27 SECTION 20 Operations and Maintenance Manual - Section Quarterly Reports Each quarterly monitoring report will provide information on activities occurring during the quarter in question (January 1 to March 31, April 1 to June 30, July 1 to September 30, October 1 to December 31) and will be submitted no later than 60 days after the last day of the quarter in question. All quarterly reports will be submitted on the forms provided by the NYSDEC or electronically as specified by the NYSDEC and shall also contain the following; 1. Report on funding levels of closure and post-closure funds and evidence that proper financial assurance mechanisms are in place. 2. Amounts of leachate collected by each primary and secondary leachate collection systems on a daily basis. 3. Amount of leachate removed from the leachate tanks on a daily basis and the disposal location. 4. Amounts and dates of fluid removed from the leak detection monitoring risers (double contained tanks and double contained leachate lines). 5. Amounts of waste (mixed municipal, water and wastewater sludges, industrial waste & sludges, ash, C&D debris, asbestos, compost, yard wastes, contaminated soils) received from each New York State County, from each State and from outside of the Country. 6. Amounts of landfill gas condensate generated at the flare knockout tank, the disposal location, and the number of landfill gas collection wells in operation. 7. Groundwater and surface water quality results. 8. The amount and types of waste utilized as alternate daily cover. 9. The results of the methane gas migration monitoring program required by 6 NYCRR Part (f)(2). 10. The results of the monitoring of the landfill surface in accordance with 6 NYCRR (f)(3). Any remediation or required modifications shall be reported as well. 11. Odor Complaint records as applicable.

125 Quarterly Gas System Report Third Quarter 2014 The C.I.D. Gas Facility has three condensate knockouts on the Existing landfill, a knockout on the main header running from the landfill to the flare, and a knockout on the 18 inch header located in the northwest corner of the Western Expansion. The three knockouts on the landfill remove condensate from the landfill gas and drain into existing leachate collection tanks. Due to this configuration, quantities of condensate passing through these knockouts are unavailable. The main header, however, employs a condensate knockout that drains to its own condensate storage tank. The condensate levels are read each week by stick measurement and emptied as needed. The knockout in the northwest corner of the Western Expansion drains into a vertical sump and is pumped through into the leachate force main and ultimately into the leachate storage tanks. Flows are tracked via flow meter and flows are recorded daily. All condensate is pumped into a tanker truck and taken to either Buffalo Sewer Authority or Niagara Falls Wastewater Treatment plant for disposal. Total Condensate generated for this Quarter: 11,147 gallons Total gallons treated at Buffalo Sewer Authority 0 Total gallons treated at Niagara Falls W.W.T.P. 11,147 gallons Currently there are 65 active gas wells on the existing site and 15 active wells in the Western Expansion. All of these wells are at the surface and monitored on a monthly basis. The entire collection system is also monitored monthly.

(CRA) conducted the third quarter surface monitoring at the Waste Management of New York, LLC - Chaffee Landfill facility located in Sardinia, New York. The monitoring was conducted in accordance with Code of Federal Regulations (CFR) Title 40, Part 60, Appendix A, Method 21. Mr. Michael Casullo and Ms. Cassidy Edwards of CRA performed the calibration evaluation and monitoring using a Thermo Environmental Instruments Toxic Vapor Analyzer (TVA) 1000 flame ionization detector (FID) to determine surface methane levels. Attachment 1 contains the monitoring instrument performance evaluation and calibration documentation. The monitoring followed a serpentine pattern at 30-meter intervals, as required in 40 CFR Part 60 Subpart , and generally followed the pattern described in the Surface Monitoring Plan for the facility. In addition, all ground penetrations were monitored when encountered, which resulted in frequent diversions from the general surface-monitoring path. The fourth quarter of 2011 marked the third consecutive quarterly monitoring period with no recorded exceedances of the 500 ppm limit for the Closed Landfill. Therefore, surface monitoring for this area is now conducted annually as stated under 40 CFR (f): Each owner or operator seeking to demonstrate compliance with (c), shall monitor surface concentrations of methane according to the instrument specifications and procedures provided in (d). Any closed landfill that has no monitored exceedances of the operational standard in three consecutive quarterly monitoring periods may skip to annual monitoring. Any methane reading of 500 ppm or more above background detected during the annual monitoring returns the frequency for that landfill to quarterly monitoring. CRA will perform the 2014 annual monitoring of the Closed Landfill later this year (date has yet to be determined). 131 November 13, 2014 Reference No CRA monitored Cell 5 and Cell 6 of the Western Expansion Landfill during the third quarter surface scan (these landfill areas are monitored quarterly). During the first quarter surface scan, one exceedance was observed at the following location: Cell Location Reading (ppm above background) 5 GCW-68 3,863 The area was marked with a yellow flag and the site Operations Manager was immediately notified. Figure 1 shows the location of the monitoring exceedance and Attachment 2 contains the data sheet giving a description of the exceedance and the follow-up monitoring. The following corrective actions were implemented immediately to address the exceedances: Additional vacuum was applied to GCW-68 On October 3, 2014, Mr. Michael Casullo of CRA performed the 10-day follow-up. During the 10-day follow-up, the GCW-68 location exhibited readings lower than the 500-ppm above background limit. On October 24, 2014, Mr. Michael Casullo of CRA performed the 30-day follow-up. No readings exceeding the 500-ppm limit were detected at the GCW-68 location, and no further action is required for the 2014 Q-3 monitoring period. The remaining cells of the Western Expansion Landfill (Cells 3 and 4) are not yet subject to New Source Performance Standards (NSPS) since the age of the waste in these areas is less than 5 years. The Chaffee Landfill facility is in compliance with the requirement of 40 CFR (d) for Municipal Solid Waste (MSW) Landfills.
