Pricing for wastes destine for combustion at Perma-Fix/DSSI is adjusted based on waste stream characteristics. Sampling and Analysis will be performed in accordance with the Perma-Fix/DSSI Facility s Waste Analysis Plan and procedures. 12 ADDITIONAL SURCHARGES, IF APPLICABLE In order to ensure that Perma-Fix facilities permits and licenses are met, certain non-conformances may be identified and corrected. Item or Discrepancy Container Labeling Documentation Container Cleaning Overpacking Beryllium or Suspect Beryllium Expedited Shipment Approvals Definition/Description All labels placed onto containers for the purpose of proper waste identification or applicable DOT shipping requirements. Any document that is required by state, federal, local governing agencies and those required per Perma-Fix Waste Acceptance Criteria and must be corrected to meet applicable requirements. Containers which arrive at Perma-Fix facilities that do not meet currently implemented waste storage requirements that as a result may not be returned to public highways. Containers which cannot meet Perma-Fix receipt and storage criteria through cleaning and which have integrity concerns affecting safe, proper, permit and licensecompliant storage. Waste containers which require the implementation of the site Beryllium Plan will be subject to a surcharge. Shipment approval requests less than 5 days of receipt date cause significant disruption in facility operations and will be invoiced as a surcharge.
