BRIER DRIVE, SAN BERNARDINO, CA (909) FAX (909) ADDENDUM #1 FOR RFP #PC507 Tree Removal and Sewer/Water Line Rehab at Medical Center PH Community The following addendum to this Request for Proposal is hereby incorporated into the bid process. PLEASE NOTE: Please change your Scope of Work as follows: BID PROPOSAL FORMS HAVE BEEN UPDATED please use the ones attached to this document to submit your Bid. Questions and Answers: The following questions and answers were asked during the Job Walk/Pre-Bidder s conference. Please review the responses and include them in your proposals. 1. Are there liquidated damages for this project if so how much? Yes, liquidated damages will be assessed in the amount of $300 per day for this project. 2. Is there a Phase Plan for this project? No. We will work out a schedule with the successful contractor during the preconstruction meeting. 3. What is the engineer s estimate for the project? The construction estimates are: Tree Removal $130,000 to $180,000 Sewer Laterals $285,000 to $291,000 Water Laterals $155,000 to $186, Is there a breakdown by size of tree for this project? Yes. The size of tree is based on a chest-high measurement of the diameter of the trees to be removed. Attached to this document is a Demo Sheet which shows the location and approximate size of the trees.

The actual number of trees may vary. Pricing for additions or deletions of trees by size will be made based on the bid proposal prices. 5. The 2-inch SDR sewer lateral material is specified as SDR 35, it is only available in SDR 13 or 26. Need clarification on this. The correct Sewer Lateral Material is SDR As per our job walk we were informed that pricing would be individually per unit. This would be significantly lower than the $25,000 limit for bonding. So is the bond still necessary? No the entire project is above the limit. 7. With the individual per unit pricing, would that mean we could possibly have more than one successful plumbing bid award? For the different buildings this would obviously impact our costs, ie.-container rental, excavation equipment, and quantity material pricing. No, we plan to award to one contractor for each trade or a single contractor for both trades, but we want a break down of each unit size, for billing. HACSB will accept invoices based on 100% of each completed building. 8. Could we get a confirmation on permits? Will they be required, and should they be included in the bid per unit? Please include the cost for permits for the sewer work in your bid submission. 9. Could the excess native soil, due to the sand embedment, be used in the tree removal areas? Yes, if it is clean.

3 10. Do we have to use SDR35? Because the smallest size is 4. I believe if we talked to the city, they would allow us to use ABS and that would be a big Savings. If the city agrees then yes if not we will have to use SDR The Whitney Young Building the water in on the other side of street with a 3 backflow. Will we be moving the backflow and water main or replacing in the same location? We will replace the lines to the curb; we will not disturb the asphalt. 12. The office has to two backflows and water meters, will we be replacing both? No, we will re-plumb the lines to the office but leave all landscaping lines alone. 13. The Johnson Building water service is under asphalt are cutting and patching or moving main? This should be the only location that we will need the asphalt cut and replaces because it is fully covered. 14. Will the Housing Authority be inspecting the onsite project work? Yes, HACSB will be performing inspections for the trees and the water lines. However, the City of San Bernardino will inspect sewer connections and lines. 15. Does the work fall under San Bernardino Development services jurisdiction? HACSB is an independent agency. The work under this project, to the best of our knowledge; is not under their jurisdiction. 16. Usually inspection fees for water hook ups are per location (SB Development Services), will this apply to this project or will housing authority apply for waiver of fees? We will try to work with the City to have these fees waived. 17. Will a Contractors class A license be accepted? Yes. 18. Will you accept a B license as long as the contractor has a subcontractor with the appropriate license? Contractors with a B license must have a subcontractor with the appropriate

4 license for the portion of work that they are submitting a bid for. The licenses required are: LANDSCAPE CONTRACTOR C-27 PIPELINE / PLUMBING CONTRACTOR C34 C Who do we need to notify when we begin digging? Contractor must call 811 (Digalert) to have utilities marked within entire job limits for each building. 20. How should we handle excavations? All excavations must be backfilled and compacted to finish grade or covered with appropriate OSHA approved protective material at the end of each day. 21. Will we need to provide any barriers or fencing? Temporary fencing will be used at each tree removal/excavation site. Approved shop drawing and submittal is required before scheduled work begins. 22. How long is the average pipe run for the sewer lateral & water service? Approximate range is lf 23. How many sewer laterals & water services should be included in our bid? Looking for a number drawings are hard to interpret. For a single unit, it is from the back of the curb to the hose spigot, and the double unit, there is a T form the main line that goes across the yard to the other unit. Brier Drive San Bernardino, CA Scope of Work: Removal of existing trees approved by Project Manager and HASBC, Remove Trees, stumps and root ball system to a depth and breadth of the distance of twice the diameter of DBH. All scheduled work will be pre confirmed and approved by Project Manager and/or HACSB. Use methods required to complete the Work within limitations of governing regulations and as described in the Project Manual. This bid includes providing all labor, materials, equipment, sanitary facilities, and inclusion of other costs associated with completing the work including disposal fees. Note: Contractor/sub-contractor will provide own sanitary facilities (portable toilets). Pursuant to and in compliance with this Invitation to Bid and other documents related thereto, and subject to all conditions thereof, the undersigned hereby proposes and agrees, if this bid be accepted, within thirty (30) calendar days from date of opening, to furnish all labor, material, equipment and to perform the specific work required thereby within One Hundred-fifty (150) calendar days after receipt of Notice to Proceed.
