1 BID PROPOSAL CITY OF HOMESTEAD, FLORIDA WWTP INFLUENT PUMP STATION The work to be performed under this Contract shall consist of furnishing all equipment, materials, supplies, and manufactured articles for furnishing all transportation and services, including fuel, power water and essential communications, and for the performance of all labor, work, or other operations required for the fulfillment of the Contract in strict accordance with the Contract Documents. 1 Aggregate For performing site preparation, mobilization and demolition work for Influent Pump Station, Electrical Building and Standby Generator as required by the Contract Documents including but not limited to all labor and materials required as well as all bonds, permits, fees and insurance, the aggregate sum amount of: 2 Aggregate of the Influent Pump Station complete, as required by the Contract Documents including but not limited to sheeting and shoring, all labor, bonds, permits, fees, insurance, other miscellaneous costs, overhead and profit, for a complete functional installation the aggregate sum of: O:Attachment B -Bid Form/ BP-1 WWTP Influent Pump Station

2 3 Aggregate of the Electrical Building serving the Influent Pump Station complete, as required by the Contract Documents including but not limited to all labor, bonds, permits, fees, insurance, other miscellaneous costs, overhead and profit, for a complete functional installation the aggregate sum of: 4 Aggregate of the Standby Generator with enclosure serving the Influent Pump Station complete, as required by the Contract Documents including but not limited to all labor, bonds, permits, fees, insurance, other miscellaneous costs, overhead and profit, for a complete functional installation the aggregate sum of: 5 Aggregate For all costs incurred for startup services for the Influent Pump Station, Electrical Building and Standby Generator as required by the Contract Documents, including but not limited performing and coordinating start-up services including performance tests for the pumps, electrical control panel, generator, level sensors devices, and telemetry system, for a complete functional installation the aggregate sum of: O:Attachment B -Bid Form/ BP-2 WWTP Influent Pump Station

3 6 Aggregate For performing site restoration and clean-up for the Influent Pump Station, Electrical Building and Standby Generator including but not limited to asphalt paving, grading, driveways, sodding, landscaping, painting, and any other miscellaneous restoration work including all labor, materials, and equipment required, the aggregate sum of: 7 Aggregate For furnishing, installing, operating (manned 24 hours per day, 7 days a week), and all maintenance including fuel and power costs and dismantling and removal costs for a Bypass Pump System to be utilized during placing into service of the new gravity interceptor, which includes the personnel, equipment installation, testing, start-up, and operational test period to ensure that the bypass system is satisfactory to the Engineer and the City of Homestead, the aggregate sum of: 8 Aggregate For furnishing materials, labor and equipment for Traffic Maintenance and Control as required by regulatory agencies having jurisdiction in the project area, including but not limited to Miami Dade County Public Works, City of Homestead and Florida Department of Transportation, the aggregate sum price of: O:Attachment B -Bid Form/ BP-3 WWTP Influent Pump Station

4 9 Subtotal A SUBTOTAL of s 1 8, all work associated with the Influent Pump Station, Electrical Building and Standby Generator: 10 Dedicated Allowance For unforeseen conditions, minor construction changes and for additional work or materials not covered by other Proposal s, the dedicated allowance of: 250, Total TOTAL BASE BID AMOUNT: The sum of s 9 and 10, inclusive: Note: For a detailed description of each Proposal, refer to Section of the Specifications entitled "Measurement and Payment". AMOUNTS SHALL BE SHOWN IN BOTH WORDS AND FIGURES. IN CASE OF DISCREPANCIES, THE AMOUNT SHOWN IN WORDS SHALL GOVERN FOR EACH BID ITEM AND TOTAL BASE BID. The low bidder will be determined on the basis of the Total Base Bid Amount on 15. The quantities shown are an estimate and are only for the purpose of facilitating the uniform comparison of bids submitted. The City does not guarantee that any specific quantities or item(s) will be utilized. The quantities of any item may be increased, decreased or any item deleted as need occurs. Payment of work performed will be based on the actual quantities completed at each site the Contractor works on. The Contract will be awarded based on the Total Base Bid Amount. The City reserves the right to award or reject any or all bids. The Contractor agrees to start work within the amount of days provided for MOBILIZATION TIME on the Bid Form. The bidder understands and agrees that the above Total Base Bid Amount is inclusive of all work materials necessary to complete the Project as described in the Contract Documents. 5 DATE NUMBER AWARD OF CONTRACT AND TIME OF COMPLETION The contract, if awarded, shall be to the lowest responsive bidder, and most responsible bidder whose bid conforming to the specifications is the most advantageous to the City, considering prices bid, the experience, the qualifications of the bidder, and the bidders current and immediately available resources. Bid must comply with the requirements necessary to render it formal. The WORK will be substantially completed within 270 successive days from the commencement date stated in the Notice to Proceed. The WORK will be entirely complete (final) within 300 successive days from the commencement date stated in the Notice to Proceed. O:Attachment B -Bid Form/ BP-5 WWTP Influent Pump Station
