5 Introduction Employee Enrollment Worksheets is your best choice when you want to illustrate each employee s cost based on the employer s contribution. Each employee will have their own sheet to help them make their plan and dependent coverage selections. Check Medical, Dental, Vision and/or Life on the Group Info step to show all on one sheet. Click on Quote This Group Again Click on Enrollment Worksheet when prompted. Fill in your group s name and census information, as you would for any other quote type. Page 3 of 11

6 Select the plan package being offered to the group. Select how the employer is contributing. You can select a fixed dollar amount, a percentage, or no contribution. You can apply the percentage contribution to a plan. Page 4 of 11

You can set the contribution separately for the Carrier and Kaiser Wrap, or use the same contribution for both. You may select the number of pay periods, rather than showing monthly premium.
