Ran 30 centralizers on casing from TD to Wash casing to bottom from to Circulate and conditioned hole for 4-1/2 hours and WO O-Tex. RU O-Tex and Pro Petro nitrogen. Pumped 200 bbl mud with 200 SCF nitrogen per bbl. Cement well with 300 sx 50:50 Class H + 8% gel + 5% salt + 3 pps Kolseal + ¼ pps Celloflake +.3% C20 +.4% CF41 L (mixed at 12.2 ppg with a yield of 2.12 cu ft/sk) followed by 700 sx Class H + 2% Gel +.9% C16A +.2% C pps Celloflake gps CF % C-44 ( mixed at 14.4 ppg with a yield of 1.23 cu ft/sk). Plug down at 8:40 pm 5/8/2012. Had full returns throughout job. Set slips with 136 K. Cut off casing. Released rig at 12:45 am 5/9/2012. 6 06/11/2012 Day #2 Unload, rack, and tally 355 joints of 2 7/8" L-80 tubing. Rig up pulling unit - ND 7 1/16" x 5000# WH and NU a rental 7 1/16" x 10,000# WH with a spool for the BOP. NU BOP & RIH with 4 3/4" skirted workover bit, bit sub, and 10 jnts of 2 7/8" L-80 tubing. Lined up pick up machine for tomorrow morning to finish in the hole with the 2 7/8" tubing string. Picked up and made up 2 3/8" swab line off of well. RU swab equipment and got ready to swab well. DWC: $23,585 CWC: $28,735 06/12/2012 Day #3 Rig up pick up and lay down machine. Pick up 2 7/8" L-80 tubing off of pipe racks and RIH to 11,274' tagged up on PBTD. Tagged up on joint # /2" casing is set at 11,314' with (1) 40' shoe joint on bottom. Pulled tubing up hole to 9004' - RU swab equipment & RD pick up machine. Swabbed fluid level down to 1000' from surface and SDFN. DWC: $7090 CWC: $35,825 06/13/2012 Day #4 Open up well and resume swabbed down fluid level in wellbore. Set in (6) 500 blls frac tanks and started filling with fresh water. Made 26 total swab runs and swabbed down to 6900' - last run swabbed from 8400'. SI well and SDFN. DWC: $11,250 CWC: $47,075 06/14/2012 Day#5 Filled (6) frac tanks with fresh water - open up well and resume swabbing. IFL at 6900' from surface - made 6 swab runs and swabbed fluid down to 8500'. POH with 277 jnts of 2 7/8" tubing, bit sub, and 4 3/4" bit. RU Axis Wireline and RIH with 3 1/8" slick guns - Perf Devonian 11,078', 79', 80', 81',& 11,082'; perf'd 11,048', 49, 50, 51, 11,052'; perf'd 10,984', 85', 86', 87', 10,988'; and perf'd 10,934', 35', 36', 10,938' - 64 total holes. POH with guns all shots fired. RD Axis WL and shut in well. SDFN. DWC: $10,530 CWC: $57,605 06/15/2012 Day #6 Open up well - tubing was on a slight vacuum. PU & RIH with a 5 1/2" Klein K-3 compression packer on 333 jnts of 2 7/8" L-80 tubing. Set packer at 10,829' with 25 points of compression. Shut in well and SDFWE for pulling unit repairs. DWC: $5100 CWC: $62,705 06/18/2012 Day #7 Open up well - 25# SITP - bled off gas preesure. RU Crane Acidizing on tubing - tested lines and start out on water. Loaded tubing with 42.7 bbls and broke down formation at 3483# pumping 1.8 bbl./min rate. Increased rate to 3.9 bpm at 3350#. Switch to acid and pumped 3000 gals of 15% NeFe HCL with 80 ball sealers. Max TP 3483# with Average TP at 2400#. Max Rate & Average 6

7 Rate was 4.3 bpm. ISIP at 1502# - 5 minute SI 156# and tubing on a vacuum in 6 minutes. Total load was 177 bbls -RU to swab well. IFL was 400' from surface swab line broke - cut sand line and pour rope socket. Made 12 swab runs with 4000' - lots of acid gas during and after each run. Lay down swab equipment and released packer. Start out of the hole with the 2 7/8" tubing string. Load to be recovered = 112 bbls. Pulled out with 10 joints and SDFN. DWC: $16,547 CWC: $79,252 06/19/2012 Day #8 Open up well - 14 hour SI TP - 120#. Open up well and bled gas off of tubing in 2 1/2 minutes with no fluid. RU swab equipment and RIH ' (2000' of FE in 15 hours) sample on run #1 spent acid and water. Made 2 swab runs and recovered approximately 10 bbls of fluid - no oil (102 BLWTR). RD swab equipment and resume POH with 2 7/8" tubing and 5 1/2" packer - laying tubing down in singles