2 Request for Quote Installation of Rest Stop WWTP Post Aeration REQUIREMENTS/SPECIFICATIONS Section 1: Intent The purpose of this Invitation is to establish the total price from one vendor to provide all labor, materials, approved drawings and permits see (section 9) and any incidentals to install Post Aeration at four (4) ODOT rest areas located in Van Wert and Wyandot counties. Section 2: Contract Term This invitation is for a one time purchase award will be made to the lowest responsive and responsible vendor meeting or exceeding the bid specifications. Section 3: F.O.B. Destination Please be advised that all pricing shall include delivery charges if applicable, Free on Board (F.O.B.) to the work locations: Section 4: Award Award will be made by lump sum total to the lowest responsive and responsible bidder. Section 7: Work hours All work under these specifications shall be performed between the hours of 7:00 a.m. and 5:00 p.m., Monday through Friday, excluding State holidays. If scheduled work must be done after regular working hours or on weekends or holidays, it shall be done with the prior consent of the ODOT District Facilities Superintendent or their designee at no additional expense to ODOT. An ODOT representative may be required on site for inspection purposes. Section 8: Completion Date: The project is to be completed within 60 calendar days from the date of the purchase order.

3 Section 8: Scope of Work 8.1 Manholes Manholes are to be a minimum eight (8) feet deep by four (4) inch in diameter with removable grating. (Reference Attachment A) Preferred four (4) foot operating depth water is required in manhole for increased oxygen transfer efficiency. Minimum Three (3) foot of water depth will be accepted. 8.2 Discusser Assembly Ball valve and drop pipe must be ¾ 304 Stainless Steel Diffuser EDI Tube Diffuser 2.44 diameter x 24 long EPDM membrane fine bubble diffuser or approved equal. Underground airlines to be ¾ polyethylene tubing connected to clarifier/airlift blower with isolation valve on blower piping outlet. Minimum twelve (12) inch galvanized pipe stub from existing blower piping before transition to polyethylene. Underground airlines only require twelve (12) inches of cover, no deep trenching required. Contractor is responsible for locating underground utilities lines before digging. 8.3 Special Instructions: Rest Area 1-27 a. Install new manhole next to existing effluent pump station. b. Reroute piping as shown from the UV chamber to effluent pump station. c. Core drill new hole into Effluent pump station and seal with pipe wall penetration seal. d. Final Manhole positioning dependent upon existing underground buried wire. Rest Area 1-29 and 1-30 a. Remove exiting manhole following UV and replace with manhole referenced and replumb inlet/outlet piping accordingly. Rest Area 1-33 a. Install new manhole within fence area following UV chamber b. Replumb existing piping accordingly. Section 9: Drawings & Permits The elevations shown in the drawing in Attachment A are for reference only. Influent and effluent piping elevations are to be field verified. Water cannot discharge into upstream UV chamber due to post aeration manhole piping elevations. ODOT has obtained and paid for the required EPA permit. All other necessary permits to complete this work is the responsibility of the contractor..

4 Section 10: Prevailing Wages The Department has determined that prevailing wages will be required for this using the classification of building trades. This web link is: Section 11: Pricing Requirement Please submit your pricing on company letterhead. Price shall be inclusive of all materials, labor, overhead and transportation costs necessary to complete the project. Pricing should be itemized with a unit cost, subtotal and final cost. Bid will be awarded based on the final cost. By returning a quote, vendors acknowledge that They accept ODOT Terms and Conditions as found at Section 12: Bid Submission Instructions The original bid response (unless otherwise stated, hereinafter referred to as "bid") must be submitted to ODOT Purchasing prior to 11:00 a.m., on the scheduled day of opening as listed on the Request to Quote (RFQ). Bids may be submitted or delivered to ODOT Office of Contracts, Purchasing Services section - 1st floor, 1980 W. Broad Street, Columbus, Ohio Delivery to any other location, does not constitute bid being received by ODOT Purchasing. Bids submitted with insufficient postage will not be. Responses can be received by and will be considered as a valid submission to ODOT Purchasing. Section 11: Payment Awarded vendor will be paid within thirty (30) days after completion of the project. No partial payment requests will be accepted.
