Instructins: Sectin Instructins fr Submitting the SOI Team Spreadsheet: When entering data int the SOITEAM.xls spreadsheet, remve / delete the spreadsheet tabs fr thse items which yur Firm / Team is nt submitting. When submitting the SOI Team Spreadsheet, it needs t be in its wn separate with n ther attachments included. Questins: The fllwing questins were received in the SOIPSB Questins flder and are addressed belw. Resulting changes are summarized belw: QUESTIONS: The Tllway has received the fllwing questins t via t the SOIPSB 17-3 mail bx. The Tllway ffers the fllwing respnses: Questin 1: Our cmpany is submitting ur SOI as the Prime and we will be perfrming the wrk in the specified categry: Special Services (Aerial Mapping and LiDAR). As a Prime we are prequalified t wrk fr IDOT. Hwever, we are sub-cntracting ut the cllectin f the aerial imagery that will be used fr mapping. Des the subcntracted cmpany wh will prvide the aerial imagery cllectin need t be prequalified fr wrk with IDOT? Since we are prequalified des that alleviate the prequalificatin requirements fr the basic aerial infrmatin cllectin? Respnse: Since yu are prequalified in the categry Special Services (Aerial Mapping and LiDAR), and if yu take respnsibility fr the wrk prduct f yur subcnsultant, then the subcnsultant des nt need t be prequalified in Special Services (Aerial Mapping and LiDAR). Questin 2: Exhibit A, page 3 asks fr verificatin that the firm will cmplete the prject within the estimated time listed in the prject advertisement. It als asks that a cmpletin date and/r number f mnths be prvided. Tllway Items 1-8 prvide n estimated time limit within their advertisements, stating nly that design fr this prject is scheduled t start in Cnstructin f this prject is nt scheduled. Please advise what statement r time frame shuld be used n Exhibit A fr these items. Respnse: It is recmmended yu assume 30 mnths. Questin 3: Questin regarding Items 12 and 13 n : Can the prequalificatin requirements be mdified s that the prime firm is allwed t fulfill the Special Services (Mechanical Engineering) prequalificatin thrugh a subcnsultant? 3 Respnse: Yes. A prime may meet the prequalificatin fr Special Services (Mechanical Engineering) thrugh a subcnsultant. Questin 4: When filling ut the current bligatins frm, shuld we be using dllars, r thusands? Respnse: Dllars. Questin 5: Is the deadline t ask questins 10 calendar days r 10 business days fr, Item 29? Respnse: Unless cntains a time limitatin specifying that days mean business days, the default is t calculate using calendar days. Questin 6: Fr Item 28. If there are n listings fr Key Persnnel under the item descriptin, is there a need t fill ut Exhibit A? Respnse: Please use Exhibit A t list the key prject persnnel that yu will be utilizing fr Item 28. The persnnel named in Exhibit A must als be listed n Exhibit D: Availability f Key Prject Persnnel. Questin 7: Fr Item N. 28, d we need a Prject Manager with an Illinis PE, r prvide an Illinis PE n the prject? Respnse: Please prvide an Illinis PE n the prject. Questin 8: Fr Item N. 28, d we need t participate in the mentring prgram? Respnse: N. Questin 9: As a prime cnsultant, if we are using sub-cnsultants in any capacity shuld a Teaming Agreement be included with ur submittal? Respnse: A prime cnsultant des nt have t submit as a Team if they are using subcnsultants. See Sectin 2.8 f fr details regarding Teaming Agreements. Questin 10: When perfrming Aerial Mapping, LiDAR and Surveying services fr the Illinis Tllway, des the Tllway use the State f Illinis, Illinis Department f Transprtatin Survey Manual as the standards t perfrm the services, r des the Tllway have their wn set f Mapping and Survey Standards? Respnse: Please use the State f Illinis, Illinis Department f Transprtatin Survey Manual as the standards t perfrm the services. Questin 11: The Delinquent Debt Review Frm is available fr dwnlad in tw lcatins. The frm prvided with the Illinis Tllway Standard Business Terms and Cnditins has the fllwing heading fr the right clumn in the subcntractr infrmatin table: Anticipated Percentage f Cntract (t the extent knwn). The frm that can be dwnladed by itself has this heading fr the right clumn in the Addendum N 2 Page 2 f 5 April 26, 2017