on pipe racks. SI & SDFN. Prep to NU frac valve in the a.m. DWC: $5240 CWC:$84,492 06/20/2012 Day #9 Open up well - 110# SI TP - bled off gas pressure in 6 1/2 minutes - no fluid. ND Basic BOP and companion flange. NU 7" - 10K Rental wellhead (J & W) clean location and RD Basic Pulling unit. DWC: $4500 CWC: $88,992 06/22/2012 Day #10 RU Cudd Energy Services to sand Frac Devonian perfs via 5 1/2" casing with 30/50 sand. Shut in TP - 219# Loaded hole with 66 bbls #. Pumped 2421 bbls of 20# gelled water carrying 97,000# of 30/50 white sand and 28,000# of SDC resin coated sand. Pump via 5 1/2" casing at 70 bpm rate. ISIP 2350# with a 5 minutes SI pressure of 1964#..10 minutes well had 1871# SI pressures. Left well shut in overnight. Built a blow down header to frac tanks. 6:30 pm well had 1180# SI casing pressure. DWC: $139,000 CWC: $227,992 06/23/2012 Day #11 Arrive on location at 7:30 am - well had 750# of SI CP. Open up well to frac tanks on 21/64" choke. Well flowed back 147 bbls of fluid the first hour. 10 am well open on 21/64" choke with 210# FCP - recovered a total of 203 bbls. Closed choke to 18/64" choke. 11 am well open on 18/64" choke recovered 20 bbls total bbls recovered this am. Well died at 12:04 pm 0# CP recovered 10 bbls of fluid in the last hour - total recovery is 233 bbls. Shut in well for build up. 3 pm well had no pressure - open up well - no fluid. Shut in well. DWC: $4500 CWC: $232,492 06/25/2012 Day #12 Open up well - 0# SICP with no flow of gas or fluid. RU Basic Well service & ND frac valve. NU 7 1/16" x 5K well head with pull thru tubing hanger flange. NU BOP and RIH with bull plugged mud anchor 2 7/8" x 32', 2 7/8" perf sub, 2 7/8" SN, 6 joints of 2 7/8" tubing, 5 1/2" TAC, and 125 joints of 2 7/8" tubing. SI well and SDFN. 7

8 DWC: $12,550 CWC: $245,042 06/26/2012 Day #13 Open up well - tubing on slight vacuum and nothing on the casing. Rig up pick up and lay down machine and continued in the hole with the 2 7/8" L-80 production tubing string. Ran a total of 333 joints of 2 7/8" L-80 tubing above the 5 1/2" TAC (339 jnts to SN). SN depth is 11,027' with TAC at 10,833' and BOT at 11,062'. Top Devonian perf is 10,934' and bottom perf is 11,082'. - tagged fill at 11,098'. RD Globe and ND BOP. NU hanger flange with 2 7/8" slips - string weight 78,000#. Set TAC with 19 points of tension and NU WH. RU swab and found 1600' from surface. Run #2 1800' fs. Run #3 and #4 1900' fs. Made 9 swab runs with the 2500' no gas after each run - recovered approximately 48 bbls ( 2242 BLWTR). SI well and SDFN. DWC: $9650 CWC: $254,692 06/27/2012 Day #14 Open up well - tubing on a slight vacuum and lite blow of gas on casing. RU swab equipment and RIH. Found IFL at 1900' from surface (600' of fluid entry) - no oil or gas shows. Made 27 swab runs 2900' with no oil or gas shows. Recovered 135 bbls of water (2107 BLWTR) SI well and SDFN. Released pipe racks, cat walk, BOP, and frac tanks picked up off location today. DWC: $6900 CWC: $261,592 06/28/2012 Day #15 Open up well - 40# tubing pressure & 90# of casing pressure. Bled off with no fluid to surface. RU swab found 2100' from surface (800' of fluid entry overnight) with a skim of oil on first run. Unloaded steel rods and RD swab equipment. PU & RIH with 1 1/4" x 16' gas anchor, 2 1/2" x 1 1/4" x 24' RHBC pump with a HVR and a 6' grooved plunger, spiral rod guide, 6) 1 1/2" K- Bars, 204) 3/4" steel rods (high alloy D grade) with FH tee couplings, 120) 7/8" steel rods (high alloy D grade) with FH tee couplings. SI well and SDFN. DWC: $48,200 CWC: $309,792 06/29/2012 Day #16 Open up well - 40# SITP and 170# SI CP. RIH with 108) 1" grade D steel rods, 1) 8' x 1" pony rod, 1) 4' x 1" pony rod, and a 1 1/.2" x 26' polish rod with a 1 3/4" x 14' liner. Spaced out pump and hung off on the stuffing box waiting on pumping unit to be set. RDPU and moved off location. DWC: $3000 CWC: $312,792 8
