1 Exhibit A Scope of Work Mn/DOT Contract No PROJECT OVERVIEW The Minnesota Department of Transportation (MnDOT) requires the services of a Contractor for construction lead inspection, materials testing and coordination, traffic control compliance and project documentation on State Project (SP) on Trunk Highway (TH) 2. SP is a Design Build project for inslope stabilization on a portion of TH 2 in the urban area of TH 2 in Crookston adjacent to the Red Lake River. The Design Build proposals results were announced March 27, Construction is anticipated for approximately May to October 2014 and may extend into 2015 depending on the Design Build option chosen. Summary: The Contractor will provide: Field inspection and materials testing Materials testing and coordination Project documentation The State and their authorized representatives will provide: Overall project oversight and project management All key decision making authority for the project Construction surveying / staking Concrete plant inspection if needed 1.0 CONSTRUCTION PROJECT MANAGEMENT REQUIREMENTS (The Activity Code for this work is 1800) Contractor will provide construction inspection, materials testing, traffic control compliance, project final documentation all which will assure that the project is completed within reasonable conformity of the plans, specifications and contract provisions Provide one full time inspector meeting the below requirements: High school graduate plus eight (8) years of experience in construction inspection with two (2) years in grading and base or bridge construction and have the following certifications: Concrete Field 1 & 2, Grading and Base 1 & 2, Bridge Construction Certification and Erosion Control Site Management. Responsible for performing highly complex technical assignments in construction, making and checking engineering computations, inspecting construction work and conducting field tests and is responsible for coordinating and managing other inspectors. Work is performed under general supervision of the Project Manager/Contractor. Para-professional engineering skill and knowledge can be applied to difficult transportation issues. Contractor must be able to make sound and rational decisions after considering alternatives. Has expertise in human relation skills and oral communication skills. Has expert technical knowledge to handle most complex problems. 2 1.3 Submit in writing to the State Project Manager the name of each person assigned to the construction project, together with a detailed resume with respect to salary, education and experience. 1.4 All services will be performed in accordance with the established standard procedures and practices of State. Prior to furnishing any services, Contractor will be familiar with State standard procedures and practices as set forth in the documents listed in Items to be obtained from State by the Contractor, section of this scope of work and with informal procedures and practices for construction, engineering and contract administration used by State. Contractor will maintain close coordination with State and the construction Contractor in order to minimize rescheduling of activities due to construction delays or changes in scheduling of the construction Contractor s activities. 1.5 All activities and decisions of the Contractor relating to the project will be subject to review by State s Project Manager. Contractor will cooperate and assist the State s Project Manager, or his representatives, in the conduct of reviews. Contractor will comply with recommended corrections within one week of notification. 1.6 Provide coordination of all activities, correspondence, reports and other communications related to its responsibilities under this agreement. 1.7 Provide lead inspection at all times while the construction Contractor is working on the construction contract. If the construction contract is suspended, the Contractor will be adjusted to correspond with the type of cessation, either complete or partial suspension. 2.0 INSPECTION, TESTING AND DOCUMENTATION REQUIREMENTS (The Activity Code for this work is 1800) It will be the responsibility of Contractor to provide services as necessary to administer the construction contract in the manner so as to assure that the project is constructed in substantial conformity with the plans, specifications and contract provisions Construction Inspection: Have a working knowledge of the current Contract Administration Manual and Mn/DOT Specification s book, and possess all the knowledge, skills and abilities required in obtaining the necessary certifications for performing the duties of the position they hold. Contractor will ensure that State s current practices, policies and procedures are met throughout the course of the project Perform technical duties in inspecting work to assure compliance with Mn/DOT Contract Administration Manual, Specification Book, special provisions, construction plan, policies and procedures, standards, Occupational Safety and Health Administration (OSHA) regulations, and Federal Highway Administration (FHWA) guidelines, as well as any other applicable laws. The Contractor will provide on-site construction inspection on a full time basis during all necessary construction activities. The Contractor will provide sufficient quality of inspection services to ensure