1 ATTACHMENT 1 QUESTIONNAIRE General Instructions The following questionnaire asks for information concerning the Respondent's organization, experience, operational approach, cost response, and reference facilities. The Respondent must submit responses to the questionnaire in a separate document (do not use the one below) in the exact order as listed below, with the same headings and numbering system. Please list the question or information request contained in the questionnaire before your response. Generally, the Respondent shall include information for the specific single business organization or entity that is submitting a response. If the information being submitted is not for the specific responding entity, please note such in the response. If a major portion of the work would be subcontracted, information for that subcontractor should be submitted and specifically referenced. All answers must be specific and complete in detail. The County reserves the right to make independent inquiries concerning the information submitted herein, conduct any additional investigation necessary to determine the Respondent's qualifications, and require the Respondent to supply additional information. Use of Attachments Schedules, resumes, reports, diagrams, and other forms of information may be used as attachments, provided that the Respondent clearly references the attachments and how the supplemental information is relevant to the topic. 1, ORGANIZATIONAL INFORMATION 1. Name of the lead entity, type of entity (corporation, partnership, individual, LLC, or other; if other, please describe). Also include the names and roles of other team partners and subcontractors. 2. Please provide the following information for the lead entity: a. Address, phone number, address, and website. b. Federal tax ID#. c. Project manager and direct contact information. 3. Submit an organizational chart showing ownership percentages and management arrangements between all entities participating in the execution of this project (technology provider, design firm, construction firm, operations & maintenance, guarantor, project financier, etc.) 4. List the name, title, mailing address, address, and phone number for the person who shall serve as the main contact for the Respondent. 5. How many years has this business used its present name? If this business has a parent company, has done business under other names, or is doing business under other names, list the parent company (including its address, phone number, address, and website) and names of the other businesses under which you have done, or are doing, business. 1

List and explain the status of any lawsuit(s) or other legal action(s) that could affect your ability to carry out the functions outlined in this RFQ, and in which you or a company affiliated with you (i.e., a parent company, a company in which you own an interest, or a company in which your parent company owns an interest, as applicable) are a party. 2. EXPERIENCE, EXPERTISE, & PAST PERFORMANCE 1. Describe the experience of the lead entity in performing work similar to that described in this RFQ. a. Include names, organizational affiliations, and contact info for references, as well as their role in the work that was done b. Include a description of the lead entity s role (i.e., prime or subcontractor, or owner) and how the project was similar to the work called for in this RFQ c. If you have not had similar experience, include experience from partnering entities and indicate how the Respondent will access this expertise. Include enough information to satisfy, at a minimum, the Experience requirements in Section 4.3 of the RFQ. 2. Describe the experience of the lead entity and key personnel in successfully managing the type of organics anaerobic digestion operation being proposed, including permitting, financing, design/construction, feedstock acquisition, product marketing, regulatory compliance, and operation and maintenance. 3. Identify personnel, their roles in relation to the work described, experience with similar projects, percent of their time dedicated to this work/project, and special qualifications they may bring to the project. Indicate where they would be located. Include resumes of key management individuals. For each staff person, be sure to include the following: a. Name; b. Years of experience performing similar work; c. Past and current employers where staff performed similar work; d. Key projects where similar work was performed; and e. Relevant certifications and training. 3. OPERATIONAL APPROACH & PROCESS General Approach to Completing the Project Respondents are requested to provide a high-level summary of how they would execute the project including: 1. Description of how your process would manage organic materials from delivery to all outputs (solids, liquids, energy, residuals). Include the estimated throughput. 2. Potential partnering arrangements, if a team of partners is planned. 3. Overview of potential partners and how risk would be allocated (design, construction, operation, and maintenance). 4. Description of what type of contracting arrangement (e.g. Public-Private Partnership, Design- Build, Design-Build-Finance, Design-Build-Own-Operate, etc.) Respondent prefers for a proposed facility. In addition, the proposed roles of the Respondent, Hennepin County, and any other parties should be clearly identified. Identify business and contractual terms and conditions that 2

