4 EXECUTIVE SUMMARY PROJECT : 7 TH FLOOR WHY MEDICAL CENTER The purpose of this project is to eliminate the existence of bacteria, fungi and mold growth activities that has caused numerous patches of mold and fungi on the 7 th floor of the Hospital. 13 1) Detail Report Resolution from the Proposal Areas Involved : Ductworks of AHU and FCU coverage Findings Work Process Step 1) Removing accumulated contaminants Contaminants build in the duct works is the root cause of microbial growth. An unclean duct works over a long period of time promotes unprecedented microbial growth that has negative multiplier effects for an acceptable indoor air quality. Once the contamination is removed from the duct works, the job is made easier and better conditions will exist in order to minimize the use of disinfectants plus staff and visitors will work in a cleaner environment. Activities Conducted Ductworks: 1. Clean the ductworks using i. Air knife Colom cleaning ii. Negative tank ( vacuum ) iii. Personal Protective Equipment 2. All FCU supply flexible duct was removed and manually clean. Main supply duct was cleaned using Air Knife system with negative tank Challenges 1. Barriers accessing to ducts due to existing obstacles 2. Dry up insulation layer 3. Dry Up supply branch ducting 1. The ductwork found to be in moderate condition and generally covered with fine dust. 2. Some of the exhaust duct from FCU are heavily covered with dust. 3. Encountered a few fresh air and exhaust flexible ducting broken, as a result the FCU is getting fresh air from above the ceiling. ( Fixed the broken ducting ) 4. Encountered a few of the return grill was not connected to the main return duct. 5. Encountered a few exhaust fan is not functioning 6. Insulation layer of the main plenum was soaked and smelly 7. Branch supply ducting to lobby lift was wet and smelly. 8. Overall supply and return ducting has a moldy smell Clean supply Recommendation Dirty Clean return 1. Need to fix the unconnected return grill to the main return duct immediately. 2. To inspect the ducting condition on periodic basis 3. Cleaning frequency as follows; I. Supply duct : subject to cleanliness inspection every 24 months II. Return duct : subject to cleanliness inspection every 24 months III. Once the contamination is removed from the duct works, the job is made easier and better conditions will exist in order to minimize the use of disinfectants plus staff and visitors will work in a cleaner environment. Activities Conducted 1. All FCU blow fan chamber generally very dirty and covered with thick fine dust 2. Water retention inside the blow fan chamber at FCU room #720. We noticed the plaster ceiling was wet with mold presence. 3. No access to filter and blow fan chamber at room #721 but we are able to clean cooling coil Clean Recommendation Blow fan chamber Dirty Ductworks FCU FCU i. Compressed air ii. Spray gun iii. Dry vacuum iv. Forward DC v. GermXit liquid vi. GermXit Gel 1. Filter and blow fan chamber need to be clean every 45 days 2. Replenishment of GermXit gel as air treatment every 45 days, with a quantity of 2 X 200 grams 3. Disinfect using GermXit Liquid every 45 days 4. To inspect flexible ducting condition every 45 days AHU Challenges 1. Access window to FCU filter and blower was too narrow. 2. No access to clean FCU cooling coil 3. To dismantle all supply flexible duct for manual cleaning 14

Once the contamination is removed from the duct works, the job is made easier and better conditions will exist in order to minimize the use of disinfectants plus staff and visitors will work in a cleaner environment. Activities Conducted 1. Pre-filters were moderately dirty. 2. Cooling coil was very dirty with moldy smell 3. Blow fan was very dirty 4. We suspect the AHU was NOT properly dry when cleaning was conducted previously. AHU room should be cleaned thoroughly because of the design flow of the return duct stops at AHU room wall. This design collects all dust particles in AHU room, mixing with fresh air intake. It is recommended to lay epoxy paint on the floor of the AHU room for easier future cleaning. 2. To conduct periodic cleaning of the AHU system every 90 days. AHU Challenges 1. To remove dirt and mold accumulated inside cooling coil. 3 sequence of coil cleaning required to remove the dirt. 15

17 1) Detail Report Areas Involved : All AHU and FCU Resolution from the Proposal Findings 1. We discovered the root cause of bad mold smell all over the area are from the AHU 2) Air decontamination at return and fresh air intake Once we have removed contaminants in the duct works, we proposed placement of GermXit at the fresh air and return air intake to kill the bacteria, fungi and mold while it is traveling through the coils. By doing so, it also gives another protecting barrier on both filters and disinfect the coils rendering. Before third cleaning Activities Conducted Recommendation i. GermXit liquid spray ii. GermXit Gel placement iii. GermXit Container where the Gel is being placed Challenges 1. To maintain cleanliness of the AHU and FCU to avoid future growth of mold and bacteria, the followings are recommended; i. Placement of GermXit at a. AHU 8KG every 45 days b. FCU 2 X 200 Grams every 45 days c. Disinfect the area using liquid GermXit every 45 days. ii. Clean the AHU and FCU every 90 days 1. We are forced to clean the cooling coil and fin three times to ensure cleanliness by removing accumulated dirt. 17
