All bids must be accompanied by valid GRA, NIS compliance certificates and Business and Vat Registration Certificates where the applicant is a business. A minimum requirement for qualification will be to have successfully executed works relating to the removal of asbestos within the last five years. The detailed qualification criteria are defined in the application documents. An Evaluation Committee will be appointed to review the bids/applications and bidders/applicants will be notified accordingly. All late bids will be rejected and returned unopened. Instruction to Applicants Submission of Applications 2.1 must be delivered in plain sealed envelope bearing no identification of the bidder and shall be marked on the top left hand side" Application to Pre-qualify for THE REMOVAL of ASBESTOS ROOF SHEETS & REPLACE WITH GALVALUME". These must be submitted to the tender box in the office of the Secretary to the Tender Board, GPL 91 Duke Street, Kingston, Georgetown, before 13:30 hrs on July 24, Tenders must be accompanied by valid National Insurance (NIS) and Guyana Revenue Authority(GRA) compliance certificates, and deposited in the Tender Box. Envelopes should be addressed as follows: The Secretary to the Tender Board Guyana Power and Light Inc. 91 Duke Street Kingston, Georgetown 2.2 Failure to provide information which is essential to evaluate the Applicant's qualifications, or to provide timely clarification or substantiation of the information supplied may result in disqualification of the Applicant. 2.3 The applications will be reviewed in detail by the GUYANA POWER & LIGHT INC., hereafter referred to as the Employer, and the result will be communicated in writing to the Applicants concerned. 3 Qualification of Applicants 3.1. Qualification Criteria Pre-qualification will be based on meeting the following criteria regarding the Applicant's general and particular experience, personnel and equipment capabilities, and financial position, as demonstrated by the Applicant's responses in the forms attached to the Letter of Application. The Employer reserves the right to waive minor deviations if they do not materially affect the capability of an Applicant to perform the work. Subcontractor's experience and resources shall not normally be taken into account in determining the Applicant's compliance with the qualifying criteria General Experience The scope of work includes: Pre-Qual Contractors Page 7 of 23

8 The work to be done is the removal of old asbestos roof sheets and the installation of pre-painted galvalume roof sheets. The asbestos roof sheets are 48 inches wide and approximately 30 feet long. The total area to be removed is nine hundred and seventy five square yard (975 Sq.Yd.). After removal these sheets are to be buried at an approved site. Air testing is required to be conducted prior to removal, during removal and after removal. An environmental permit will have to be obtained from the E.P.A before work commences. Approved protective gears for workers will have to be provided by the contractor. In addition, there are some carpentry works to be done to accommodate the installation of the pre-painted galvalume roof sheets as well as the replacement of the entire roof gutters system. The fascia boards will have to be replaced so as to accommodate the new roof gutters system. Please note that all other demolition materials will have to be removed from site and dispose of otherwise instructed by the Engineer. The Applicant shall demonstrate the following: a) Substantial average turnover for contract works over the last two years. b) Successful experience as a Contractor in the execution of at least three contracts of nature comparable to the proposed contracts within the last five years; this experience should include: c) Co-ordination, supervision and recording of works; and d) Management of work and progress reporting. The above experience shall be demonstrated by the Applicant by completion of the appropriate application forms and by the submission of supplementary information. Resources The Contractor shall: 1. Make available, closely supervise, and manage a work force that is both experienced and competent to carry out the intended works. 2. Ensure that especially the senior members of the work force are appropriately trained and qualified to carry out the works in a safe and effective manner. Pre-qualification for Contractors. 9 3.3. Personnel Capabilities The Applicant must have suitably qualified personnel to fill the following positions. Such equipment shall be listed in the application form provided (Form 6) Financial Position The Applicant should demonstrate that he has available, or assured access to, adequate funding to enable him to meet the cash outflow associated with the works. The Applicant is required to provide evidence of all sources of funding which he identifies as having given assurance of access. Applicants are required to support their financial status by submitting a full copy of their audited accounts for each of the last three years. Pre-qualification of Contractors. Page 9 of 23

10 3.6. Litigation History The Applicant shall provider accurate information on any litigation or arbitration resulting from contracts completed or under execution by him over the last five years. A consistent history of awards against the Applicant or any partner of a joint venture may result in failure of the application Safety Record The Applicant shall demonstrate a satisfactory safety record over the last three years by completing the information requested in form 9, and shall be in possession of the basic safety equipment General Only firms that have been pre-qualified under this procedure will be invited to participate in the works. The Employer reserves the right to reject or accept any application. The Employer shall not be liable for any such action nor be under any obligation to inform the Applicant of the grounds for them. Applicants will be advised in writing within a reasonable time from the date of submission of the applications of the result of their application without assigning any reason for the Employer's decision. Pre-qualified Applicants may be required by the Employer to update information used for pre-qualification at any time, to confirm their continued compliance with the qualification criteria. Pre-qualified Applicants shall be required to provide certificates of compliance from the National Insurance Scheme and the Guyana Revenue Authority and Public Liability Insurance, at the time of any eventual contract award. 14 APPLICATION FORM (2) General Experience Record IName of Applicant: All firms are required to complete the information in this form. The information required is annual turnover of the Applicant, as invoiced to clients for each year for work in progress or completed during the year, over the last three years. Where a contractor prepares accounts to a year-end other than 31 December each year, the turn over is to be apportioned on a time basis in presenting figures for each calendar year. Applicants are requested not to enclose testimonials, certificates or promotional materials with their applications, as these will not be taken into account in the evaluation of qualifications.
