8 SILT FENCE INSTALLATION DETAILS: STABILIZATION PRACTICES: ALL DISTURBED AREAS ARE TO BE STABILIZED. ALL STABILIZATION PRACTICES SHALL BE PERFORMED AS SOON AS PRACTICAL AT LOCATIONS WHERE CONSTRUCTION ACTIVITIES HAVE TEMPORARILY OR PERMANENTLY CEASED. LOCATION OF STAKED SILT FENCE & STAKED TURBIDITY BARRIER: STAKED SILT FENCE SHALL BE PLACED AS SHOWN. SILT FENCE SHALL BE USED AROUND ALL EXCAVATED MATERIAL IN ITS TEMPORARY LOCATIONS DURING CONSTRUCTION. OFF-SITE VEHICLE TRACKING: LOADED HAUL TRUCKS SHALL BE COVERED WITH TARPAULIN. EXCESS DIRT ON THE ROAD SHALL BE REMOVED DAILY. ALL EROSION AND SEDIMENTATION CONTROL ITEMS SHALL BE REMOVED UPON COMPLETION OF THE PROJECT. 3) THE CONTRACTOR SHALL BE RESPONSIBLE FOR DAMAGE TO UTILITIES, STRUCTURES AND PROPERTY ON AND ADJACENT TO THE SITE CAUSED BY CONSTRUCTION ACTIVITIES. ALL DISTURBED AREAS SHALL BE RETURNED TO THEIR PRE-CONSTRUCTION CONDITION UPON COMPLETION OF THE PROJECT AT THE CONTRACTOR'S EXPENSE. ALL COSTS TO BE INCLUDED IN PAY ITEM CLEARING AND GRUBBING. 4) THE CONTRACTOR SHALL BE RESPONSIBLE FOR THE SAFE MAINTENANCE OF PEDESTRIAN AND VEHICULAR TRAFFIC AT ALL TIMES DURING THE DURATION OF THE PROJECT IN ACCORDANCE WITH FDOT STANDARDS AS INDICATED IN THE MAINTENANCE OF TRAFFIC NOTES. 5) ALL CONSTRUCTION ACTIVITIES SHALL REMAIN WITHIN THE ROADWAY RIGHT OF WAY. THE CONTRACTOR SHALL IDENTIFY THE RIGHT OF WAY BEFORE BEGINNING CONSTRUCTION. 6) SILT, SEDIMENT, TRASH, OR CONSTRUCTION DEBRIS DEPOSITED ON PRIVATE PROPERTY SHALL BE IMMEDIATELY REMOVED BY THE CONTRACTOR. AREAS DISTURBED BY REMOVAL OPERATIONS SHALL BE RESTORED TO PRE-EXISTING CONDITION. ALL COSTS TO BE INCLUDED IN PAY ITEM CLEARING AND GRUBBING. 7) SIGNING AND PAVEMENT MARKINGS SHALL BE PLACED AS SHOWN ON THE PLANS AND PER THE 2009 MANUAL ON UNIFORM TRAFFIC CONTROL DEVICES AND THE FDOT 2014 DESIGN STANDARDS. 8) ALL EXISTING ROADWAY SIGNS ARE TO REMAIN IN PLACE UNLESS OTHERWISE SHOWN ON THE PLANS. SIGNS THAT ARE DISTURBED DURING CONSTRUCTION SHALL BE RESET TO CURRENT STANDARDS AT THE CONTRACTORS EXPENSE. ALL COSTS TO BE INCLUDED IN PAY ITEM CLEARING AND GRUBBING 9) IT SHALL BE THE CONTRACTORS RESPONSIBILITY TO TO COORDINATE MATERIALS TESTING WITH THE TESTING LAB AND SUBMITTING TEST REPORTS TO THE COUNTY. 10) CONTRACTOR SHALL RESTORE AREAS DISTURBED DURING CONSTRUCTION WITH SOD. THE CONTRACTOR SHALL BE RESPONSIBLE FOR MAINTENANCE AND REPAIRS OF EROSION AND SEDIMENT CONTROL DEVICES AND REMOVAL OF EROSION AND SEDIMENT CONTROL DEVICES AFTER THE NOTICE OF TERMINATION. MAINTENANCE AND REPAIR REQUIRED FOR THE CONTROL AND ABATEMENT OF EROSION AND WATER POLLUTION SHALL BE INCLUDED IN THE PROJECT COST. POLLUTION CONTROL MEASURES SHALL BE INSPECTED DAILY. WRITTEN DOCUMENTATION OF INSPECTIONS SHALL BE WRITTEN EVERY SEVEN CALENDER DAYS AND WITHIN 24 HOURS OF A RAIN EVENT OF 0.50 INCHES OR GREATER. THE UNDERSIGNED ENSURES THAT THE SURFACE WATER AND STORMWATER MANAGEMENT MEASURES PROPOSED IN THE PLAN WILL BE EFFECTIVELY IMPLEMENTED UNTIL PERMANENT SURFACE WATER MANAGEMENT SYSTEM IS OPERATIONAL Note: See FDOT 2014 Index 201, 232, 250, & 280 For Additional Details and Notes Regarding Construction MAINTENANCE OF TRAFFIC NOTES: 1. MOT TO BE PER 2016 FDOT ROADWAY AND TRAFFIC DESIGN STANDARDS INDEX 600 SERIES, INCLUDING BUT NOT LIMITED TO, INDEX 600 GENERAL INFORMATION AND INDICES 602, 603, 605 AND OTHERS THAT APPLY. 2. The Contractor shall confirm that existing signage will not pose a conflict for work zone traffic control. The Contractor shall cover or remove all signing in conflict with traffic control during construction operations, after consulting with the County Engineer's Office. The Contractor shall also provide a safe storage of signing materials to be reinstalled at the completion of the project. Any, and all cost incurred in these operations shall be included under the Pay Item "Maintenance of Traffic". 3. Any signs, including work zone and permanent signage, not in effect at any such time during construction shall be bagged, covered, or removed until such time that that said sign is again needed. 4. The barricade spacing for this project is 35 ft. maximum with Type II barricades or vertical panels expected unless otherwise specified or approved. Steady burn lights are required. 5. Conflicting or existing pavement markings shall be removed by water blasting. All existing pavement markings outside the limits of construction, which are altered and/or damaged shall be replaced with like materials, and all costs shall be included under the Pay Item "Maintenance of Traffic". 7. The Contractor is alerted to the fact that optional shoulder treatment or other means may be necessary to abide by Index No The Contractor shall include the costs of any and all items and incidentials necessary to abide by Index No. 600 and to provide proper work zone traffic control under the Pay Item "Maintenance of Traffic". 9. Failure to comply with the prescribed (or approved) Work Zone Traffic Control may result in the immediate suspension of construction activities; with no allowance for contract time delays. 10. The Contractor shall maintain proper drainage during construction operations. 11. As a minimum, one-lane two-way operations shall be maintained through the use of visable, properly trained flaggers during lane closures. 12. The Contractor shall notify the following of road closures and detours two working days in advance of such activities: Florida Highway Patrol Hernando Sheriff"s Office Florida E.M.S.
