2 Plans Submittal Requirement All drawings shall be dimensioned and drawn to scale. General Requirements for ROW: 1. The centerline of the right-of-way or road to the proposed utility installation 2. The road right-of-way width and pavement width 3. The distance from edge of pavement/back of curb to the utility 4. Sidewalks and property lines 5. The location of all other utilities within the area of work 6. Be aware of restoration responsibilities. 7. Photos showing existing condition of right-of-way 8. Engineering Standards Manual (ESM) to include a plan s legend 9. If the job site is within Downtown Core area, obtain approval from Downtown Development Board (DDB); contact Bob Fish at (407) or robert.fish@cityoforlando.net. 10. Obtain approval from other local and state agencies if work performed within their jurisdictions: - Florida Department of Transportation (FDOT) (407) Orange County Development Engineering (407) CSX Transportation/Railroad (800) ROW Requirements under Site Engineering: 1. Cross section showing the location of existing and proposed utilities for the following locations: - Under the City roadway - Under driveways - Beside inlets - Conflict locations between the City s utilities and the permittee Where crossing City utilities, indicate vertical clearance between the utilities 2. Identify method of installation on the plans (i.e. open cut or directional bore) 3. In case of using directional bore, location of entrance and exit pits 4. Restoration detail 5. Begin and end location of work 6. Indicate how many linear feet of sidewalk are proposed to be removed and restored in applicable. Sidewalk restoration shall not include rebar. 7. Only indicate on the plans what will be performed. No proposed open cut if needed shall be called out on the plans. - Both scenarios can be included in the plans; however, permittee shall also submit an engineering permit. 8. An engineering permit shall be submitted for open-cut pavement work with the following information: - Identify the location and pavement restoration method on the plans. Restoration shall meet the ESM section D - Provide cross section at the open cut location. Plans shall include all existing utilities surrounding the area. - Please submit public improvements cost sheet. A performance bond for public improvement is needed. Cost sheet forms and instructions are available at our website - Permittee shall contact Owen Blakely at (407) or russell.blakely@cityoforlando.net and Johnnie Miller, Permitting Services inspector at (407) or johnnie.miller@cityoforlando.net prior to any soft digs and/ or open cuts within the City roadway. 3 ROW Requirements for Street, Lane and Sidewalk Closures: 1. Maintenance of traffic shall be in accordance with the Manual on Uniform Traffic Control Devices (latest edition and revisions). 2. The contractor shall call (407) one week prior to the start of any phase of work to request street, lane or sidewalk closure authorization. 3. Local residential/business access shall be maintained at all times. 4. No lane closures will occur between the hours of 6 a.m. and 9 a.m. or between the hours of 3:30 p.m. and 6:30 p.m. Monday through Friday. 5. The use of traffic cones will be restricted to active work periods only. Channelizing devices (Type I, II, Drums) shall be used during inactive work periods unless otherwise specified. 6. The City of Orlando reserves the right to require additional devices and/or changes to the traffic control plan based upon changing traffic conditions. 7. Pedestrian control shall be maintained on one side of the road at all times. If unable to do so, the contractor shall provide temporary walkways, boardwalks and/or temporary concrete sidewalks. 8. The contractor shall be responsible for insuring each employee supervising the selection and placement of Maintenance of Traffic (MOT) Control Devices shall be properly trained by attending and successfully completing a Florida Department of Transportation (FDOT) approved MOT course. The training shall be at a level applicable to the employee s level of involvement. Copies of certifications shall be provided to the City Transportation Engineering Division prior to implementing any phase of MOT. 9. The contractor shall provide a Traffic Control Supervisor (TCS) who is responsible for initiating, installing and maintaining all Traffic Control Devices. 10. The TCS shall be available on a 24-hour per day basis, participate in all changes to traffic control and review the project on a day-to-day basis. 11. The TCS shall be present during the initial setup of the traffic control plan and all subsequent phases or changes to the traffic control. 12. The TCS shall immediately correct all deficiencies. 13. The contractor shall ensure the TCS be available on site within 45 minutes of notification of an emergency situation and is prepared to respond to and correct the traffic control or provide alternate arrangements for corrective actions. 14. The TCS shall be responsible for performing weekly, daytime and nighttime inspections of all traffic control devices, traffic flow and pedestrian, bicyclist movement through the work area and business accommodations. 15. The City may disqualify and remove from the project a traffic control supervisor that fails to comply with these provisions. The City may also suspend all work activities until corrective actions have been completed. Revisions (NOTE: In order to facilitate prompt review, revisions may be uploaded only after all plans examiners have completed their reviews of the previous submittal) Revised files must be properly named. DO NOT change any file names when resubmitting plans. The corrected files MUST have the same file name as their original. (Revised files that are uploaded correctly will automatically show V2 after the file name) Upload only the sheets that are being revised and/or new additional sheets. All revisions shall be identified and clouded. Upload a narrative or response to comments on company s letterhead. Complete Applicant Resubmit Task under Project Tasks. Page 3 of 5

4 Inspections (NOTE: Please refer to Chapter 4 of the 5 th edition Engineering Standards Manual (ESM) for all inspection requirements) A City of Orlando inspector shall visit the project site to make a visual inspection of the work in progress, along with methods of construction. All work not permitted to be inspected by the inspector will be considered as concealed work and unapproved. It is the contractors responsibility to call in inspections. Schedule inspections by using PROMPT automated phone system (407) or Call 48 Hours before work is performed, SUNSHINE at (800) First Inspection (Pre-Walk) Must have the following items: Permit(s) Approved City-stamped plans Door hanger Photos of existing condition of right-of-way Approved Sunshine clear (locate request) tickets and positive response tickets White line in place per City approved plans After Pre-Walk Inspection (Open Cut/Jack and Bore) Contractors are to call in inspections when work is occurring Final Inspection Must be called in after completing projects All inspections shall be performed during normal working hours between 7 a.m. and 3:30 p.m. excluding weekends and holidays. Should inspection be required at job sites beyond the normal hours, then the contractor is to request an overtime inspection in writing 48 hours in advance. Emergency Work (Interruption of service only) Emergency repairs may be performed without obtaining a permit first. 1. The City of Orlando must be notified of the nature of the problem and location by 10 a.m. the day following the commencement. 2. Must have locates prior to digging. 3. Contact Traffic Control/MOT at (407) Contact Johnnie Miller at (407) or Darrell Johnson at (407) ROW application must be received within 24 hours. 6. Pre-Walk inspection must be called in. 7. If work is temporary, the final placement cannot be done until the permit is issued. Page 4 of 5

5 Fees Aerial $50 for the first 30 linear feet or fraction thereof of construction in the right-of-way, plus $25 for each 110 linear feet of construction or fraction thereof over 30 linear feet. Underground $50 for the first 30 linear feet or fraction thereof of construction in the right-of-way, plus $50 for each 110 linear feet of construction or fraction thereof over 30 linear feet. Days (Construction Staging) $50 plus $10 per day. Maintenance of Traffic/Street Closure $120 Penalties Any work commenced or completed without the required permits will be charged double the amount of the permit(s) fee. Communications Services Providers are subject to penalties set forth in Chapter 23 of the City Code.
