I. Overview This nnual Grounwater Monitoring Report (Report) has been prepare to report the status of activities for the preceing year for an existing Bottom sh CCR unit at ppalachian Power Company s, a wholly-owne subsiiary of merican Electric Power Company (EP), John E. mos Power Plant. The USEP s CCR rules require that the nnual Grounwater Monitoring Report be poste to the operating recor for the preceing year no later than January 31, In general, the following activities were complete: Monitoring wells that were previously installe an evelope were certifie as the grounwater monitoring system aroun the CCR unit, in accorance with the requirements of 40 CFR 2.91 pursuant to EP s Grounwater Monitoring Network Evaluation Report (March 2017); Grounwater samples were collecte an analyze for ppenix III an ppenix IV constituents, as specifie in 40 CFR 2.94 et seq. an EP s Grounwater Sampling an nalysis Plan (July 2016); Grounwater ata unerwent various valiation tests, incluing tests for completeness, vali values, transcription errors, an consistent units; Backgroun ata for each ppenix III an ppenix IV constituent were establishe; Detection Monitoring sampling was initiate; an EP s Statistical nalysis Plan (pril 2017) was prepare, certifie, an poste to EP s CCR website in pril The Statistical nalysis Plan (pril 2017) is a statistical process in accorance with 40 CFR 2.93 to evaluate grounwater ata. The statistical process was guie by USEP s Statistical nalysis of Grounwater Monitoring Data at RCR Facilities, Unifie Guiance ( Unifie Guiance, USEP, 2009). Data evaluation is uner way. The major components of this annual report as specifie in 40 CFR 2.90, to the extent applicable at this time, are presente in sections that follow: figure showing the CCR management unit(s), all grounwater monitoring wells an monitoring well ientification numbers; Ientification of any monitoring wells that were installe or ecommissione uring the preceing year, along with a statement as to why that happene; ll of the monitoring ata collecte, incluing the rate an irection of grounwater flow, plus a summary showing the number of samples collecte per monitoring well, the ates the samples were collecte an whether the sample was collecte as part of etection monitoring or assessment monitoring programs (ttache as ppenix I); 1

summary of any transition between monitoring programs or an alternate monitoring frequency, for example the ate an circumstances for transitioning from etection monitoring to assessment monitoring, in aition to ientifying the constituents etecte at a statistically significant increase over backgroun concentrations (ttache as ppenix II, where applicable); an Other information require to be inclue in the annual report such as alternate source emonstration or assessment of corrective measures, if applicable. In aition, this report summarizes key actions complete, an where applicable, escribes any problems encountere an actions taken to resolve those problems. The report inclues a projection of key activities for the upcoming year. 2

II. Grounwater Monitoring Well Locations an Ientification Numbers The figure that follows epicts the PE-certifie grounwater monitoring network, the monitoring well locations, an their corresponing ientification numbers. The grounwater quality monitoring network inclues four upgraient wells (MW-6, MW-1601, MW-1602, an MW- 1603) an 6 owngraient wells (MW-1, MW-4, MW-5, MW-1604, MW-1605, an MW- 1606). The monitoring well istribution aequately covers owngraient an upgraient areas as etaile in the Grounwater Monitoring Network Evaluation Report that was place in the merican Electric Power CCR public internet site on March 9,

III. Monitoring Wells Installe or Decommissione There were no monitoring wells installe or ecommissione in The network esign, as summarize in the Grounwater Monitoring Network Evaluation Report (March 2017) an as poste at the CCR web site for John E. mos Plant, i not change. That evaluation report, viewable on the EP CCR web site, iscusses the facility location, the hyrogeological setting, the hyrostratigraphic units, the uppermost aquifer, an the esign of the grounwater monitoring well network incluing owngraient an upgraient monitoring well locations. IV. Grounwater Quality Data an Static Water Elevation Data, With Flow Rate an Direction ppenix I contains tables showing the grounwater quality ata collecte uring the establishment of backgroun quality an the first etection monitoring event. Static water elevation ata from each monitoring event also are shown in ppenix I, along with the grounwater velocity eterminations, grounwater flow irection, an potentiometric maps evelope after each sampling event. V. Discussion bout Transition Between Monitoring Requirements or lternate Monitoring Frequency s of this first annual grounwater report ate there has been no transition between etection monitoring an assessment monitoring. Detection monitoring will continue in The sampling frequency of twice per year will be maintaine for the ppenix III parameters (boron, calcium, chlorie, fluorie, ph, sulfate an total issolve solis). Regaring efining an alternate monitoring frequency, the grounwater velocity an monitoring well prouction is high enough at this facility that no moification of the twice-per-year etection monitoring effort is neee. VI. Other Information Require t the appropriate time, the geochemical analyses couple with the statistical analyses of the grounwater quality ata will etermine whether an alternate source or alternate sources are affecting grounwater chemistry. In those cases where an alternative source(s) emonstration is mae, those analyses an supporting information will be presente as well. 4

VII. VIII. Description of ny Problems Encountere in 2017 an ctions Taken No significant problems were encountere. The low-flow sampling effort went smoothly an the scheule was met to support this first annual grounwater report preparation. Projection of Key ctivities for the Upcoming Year Key activities for 2018 inclue: Detection monitoring on a twice per year scheule; Evaluation of the first etection monitoring results from a statistical analysis viewpoint, looking for any statistically significant increases, or ecreases when ph is consiere; Responing to any new ata receive in light of what the CCR rule requires; an Preparation of the secon annual grounwater report. 5

PPENDIX I ppenix I contains Table 1 isplaying a summary of the grounwater monitoring ata collecte, Table 2 isplaying the velocity of grounwater flow, an Figures 1-9 epicting the irection of grounwater flow as potentiometric surface map. The ates that the samples were collecte also is shown. Since this is the first annual grounwater report, which summarizes only the 2017 activities, there is no nee to state whether each sample was collecte as part of a etection monitoring or an assessment monitoring program. 6