that all construction is in accordance with approved plans, specifications, special provisions, and that compliance with State construction standards and policies is maintained Attend any conferences or meetings necessary for close coordination during day-to-day progress of the work or as required to carry out the Contract. Meetings will include a pre-construction and close out meeting Be familiar with the standard construction practices of State, the construction plans and contract(s) for the project, and the construction Contractor s proposed schedule of operations prior to beginning field services under the Contract Notify the State Project Manager immediately of any unanticipated project conditions Advise the State S Project Manager in writing of any omissions, substitutions, defects and deficiencies noted in the work of the construction Contractor and the corrective action taken Coordinate and direct State inspection forces in the field to assist Contractor Lead inspector in all aspects of the construction contract from materials testing to daily inspections. 3 Perform field operations in accordance with OSHA regulations and accepted safety practices Provide for Contractor personnel transportation, supplies, materials and incidentals as are needed to accomplish the services required under the Contract Provide on-site construction inspection on a full time basis during all pertinent construction activities Contact the District 2 Surveys Office and coordinate staking as necessary Perform daily reviews of construction signing, detour signing, and construction traffic control maintenance. Take any corrective action necessary Perform reviews of temporary and permanent erosion control measures on the project for compliance with applicable permits and specifications. Keep a diary of reviews and recommendations made to the construction Contractor for remedial action to be taken Provide pavement marking activities such as surveying and documenting inplace striping, spotting for interim and final marking, and inspecting the installation of pavement markings Materials Testing Services Performed by Contractor Perform and/or coordinate State forces in sampling and testing of all component materials used on the project and will also obtain samples for submittal to State s District Laboratory. Field testing may include, but is not limited to, concrete (slump tests, cylinders, etc.), bituminous (asphalt cement samples, temperatures, quantity weigh tickets, gradations, etc.), grading and base materials (gradations, moisture, density, etc.), pile driving (driving depth, etc.) in accordance with the current schedule of materials control. The Contractor will sample and test as appropriate, materials to be incorporated in the work and will reject construction Contractor s work and materials not meeting the designated criteria. State will be providing all of the testing equipment for the project, including but not limited to, concrete air test equipment, slump cones, gradation sieves, dynamic cone penetrometer, sand cone density equipment, proctor kits, etc Materials testing will be done in accordance with current State s schedule of materials control and State standard specifications or as modified by the Contract special provisions. Where the schedule of materials control specifies laboratory/agency testing, those tests will be performed at the District Materials Laboratory. State will be responsible for delivering the samples to the District Materials Laboratory Keep daily diaries, logs and records consistent with State policy as are needed for a record of the construction Contractor s progress. Necessary concrete, bituminous and grading and base reports will be prepared by or in coordination with the Contractor and submitted to responsible parties as required during the same week that the construction work is done or as otherwise directed by the Construction Project Manager Testing equipment used by the Contractor that was not provided by State will be calibrated at least once per year, or at the specified intervals, in accordance with State s Laboratory, Bituminous, Concrete and Grading and Base Manuals No provisional certifications will be allowed for these testing procedures Contact the District 2 Independent Assurance Inspector and request an Independent Assurance review for the project. This review may be necessary for all testers and observers; at least one documented check of each tester and observer is required each year. The individual tester or observer may be required to perform the test(s) in either the field or the District Materials Laboratory. The tester is defined as the individual who actually runs the test. The observer is defined as the inspector who watches the Contractor run the test. 2.3 Project Documentation performed by the Contractor Make certain that test report records or certificates of compliance have been received, prior to the incorporation of materials in the work, for materials tested off the project site Prepare and submit such periodic, intermediate and final data and records as may be required by the State and as are applicable to the project Collect, properly label or identify, and deliver to State all original diaries, logs, notebooks, accounts, records, reports, and other documents prepared by the Contractor in the performance of the Contract, upon completion or termination of the Contract. Keep daily diaries, logs, and records consistent with State practices as are needed for a record of the construction Contractor s construction progress Review and monitoring of construction Contractor s schedule for construction and contact the Independent Assurance Inspector when the project starts to provide servicing of the project. 