1 JOB DESCRIPTION JOB TITLE: REPORTS TO: BASE: OBJECTIVES OF POST: Practice Lead Foundations To demonstrate expert and effective practice in complex situations, assessing and managing medium/higher levels of risk. To manage a complex caseload and offer an expert opinion to others. MAIN RESPONSIBILITIES: 1. Principal accountabilities Undertake comprehensive assessments in order to assess the eligibility and suitability of a client in relation to the Aspire service. Where the client is assessed as suitable and eligible, utilise this assessment to create an individually tailored package of care for the client, in line with statutory requirements. To facilitate positive change for the client through the delivery of the care packaged including, where appropriate, psychosocial and psychological interventions. Conduct risk/need assessments and implement risk management/care plans in line with Aspire policy and procedures. Develop packages of care to meet the treatment needs of medium risk/complex clients, liaising with other health and social care professionals where necessary. Facilitate risk/need reviews as per Aspire requirements Function as part of a multi-disciplinary team, contributing to team meetings as required. Facilitate where appropriate whole family assessments, in order to ensure appropriate provision for young people Case-manage a caseload of medium risk/complex Substance Misusing clients in line with a recovery focused model, utilising and group-work sessions as appropriate. Work with complex childcare cases where there are issues of child protection or safeguarding children and partnership working with children s services. Make referrals to other services as appropriate and manage risk in complex cases where there are issues of safeguarding children. To undertake social work tasks working closely with other professional staff and to assess and review the needs of clients to deliver packages of care and therapeutic interventions to meet individual requirements where there is a complex need. 2 To assist in the decision making process regarding the referral of service user s children to children s services assessing and interpreting complex and conflicting information Support the Senior Social Worker for MARAC, liaising as appropriate with other MARAC leads from other agencies Ensure all possible steps are taken to account for personal safety of staff and clients, and escalate areas of concern where appropriate. Compile reports and participate in case conferences and multi-disciplinary team meetings and any other appropriate care planning meetings in relation to clients. Compile reports and participate in multi-agency meetings in line with requirements of client engagement with the service, Take personal responsibility for maintaining practice in line with developments and new initiatives. Test for Drug and Alcohol use. AE1 Collate statistical information as required to meet the needs of key stakeholders, both internally and externally Develop and deliver training packages for internal and external use Contribute towards the development of multi-disciplinary services for people with alcohol and drug misuse problems Proactively encourage drug and alcohol users to access substance misuse services, housing provision and, where appropriate, social support services; to promote and facilitate the harm reduction model of treatment provision in all aspects of work, to enable substance users to adopt safer practices and healthier lifestyles. Supervision and Training Keep personally up-to-date with developments and new initiatives AC1 Participate in regular supervision sessions with the line manager AC2 Provide supervision to case managers and social work students AC3 To participate in the appraisal process and work with the line manager to develop a personal training and development plan. Attend and participate in training sessions as required both as trainer and AC1 trainee. Provide an effective learning environment for all learners Maintain registration requirements in accordance with regulations 13. Safeguarding The Alcohol & Drug Service is committed to safeguarding and promoting the welfare of children, young people (anyone under the age of 18) and adults. It expects all staff, volunteers and students to share this commitment and act in such a way that safeguards and promotes the health and well-being of children, young people and adults. Staff must ensure that they are familiar with the signs and symptoms of abuse and know what to do if any such concerns are raised. Page 6 of 11
