Failure to do so may result in the disqualification or rejection of the bid. As specified in Section 9 of the Kenai Peninsula Borough s invitation to bid, we have agreed to answer written questions received through April 2, Attached are formal responses to all the questions that we have received. SCOPE OF WORK Remove and replace Section , Summary. Changes to the scope of work for the Soldotna area are included in this addendum. BID FORM The bid forms for the Soldotna Area have been changed to reflect the changes to the scope of work. Please replace Pages 2, 3 and 4 of the original bid form with the attached Addendum 1 Bid Form. CLARIFICATION Any reference in the bid document to Door Replacement Project 2015 should be changed to reference Flooring Replacement Appropriate AHERA documents will be provided to the winning contractor. Divider page IV, Specifications, is incorrect, please remove and replace with the attached. We look forward to receiving your bid by no later than 2:00 PM on April 8, Note: Information in this addendum takes precedence over original information. All other provisions of the document remain unchanged.

2 April 3, 2015 Page 2 Addendum #1 KPBM Flooring Replacement 2015 Questions and Answers Question #1: Are the transition strip to be replaced in class rooms at Redoubt Elementary? Answer: Yes, new transitions strips will be required. Question #2: Is contractor responsible for Demo? Answer: See PART IV SPECIFICATIONS Summary of work PART 1 GENERAL 1.1 DESCRIPTION A. 1. Provide all labor, materials, equipment and tools to remove and dispose of existing flooring at all facilities except Redoubt Elementary and Soldotna Elementary where flooring will be removed by asbestos contractor. Question #3: Are the stringers and cove base to be replaced at Soldotna High Stairs? Answer: Yes the stringers and cove base are to be replaced with new. Question #4: What type of stair tread will be wanted? Answer: One piece rubber stair tread, with epoxy on the nose, without abrasive step insert on the top edge. Question #5: What type of flooring on the landings of the stairs? Answer: The same make-up as the one piece rubber stair treads. Question #6: In the areas that require Sheet Vinyl Flooring will contractor be installing cove base or wrapping the sheet vinyl? Answer: Cove base is to be installed in these rooms. Question #7: Will the bathroom stalls need to be removed by contractor? Answer: Stalls and toilets will be removed by owner. Question #8: Does owner know how adhered the carpet is to the floor at Nanwalek? Answer: The carpet appears to be easily removed from the floor. Question #9: There is a 5% bid bond for over $100,000, what about the payment and performance, is that only if the bid exceeds $100,000? Answer: Bid bonds and performance and payment bonds are required if the bid exceeds $100,000 (per Section 5 of the Instructions to Bidders). Performance and payment bond forms are included.

Provide all labor, materials, equipment and tools to remove and dispose of existing flooring at all facilities except Redoubt Elementary and Soldotna Elementary where flooring will be removed by asbestos contractor. 2. Prepare the floor. 3. Furnish and install: SOLDOTNA AREA Base Bid: At Redoubt Elementary School (486 West Redoubt Avenue, Soldotna AK 99669) approximately 8720 square feet of vinyl composite tile flooring, 408 square feet of sheet vinyl flooring and 1112 lineal feet of cove base and transition strips; At K- Beach Elementary School (1049 Poppy Lane, Soldotna, AK 99669) approximately 3321 square feet of carpet tiles and 348 lineal feet of cove base and transition strips At Soldotna High School (425 West Marydale Avenue, Soldotna, AK 99669) no work in base bid. At School District Warehouse (139 East Park Avenue, Soldotna AK 99669) approximately 1194 square feet of carpet tiles, 44 square feet of walk off tiles and 234 lineal feet of cove base and transition strips; and At Soldotna Elementary School (162 East Park Avenue, Soldotna AK 99669) approximately 400 square feet of sheet vinyl flooring 108 lineal feet of cove base and transition strips. Additive Alternate 1: At Soldotna High School (425 West Marydale Avenue, Soldotna AK 99669) approximately 72 square feet of rubber stair flooring; 758 lineal feet of rubber stair tread; 1 00 lineal feet of cove base and 592 lineal feet of rubber stringers. Additive Alternate 2: At School District Administration Building (148 North Binkley Street, Soldotna, AK 99669) approximately 177 square feet of walk off carpet tiles, 110 square feet of rubber stair flooring, and 221 lineal feet of rubber stair tread. HOPE AREA At Hope School (19635 Hope Highway, Hope AK ) approximately 4037 square feet of vinyl composite tile flooring and 446 lineal feet of cove base and transition strips. HOMER AREA At Homer Middle School (500 Sterling Highway, Homer AK 99603) approximately 960 square feet of carpet tiles and 212 lineal feet of cove base and transition strips. Additive Alternate 1:

4 FLOORING REPLACEMENT 2015 SECTION SUMMARY OF WORK At Nanwalek School (63550 Alexandrovsky Street, Nanwalek AK 99603) approximately 464 square feet of carpet tiles and 90 square feet of sheet vinyl flooring 150 lineal feet of cove base and transition strips. Contract documents do not purport to describe in detail absolute and complete construction information. In many instances drawings are diagrammatic and not to scale. Drawings may not portray exact conditions at any particular location and should not be used for bidding purposes. The Contractor is responsible for field verifications of conditions and quantities. It shall be the Contractor s responsibility to determine conditions and requirements at each particular location and provide all items necessary for completion of the work in accordance with the contract documents B. Particular Project Requirements: 1. During construction the Contractor will be afforded reasonable access to the premises as necessary for orderly and timely execution of work under the contract. All of the buildings will be available May 26, 2015 except Redoubt Elementary and Soldotna Elementary which will both be available July 1, All work must be substantially completed by August 7, Owner will make available space for storing and staging Contractor s materials subject to the following conditions a. Use of such area shall be covered by insurance required in General Conditions. b. Materials will be maintained in a neat and orderly manner at all times and shall not interfere with the scheduled activities of the facility. C. Dimensions: Verify dimensions indicated on drawings with field dimensions before fabrication or ordering of materials. Drawings not to scale. D. Cooperation: 1. Contractor shall cooperate fully with Borough Maintenance representatives and management. E. Installation Requirements, General: 1. Inspect substrates and report unsatisfactory conditions in writing. 2. Do not proceed until unsatisfactory conditions have been corrected. 3. Install materials in exact accordance with manufacturer's instructions, all applicable codes and regulations. 4. Install materials in proper relation with adjacent construction and with proper appearance. 5. Refer to additional installation requirements and tolerances specified under individual specification sections. 6. All Materials proposed for incorporation into this work shall be specified or, if not specified, shall be of the first quality of their respective kinds, as selected by the Contractor subject to approval by the owner. 7. Contractor will dispose of all trash, and construction debris that are generated by the project and leave the site and structures in a new and clean condition. F. Limit of Use: Limit use of work as indicated. Keep driveways and entrances clear. G. Existing Construction: Repair damage caused by construction operations. Protect building and its occupants. H. Definitions: 1. Provide: Furnish and install, complete with all necessary accessories, ready for intended use. Pay for all related costs. 2. Match Existing: Match existing as acceptable to the Owner. I. Writing Style: Specifications are written in the imperative mode. Except where specifically