4 2.3.5 Prepare all appropriate Contract Time Documents (Weekly Construction Diaries & Status Reports) in accordance with the Specifications, Special Provisions of the Contract Administration Manual. Submit to State for review Complete daily inspection reports (including TRACS daily reports), weekly concrete reports, daily concrete accomplishment reports, material certification, and Contract correspondence. Collect documentation, in accordance with the Contract Administration Manual, for work quantities completed. Measure and compute quantities of all materials incorporated in the work and items of work completed, and record date in the Field Operations (OPS) database Document changes in plans and specifications for As-Built Plan preparation Prepare partial estimate pay vouchers using Field OPS and provide to State for approval. Partial pay vouchers will be generated at regular intervals not to exceed once a month Prepare final documentation of construction pay items, quantities, material certification and other requirements as per the plans, specifications, special provisions and the Contract Administration Manual Final Inspection Conduct final inspection and recommend project acceptance Prepare final documentation of construction pay item quantities, as per the plans, Specifications, Special Provisions, and the Contract Administration Manual Final Payment Voucher Documentation Prepare final project documentation in accordance with State Construction Manual As-Built Plans and Plan corrections denote plan changes on paper copy of project plan The OMDCM will thoroughly understand all functions of the TRACS document control system. They will maintain the MnDOT document management system in TRACS. They will be responsible for organizing the documents in the TRACS system and enforcing that all documents are properly submitted in TRACS. They will coordinate document control items with the Design-Builder s document control manager to ensure that submittals from the Design-Builder are done properly. MnDOT will perform the initial training to both the DBVC and Design-Builder. The OMDCM will be responsible for identifying document control procedure errors in a submittal from the Design-Builder and then will inform and train the Design-Builder, so the Design-Builder corrects their errors and ultimately begins making submittals following the correct procedures Serve as Compliance Officer to ensure Contactor s compliance with Affirmative Action/Equal Employment Opportunity (AA/EEO) plans. The Contractor will arrange a meeting with the State s Project Manager no later than 2 weeks after the Design-Build Contract award to determine exact reporting requirements needed for this task Serve as Compliance Officer to ensure Contactor s compliance with Disadvantaged Business Enterprise (DBE) requirements. Contractor will schedule and facilitate monthly meetings with State s Project Manager to review DBE requirements. 3.0 ITEMS TO BE OBTAINED FROM STATE BY THE CONTRACTOR: 3.1The most current printed documents and publications listed below are to be obtained by the Contractor prior to beginning work. Compliance with these guides, manuals, procedures, and directives will be a requirement of this contract. The Contractor will review this list and request all applicable documents from the appropriate office. 3.2 The following publications are available for purchase by the Contractor at Mn/DOT Manual Sales Office at (651) , or available on Mn/DOT s website. Office at (651) ) Mn/DOT Technical Memorandums are available on the Mn/DOT website State will furnish the Contractor those computer services normally provided to Engineers required to perform contract administration activities, including but not limited to, Transportation Automated Control System (TRACS), Contract Management Field Operations Module (Field OPS) (Contract Administration Advanced Record System previously CAARS), the construction quantity documentation and payment program. 3.4 Two (2) sets of Construction plans and proposals will be furnished by the State 3.5State will furnish a source to obtain all standard forms to be used in carrying out technical services under this Contract. 4.0 ITEMS FURNISHED BY CONTRACTOR: Photographic equipment, tapes, rules, and any other items necessary. A clearly identifiable vehicle to maintain the necessary coverage of the project. The vehicle will have the necessary safety features to be within the work zone and be able to safely carry concrete cylinders, density equipment, or any other equipment or materials as deemed necessary by the Construction Project Manager. Furnish and maintain a cellular phone for the Contractor field person assigned to the project. The phone number will be available to the State Project Manager and the construction Contractor. A computer capable of running the Field Ops software. The hardware and software to be utilized will be compatible with hardware and software being utilized by State.