3 would be required by the Respondent, e.g., ownership and operation models, Respondent s roles, county roles, roles of any other parties, feedstock expectations, etc., should all be clearly identified. 5. Description of the plan to secure appropriate facility site, permits, and approvals. If Respondent already has a candidate site, provide a description of the proposed site and its current status. 6. Provide estimated timeline, with major milestones, for the project from potential contract award to facility startup. Include estimated duration of each major step (e.g. site acquisition, permitting, construction, etc.). Preliminary Design Concept Provide a description of the design concept, including retention time of the materials in each stage of the proposed digestion process from receiving to final disposition. Include the following elements: 1. Feedstock Description - Describe the types and amounts of each incoming stream needed from the County and other sources. Refer to Attachment 3 for details about source-separated organics currently collected in the County. Include tons per year and percent of total each stream represents. Include information about any non-food items you intend to accept and seek to secure and proposed management methods. 2. Feedstock Sourcing If some of the feedstock described above is already secured, describe the type and quantity. 3. Receiving Incoming Material - Describe equipment and process, feedstock acceptance standards, prohibited items, contamination limits, material handling methods, and ability to accommodate variable feedstock. 4. Pre-Processing (if applicable) - Describe sequencing, type of process (separation, size reduction, etc.), throughput, quantities, and typical characteristics of resulting streams. Identify and describe major equipment used. 5. Processing - Provide a description of the technology, equipment, and process. Describe how all feedstock is fed into the process, how it is converted, major equipment used, throughput capacity (tons/hour), any additives used, quantity and characteristics of solid and liquid residues produced. 6. Post-processing (if applicable) - Describe equipment and type of process (dewatering, size reduction, etc.) 7. Energy production (if applicable) - Describe the energy production process and equipment, and the types of energy/fuel produced. 8. Non-recoverable residuals management - Describe the sequence of operations and major equipment used to remove residuals, types and characteristics of residuals, estimated quantities of each, management and disposal methods. 9. End products - Describe types, characteristics, and typical annual quantities of end-products produced; the markets to which the products may be supplied; and their use. Describe any additional processing or conditioning that needs to occur to make end products usable and marketable, including where any additional processing will take place. Indicate if end use markets are already secured. 10. Describe any ancillary processes/operations that would enhance the overall project (e.g., a greenhouse and/or aquaponics operation for producing local food, educational facility focused on teaching the public about the value of recovering organic materials and the processes used at the facility, etc.) 3

4 11. Nuisance and environmental control - Describe the design, equipment, and/or operational procedures used to mitigate and/or manage odor, vectors (e.g., birds, rodents and other animals), dust, leachate, stormwater, litter, noise, and traffic impacts. 12. Attach conceptual and process schematic flow drawings which, at a minimum, show: exterior detail, material flow, and major processes at the proposed facility. 13. Describe how the facility could be expanded to meet the various potential additional annual tonnage as it becomes available from Hennepin County and the rest of the region (20,000; 30,000; 40,000 and 50,000+ respectively). 4. REFERENCE FACILITY Respondent must provide information on reference facilities to demonstrate the Respondent s experience and capabilities and the proposed technology s viability in enough detail to satisfy the requirements of Section 4.3 of this RFQ. Information may be provided on up to three (3) facilities, and should focus on facilities processing the same type and quantity of feedstock as Hennepin County s. For each operating reference facility, provide the following information, including additional information as necessary within the submission. 1. Facility name, location, owner, and operator. 2. The scale of the facility (e.g. demonstration, commercial). 3. Unit size (in tons per day) and number of units. 4. Design and actual operating throughput. 5. Description of all feedstock accepted, percentages of each, source of feedstock, contamination maximums or tolerances. 6. End-products/outputs produced, including quantities of each, use of each, and disposition of all end products including contaminants. 7. Status of facility (e.g. operating, under construction). 8. Detailed description of system. 9. Operating history (e.g. date commissioned, period of operation, availability over the past three years). 10. Major problems/shut downs and lessons learned. 11. Environmental permits, inspection and compliance status. 12. Primary revenue sources. 13. Financial status of the facility. 14. Relationship of respondent to the reference facilities. 5. ESTIMATED COSTS Provide estimated costs for proposed facility as noted below. 1. Base capital costs - Provide estimated costs for each component of the site preparation and proposed facility. 2. Operation and Maintenance - Provide planning level operating and maintenance costs for the facility design concept. 3. Tip fees - Provide an estimated range of per ton tip fees to be charged for each of the following annual tonnages of organics materials received: 25,000; 30,000; 40,000; and 50,

5 6. FINANCIAL CAPABILITY Respondent must provide the following. 1. Provide a preliminary financing plan that reasonably demonstrates the Respondent has the capability to structure private project financing, if required, including indications of any County commitments that would be necessary to support that financing. 2. Credit references 3. Other information, as appropriate, to demonstrate the financial resources and capability of the Respondent 7. SUSTAINABLE BUSINESS PRACTICES 1. Environment - Describe in general terms how your firm lessens its negative operational impacts on the environment. Describe your firm s commitment to environmentally conscious business practices. Include sustainable building design elements to be incorporated into a proposed facility. 2. Economy - Describe in general terms how your firm supports the local community such as community economic development and community workforce development. 3. Community - Describe how your firm supports its employees by providing living wages and benefits. Describe your employee compensation structure, healthcare and other benefits provided to your employees. 5
