Standards, Harmonization, and Interoperability IHE Update Erin Sparnon Sr. Project Engineer ECRI Institute

Why are we here? At HIMSS, many suppliers claim interoperability but it s not that easy!

Presentation Overview Brief IHE Overview What s New? Q&A & Wrap

IHE Overview In operation since 1997 (Radiology domain), consistent growth in clinical disciplines and geographic dispersion Members from 300+ healthcare related organizations worldwide IHE membership spans clinical, IT, standards development, education, research and government stakeholders

National and Regional Projects Using IHE Profiles UK CfH (Radiology WF) Netherland Amsterdam France DMP Denmark (Funen) Italy (Veneto) Spain (Aragon) Lower Austria Austria Italy (Conto Corrente Salute) Quebec, Toronto, Alberta, British Columbia Canada Infoway VITL-Vermont Boston Medical Center - MA Philadelphia HIE CPHIC Pennsylvania CareSpark TN & VA South Africa CHINA-Shanghai 5 Imaging Info Sharing Malaysia CHINA-MoH Lab results sharing JAPAN-Nagoya Imaging Info Sharing THINC- New York NCHICA N. Carolina

IHE the process Identify Interoperability issues Develop Integration Profiles Actors and Transactions Workflow Gather relevant standards Connectathon Testing Publish Integration Statements Vendors list tested profiles 6

What are IHE Profiles? Based on clinical workflow and include transactions that spell out messages that are sent between actors Transactions: a descriptive name and an identification number assigned by the domain Messages: the content and format of information transferred Actors: labels with defined roles and responsibilities Suppliers document their support of profiles with Integration Statements

IHE Development Cycle Test at IHE Connectathons IHE Profile Drafted & Revised (Months 613) Profile Selection by Committees (3 (3 5) IHE Call for Proposals Opens (Months 1 2) 11 2) IHE Improves Safety, Quality and Efficiency in Clinical Settings Demonstrate at a Final Results Adopted by ISO: TR Install Interoperable products in Clinical Settings worldwide

Problem to Profile

What s available now? Integration Profiles tested and verified last year

How can I tell if there is a profile that describes the clinical workflow I need? Visit the Clinical/Operational Domains: Anatomic Pathology Cardiology Eye Care IT Infrastructure* Laboratory Patient Care Coordination* Patient Care Devices* Pharmacy Quality, Research and Public Health* Radiation Oncology Radiology Each Domain s Technical Framework will spell out the transactions supported in each profile * HIMSS Sponsored Domains View the full listing and contacts for the IHE Committees here:

How can we tell if a system supports these profiles? 1. Visit the suppliers on the exhibit floor, and 2. Ask to see an Integration Statement for the profile you re interested in 3. If an integration statement is unavailable, request conformance in an RFP

What s new in IHE? CDA Consolidation Project Meaningful Use requires information transfer that conforms to the HITSP C32 standard But HITSP C32 is based on several different Clinical Documentation Architecture (CDA) implementation guides And the different groups that developed these guides didn t always use the same language or templates Health Story, HL7, and IHE are collaborating to Harmonize the current CDA implementation guides Provide a single package of human readable and machinereadable information for implementation View the full listing and contacts for the IHE Committees here:

What s new in IHE? Continua IHE WAN Coordination View the full listing and contacts for the IHE Committees here:

What s new in Patient Care Devices? User Guide Helping facilities navigate and PCD profiles and request them in RFPs Rosetta Terminology Mapping Terminology support for parameters and units of measurement Point of care Infusion Verification profile Supporting closed loop medication administration View the full listing and contacts for the IHE Committees here:

What s new in Radiology?

What s new in Radiology? National Institute of Biomedical Engineering and Bioengineering (NBIB) XDS I contract 5 academic medical centers (Mayo Clinic, Mt Sinai, UCSF, UMD, Un of Chicago) XDS I clearinghouse acts as a secure server 300,000 patients over 2 years, with plans for workflow refinements View the full listing and contacts for the IHE Committees here:
