check in!

Plagiarism Forms There are two copies of the plagiarism form. One is the department copy and the other is the student copy. They are different colors! Make sure that you do not give the stockroom the student s copy of the plagiarism policy. This is for them to reference. Encourage them to hold on to it so they are fully aware of our policy. Ensure students understand the university and departmental policies before having them sign

This is the student copy of the plagiarism form. Note it is a different color. Encourage them to keep these forms so that they are aware of the policy.

Compliance Forms Quick Quiz! What is the mistake on the following forms?

Checking into Drawers Only assign drawers to students who are on the computer roster Write waitlist if the student is waitlisted or the correct section for students who are makeups on the student's compliance form if they are not in the computer rooster Have students check their drawer for completeness. This means take everything out and put back only what is on the equipment list. If anything is missing please send them to the stockroom to get the new items

After Safety Training When finished: Collect the compliance forms Ask students questions about the form contents to ensure the student understands Look over the completed compliance form before the student leaves lab. Make sure it is correct and complete! Immediately following class take ALL compliance forms to the stockroom.

Reporting M/B Equipment If an item is found missing or broken within first 30min of Lab. *CHM 102 and CHM 114 have 1 hour at the start of lab Due to the time spent on the concept review Check to make sure item really is missing or broken Fill out MISSING/BROKEN request form online Send student to the stockroom immediately for a new item In this situation the student will not be charged for said missing/broken item as they have not used equipment yet. If an item is found dirty, please have your student rinse it out with water. If it can't be cleaned, mark it as broken. Students should clean glassware when finished with experiment! TA should check all communal equipment in the lab for breakage prior to the class. Be observant during your lab and report any breakage of communal equipment to the Stockroom (provide the student name, ID # and description of broken equipment)

Avoiding Theft from Student Drawers Inform CHM 212, 226 and 425 students that they own their drawer contents for the semester Rooms 120, 130, 135, 140, 145, 150, 155, 165 Remind all students to lock their drawers They will be held responsible for the contents of the drawers. All drawers have combination locks and students should be given the combination during check-in Combinations to all in-use drawers can be found in TA Book located in the TA drawer *Please use discretion with these combinations

Student Safety Training Students must complete an online safety quiz to demonstrate that they understand departmental safety policies Students will NOT be allowed to participate in lab until the safety quiz has been completed If it is not completed their name will be colored red on the attendance screen.

Stockroom Policies Stockroom Hours are 8am -5pm If something is broken the TA should immediately report the item via the online system and the student should immediately come to the stockroom to replace the item.
