3 WEEKLY INSPECTION CHECKLIST Note: Personnel undertaking this inspection are to have been briefed on the CEMP Inspection Item Yes No Comments/Required Actions Site Security and Access Is perimeter stock fence in place and intact? Have security gates been locked whenever the site was unattended? Hours of Operation Is work occuring only within approved hours of operation (i.e. 7am - 6pm Mon - Fri, 8am - 1pm Sat)? Site Signage Requirements Are all appropriate construction signs installed? Soil and Water Management Are all ESC measures implemented as per ESCP e.g. Temporary sediment basins, silt fencing, clean water diversion drains? If no, does ESCP need reviewing or do measures need implementing/maintaining? Are ESC measures functioning adequately? E.g. Do temporary sediment basins show signs of overtopping or sediment fences show evidence of water bypass? Do temporary sediment basins require clean out? Do temporary sediment basins require pump out? If yes, has water quality testing been conducted and 50mg/L TSS concentration been confirmed prior to discharge?if yes and TSS concentration >50mg/L, has sediment been flocculated in accordance with SWMP? Are there records that water quality monitoring has been conducted for the sediment ponds in the past week? Does the stabilised site access require maintenance? Is there any evidence that mud-tracking is occuring off-site? Are stockpiles adequatedly stabilised/protected? works? Noise Management Are all plant items being maintained to limit excessive noice generated by faulty/broken machinery? Are all plant items turned off when not in use? Have levels of reversing alarms of all plant on site been measured this month?if yes, are these below 85 db (A) when recorded 7m behind the vehicle? Air Quality (dust and odour) Are internal access tracks being watered appropriately? Are exposed area being stabilised as soon as possible? Are excavation works only occuring during periods of low wind speed? Are all trucks leaving the site covering their loads? Is excessive dust being generated from the site? Is excessive dust being generated external to the site and travelling into the site? If yes, is this being recorded (time, date, source)? Is there any offensive odour being generated? Storage of Hazardous Materials Is the hazardous substances register being updated as new substances are introduced to the site? Are hazardous materials stored above the 100yr flood level? Are bunds sized appropriately (110% of largest container) and in place? Are spill kits located and stocked appropriately? Is refuelling being undertaken in bunded areas minimum 12m away from the site boundaries? Are MSDSs located on site? Are MSDS records being updated as new substances are introduced on site? Waste Management Do bins require empting? Is site free of litter/construction waste? Do temporary sewerage facilities require pump-out? Traffic Management Is the Constrcution Traffic Management Plan being implemented? Material Management (tracking and documentation) Are plans fo the site being marked up daily by the site supervisor? Has the materials tracking report been submitted this month? Electrical Transmission Easement Management

4 Are stockpile height limits being observed? Are height markers and signage intact and visable? Is there any evidence that there are unsafe practices around the electricity easement Groundwater and Acid Sulfate Management Is groundwater monitoring infrastrucutre present and in good condition? Are all appropraite measures being undertaken to prevent the excavation/disturbance of acid sulfate soils (ASS)? Have ASS been identified on site? If yes, are these being managed in accordance with the Acid Sulfate Management Plan? Imported Fill Management Is the imported fill material management plan being implemented? Vegetation Management Is no-go signage and fencing around retained vegetation in-tact and undisturbed? If no, does this require maintenance/re-instatement? Are weed control procedures being implemented? Are any existing trees showing signs of degradation? If yes, can this be attributed to construction works? Is re-vegetation occuring in accordance with the biodiversity and offset management plan? Is landscaping occuring in accordance with the landscape management plan? Heritage Management Have any indigenous or Non-indigenous heritage items been uncovered on site during the past week? If yes, has work stopped and the item/s been reported to the DoPI/NSW Police/other organisation as required? Bushfire Management Are fire extinguishers on site, tagged and appropriate for potential hazards? Are clear zones being maintained? Are weeds under control? Training and Implementation Have all new personnel on site been inducted? Have these inductions been recorded in the site induction register? Has a toolbox talk occured this week? Complaints Have any complaints been received? If yes, have these been recorded in the complaints register? If yes, have these been responded to within 48hrs?
