Kuriger Associates At the request of the Pennimans, this Phase V Status Report and Monitoring Report provides information on the status of remedial activities at 131 Burrage Street, Lunenburg, MA, part of the disposal site at 110 Burrage Street, Lunenburg, MA since the Phase V Status Report 2 was filed on July 18, (2) Phase V Status Report (a) a description of the type and frequency of operation, maintenance and/or monitoring activities conducted As a review, a new carbon filter was installed at 131 Burrage Street, Lunenburg on June 12, The new carbon filter system is shown in photographs in the Phase IV Completion Report dated January 15, After initial testing described in the Phase IV Final Inspection Report, the drinking water before and after the charcoal filter was again sampled on December 31, 2012 after running the drinking water from the kitchen sink for 10 minutes. A groundwater sample from disposal site well MW-101 was also sampled for analysis after purging the well of three volumes of groundwater on December 31, The analytical results for the water samples are listed below in Table 1. Kuriger Associates Vinyl Chloride 2.55 ug/liter 2 ug/liter Based on the results, the new carbon filter was working and removed the majority of chlorinated chemicals present in the drinking water. The level of cis-1,2-dichloroethene (DCE) in the After the Filter sample may have been due to laboratory variability, an accumulation of the compound in the filter, the degradation of tetrachloroethene or trichloroethene resulting in DCE in the filter, or a limitation of the charcoal filter. The results for tetrachloroethene and trichloroethene in the After sample were significantly below the levels in the Before sample, which provided evidence that the filter was working. Based on these results it was recommended to the Pennimans that the charcoal filter be changed in January 2013, even though the levels of contaminants in the drinking water before and after the new charcoal filter were all below Massachusetts Contingency Plan (MCP) GW-1 risk characterization standards. The charcoal filter at 131 Burrage Street was changed on January 23, It was also recommended to the Pennimans that drinking water samples from 131 and 110 Burrage Street be collected quarterly (seasonally) as is possible to further evaluate the new carbon filter and to also track the levels of the chemicals in the drinking water well and at MW-101 at 110 Burrage Street. The results for the groundwater sample collected on December 31, 2012 at MW-101 were below the GW-1 risk characterization standard for cis-1,2,-dichloroethene and above the GW-1 and GW-2 standards for vinyl chloride. The charcoal filter, and existing rope sediment filters at 131 Burrage Street were changed on October 15, 2013, after new drinking water samples, After the Filter and Before the Filter were collected. A sample from MW-101 was not collected due to budget limitations. A summary of the results of the samples are listed below in Table 2. Table 2. Kuriger Associates Tetrachloroethene 0.66 ug/liter 5 ug/liter Trichloroethene 3.58 ug/liter 5 ug/liter The level of cis-1,2-dichloroethene was slightly higher in the After the Filter sample than in the Before the Filter sample, as it was in the December 31, The levels of tetrachloroethene, and trichloroethene were lower in the After sample, as they were in December 31, The level of cis-1,2-dichloroethene in the After sample may be due to degradation of tetrachloroethene and/or trichloroethene in the filter. The level of trichloroethene was higher in both samples than in December 31, 2012 samples. Mr. and Mrs. Charette, residents of 131 Burrage Street, were notified of the results of the laboratory tests via mail with a copy of the results and a Notice of Environmental Sampling Form BWSC 123. (b) a description of any significant modifications of the operation, maintenance and/or monitoring program made since the submission of the preceding Phase V Status Report This is the third Phase V Status Report. The charcoal filters, along with existing house sediment filters at 131 Burrage Street were changed on October 15, The sampling of groundwater well MW-101 was not conducted seasonally due to budget limitations. There were no other significant modifications of the operation, maintenance and/or monitoring program since the installation of the new charcoal filter. Drinking water and groundwater sample collection has been proposed to the Pennimans for the winter (c) an evaluation of the performance of the remedial action during the period of time since the last Status Report, including whether the remedial action is achieving remedial goals specified in the Phase IV Remedy Implementation Plan as described in 310 CMR (3), and a description of any conditions or problems noted during the period that are or may be affecting the performance of the remedial action Based on the levels found with the filter in place over 10 months, from January to October, it is recommended that the charcoal filter be changed every six months. Alternatives to the present remedial action of continued sampling, analytical testing and reporting of concentrations of chemicals in drinking water from the private drinking water well at 131 Burrage Street and the entire disposal site would be the installation of an extension of the municipal drinking water line through the neighborhood of Burrage Street in Lunenburg. The Town of Lunenburg, through the Lunenburg Water District, did install an extension of drinking water lines to Reservoir Road and Page Street in the past. That location is about 3,000 feet east on Page Street and south of Burrage Street to 131 Burrage Street. Penniman, 110 Burrage Street, Lunenburg, MA, Phase V Status Report-3, RTN Page 4

Kuriger Associates Installation of an extension municipal drinking water and connection to the municipal line would eliminate the need to consider MCP risk characterization GW-1 standards for the contaminants of concern, and also provide a source of drinking water to the residents of 131 Burrage Street and the neighborhood that would not require regular monitoring by residents. The installation of an extension of the drinking water line would also provide new revenues to the Water District and possibly facilitate further development in the area. Fran McNamara, Superintendent of the Lunenburg Water District stated recently that an extension of the water line of about 3,000 feet in that part of Lunenburg would be very expensive. Another alternative may be to install a new drinking water well at 131 Burrage Street. But there would be no certainty that the new well would be free of contaminants. (d) a description of any measures taken to correct conditions which are affecting the performance of the remedial action The charcoal filter changes at the recommended six month intervals will continue to aid in the determination of what if any changes in the concentrations of the chlorinated solvents takes place over time. Box 135 Lunenburg, MA Re: Public Notification of a Phase V Status Report for the Disposal Site at 110 Burrage Street, Lunenburg, MA, RTN Dear Sir or Madam: This letter provides public notification of the completion and submission of a Phase V Status Report and Monitoring Report for the Disposal Site at 110 Burrage Street, Lunenburg, MA, RTN meeting the requirements of 310 CMR (3)(e). In summary, based on a Phase IV Remedy Implementation Plan (RIP) prepared by others and submitted to the Department of Environmental Protection (DEP) in March 2007, and as part of this Phase IV Completion Report, the Pennimans, the responsible party for the disposal site at 110 Burrage Street, Lunenburg, MA, have operated, maintained and monitored a point of entry charcoal treatment (POET) system for drinking water at 131 Burrage Street, Lunenburg, MA. Based on drinking water samples collected and analyzed from that location, the POET is working correctly. An alternative solution to the contaminant problem that we would recommend would be the installation of a municipal water line to 131 Burrage Street and the neighborhood. The responsible party has stated that they intend to maintain the system as needed, and will conduct the appropriate testing as needed. The charcoal filter is removing low levels of chlorinated solvents, some of which exceeded Massachusetts Contingency Plan (MCP) GW-1 drinking water standards before charcoal filtration was installed.
