The following reagents and supplies are regarded as critical to performance and accuracy.

1 Subject: Blood Gas Reagent and Supply Storage and Maintenance II. Purpose: To assure the accuracy and integrity of all reagents and supplies critical to the proper calibration and performance of blood gas instrumentation. III. Policy: Each new lot of shipment of reagents critical to the proper calibration or performance of blood gas instrumentation must be checked against established criteria before patient samples are run. All reagents are labeled as to content and quantity. Storage requirements, date received are prepared and expiration date. The following reagents and supplies are regarded as critical to performance and accuracy. A. Calibrators, reagents, and controls- Store refrigerated (2-8 degrees C) 1) i-stat G3 cartridges 2) i-stat CG4 cartridges 3) i-stat EG7 cartridges 4) i-stat blood gas, electrolytes controls 5) RNA i-stat hematocrit controls 6) i-stat calibration verification materials 7) RNA AVOX co-oximeter calibration verification material 8) Eurotrol Hyperbaric Linearity material 9) Eurotrol Hypoxic Linearity material Store at room temperature (18-25 degrees C) 8) RNA AVOX co-oximeter controls 9) AVOX optical quality control filters 10) AVOX test cuvettes 11) i-stat external simulator B. Supplies- 1

2 Store at room temperature (18-25 degrees C) 1) Heparinized blood gas syringes 2) 9V lithium batteries IV. Procedure: A. i-stat calibrators and reagents 1) i-stat cartridges, controls, and calibration verification materials are shipped in refrigerated cartons. 2) Upon receipt, verify that the shipment temperature indicator strip in within the proper range. If not, do not use and notify manufacturer. 3) Record quantities and lot numbers of each cartridge type in the shipment log, and refrigerate immediately. 4) Remove from refrigeration: two (2) of each new lot number of i-stat EG7 cartridges, two (2) i-stat G3 cartridges, two (2) CG4 cartridges, and two (2) i-stat controls -level 1 and level 3. 5) Allow cartridges and reagents to equilibrate to room temperature (18-25 degrees C) over four hours. 6) If a new lot of controls are received in, obtain EG7 and GC4 cartridges previously controlled with old lot number. Allow cartridges and new controls to equilibrate to room temperature (18-25 degrees C) over four hours. Run cartridges with new lot of controls. 7) Run both blood gas/ electrolytes/ lactate levels 1 and levels 3 on i-stat analyzer. Verify that results are within expected assayed ranges. 8) Record results in the reagent receipt log. 9) If ALL controls fall within expected ranges, the cartridges may be released for use. Place in back of any older lots. 10) Should any results fall outside of expected ranges, notify supervisor and continue sequestration of lot. DO NOT USE. 11) Supervisor may rerun the failed controls at his/her discretion on the cartridges of the failed lot. If all results for the repeated runs fall within ranges, the supervisor may release the lot for use. 12) Cartridges are stamped with a two (2) month expiration date (not to exceed the lot expiration date) when removed from refrigeration. These cartridges are distributed to the download station location in the hospital for storage at room temperature (18-25 degrees C). 13) CARTRIDGES MAY NOT BE USED THAT ARE OUTSIDE THE 2

3 EXPIRATION DATE. Return expired cartridges to Cardiopulmonary department and place in expired bin. B. AVOX co-oximeter calibrators and reagents 1) Refrigerate RNA AVOX calibration verification materials when received. 2) Store received cuvettes and controls at room temperature. 3) When supply of old lot of cuvettes is exhausted, place new lot of controls near temperature probe of AVOX unit. Allow one hour for temperature equilibration. 4) Verify that the humidity indicator in the bag of new lot cuvettes is blue, indicating that the desiccant pouch in the sealed bag is effective. 5) Enter the calibration code and the cuvette lot number printed on the bag into the AVOX analyzer. 6) Run level 1 and level 3 controls on the new lot of cuvettes. 7) Record results on the co-oximetry control log. If results are in range, the cuvettes may be used. 8) Should any results fall outside of expected ranges, notify supervisor and continue sequestration of lot. DO NOT USE. 9) Supervisor may rerun the failed controls at his/her discretion on the cuvettes of the failed lot. If all results for the repeated runs fall within ranges, the supervisor may release the lot for use. 10) Cuvettes must be stored in the supplied zip-lock plastic bag with desiccator pouch. The bag must be kept closed between removals of single use cuvettes. 11) If the humidity indicator begins to turn from blue to pink, notify supervisor to replace desiccant pouch. 12) Wipe off cuvettes before use. C. Reagent and Supply Maintenance 1) Reagent storage refrigerator temperature is monitored and recorded daily. 2) Satellite supply storage areas are inventoried Monday through Friday and supplies checked and replenished. 3) Expiration dates of cartridges, syringes, and lithium batteries are checked daily and before use. 4) Out of date supplies are removed. Cartridges with expiration dates less than seventy-two (72) hours may be relocated in an area of higher use 3

4 5) Record temperature daily on control log for reagents stored at ambient temperature. If temperature exceeds established ranges, do not use reagents and notify supervisor.