4 subcntractr infrmatin table: Anticipated Amunt t be Paid (t the extent knwn). We prefer t prvide the anticipated percentage. Is that acceptable? Respnse: N. Please nte that the Delinquent Debt Review frm was revised in Addendum 1. Yu need t utilize the frm prvided in Addendum 1 that requests the Anticipated Amunt t be Paid (t extent knwn). Questin 12: We have requested a Team Name. After receiving the name frm the Tllway, if we decide t mdify the team lead r team members, is that acceptable? T determine the designated SOI Firm Name Cde g t the Tllway s website under Ding Business>Cnstructin and Engineering - see Jint Resurces and click n the link t the Prfessinal Services Bulletin page. The current SOI Firm Name Cde List will be psted under the Required Frms sectin. If yur firm is nt included n the list, r yur firm name has changed, please cntact Eleanr Curcur at (630) , extensin 3851, fr an assigned SOI Firm Name Cde. When submitting Team files please, use the new SOI Firm Name Cde assigned by the Tllway t the Team. The SOI Firm Cde Name is exclusive t the electrnic submittal meaning it des nt establish the name f the Team. Please cntact Eleanr Curcur at (630) extensin 3851, fr an assigned SOI Firm Name Cde. All electrnic submittals f yur firm s s subject lines MUST read: SOIPSB17-3, SOI FIRM NAME CODE, ITEM (# s). Any variatin excluding SOIPSB17-3 frm the subject line will result in yur firm s submittal(s) being directed t a SPAM FOLDER which will autmatically delete the Instructins fr Submitting the SOI Team Spreadsheet The Tllway requires an electrnic submittal f the Statements f Interest, and a SOI Team spreadsheet (SOI Team: Prime and subcnsultant ) that captures critical infrmatin used in the Selectin Prcess. The instructins are as fllws: Vendrs must btain the SOI Team spreadsheet frm the Tllway Website: Prime and subcnsultant Spreadsheet. The SOI Team Spreadsheet includes a separate wrksheet fr each Item advertised in the PSB. Vendrs are required t cmplete the wrksheet fr each Item, cmpleting all clumns shwn fr the Prime Cnsultant, and fr every subcnsultant that is being prpsed. D nt enter TBD r enter a range f percentages, and d nt leave blank cells. When entering data int the SOITEAM.xls spreadsheet, remve / delete the spreadsheet tabs fr thse items which yur Firm / Team is nt submitting. Fr each subcnsultant, the Prime s full legal firm name must be entered in the first clumn. The wrksheet is t be cmpleted fr each Item fr which yur firm is submitting a SOI; all ther wrksheets may be deleted befre submitting t the Tllway. The SOI Team Spreadsheet frmat cannt be altered it cntains imbedded frmulas; d nt cut and/r cpy, link r paste any infrmatin. All infrmatin must be typed - nt cpied in. DO NOT ADD, r DELETE clumns, r refrmat the sheets r any setting in the cell (fnt/type). DO NOT MODIFY THE COLUMNS, ROWS AND/OR ANY CELLS. DO NOT skip rws when entering infrmatin; there shuld be n blanks between infrmatin. DO NOT write-prtect yur submissins. DO save yur file with yur cursr in cell B3 and as an Excel 2010 file befre submitting. Vendrs are required t cmplete the wrksheet fr each Item, cmpleting all clumns shwn fr the Prime Cnsultant, and fr every subcnsultant that is being prpsed. Disclsures submitted n behalf f a Teaming Agreement shuld use the unique 2-6 character SOI Firm Name Cde fr each individual Team firm name fllwed by DS_TM. (Example: abcds_tm.pdf, defds_tm.pdf and ghids_tm.pdf). If the firms are registered n the IPG, include yur IPG registratin number in the bdy f yur . NOTE: Firms previusly assigned a JV cde name may be allwed t use this designatin fr histrical cnsistency, but shuld cnfirm with the Tllway. Fr yur firm s Current Obligatins (Exhibit C), use yur 2-6 character SOI Firm Name Cde, fllwed by WL (Example: abcwl.pdf). Exhibit C submitted n behalf f a Teaming Agreement shuld use the unique 2-6 character SOI Team Name Cde (Example: abc_tmwl.pdf). Fr yur firm s SOITEAM Spreadsheet, use yur 2-6 character SOI Firm Name Cde, fllwed by SOITEAM.xls. (Fr Teams, the subject line f each shuld utilize the Teaming Agreement Name cde assigned by the Tllway. The Team lead will submit n behalf f the participating partners and identify the partner with a (TM) fllwing their firm name in the wrk sheet.) (Example: abc_tmsoiteam.xls) Signatures must be scanned, then cut/cpied and pasted int the apprpriate signature blcks. The files (with the exceptin f SOITEAM) must then be cnverted t a PDF. abc02, abc09 and abc24 are the Statements f Interest fr the submitted Items and must include Exhibit A within the submittal. Fllwing Exhibit A, include resumes f key prject persnnel listed in Exhibit A. If Exhibit B is required, include next, fllwed by resumes f individuals listed n page 2 f Exhibit B. abcds is the Disclsure Frms. One cpy f the Disclsure Frms is required (unless yu are submitting as a Team if a Team, include a DS fr each Team member and name as abcds_tm.pdf). abcwl is the Current Obligatin Frm (Exhibit C).
