1 Edmonton Catholic Schools is now accepting applications for the position of Coordinator, Family Supports for Children with Disabilities (New position) Edmonton Catholic Schools is a large urban school district whose mission is to provide a Catholic education that inspires students to learn and that prepares them to live fully and to serve God in one another. The District is comprised of over 39,000 students and 3,500 employees in 89 schools and other sites. This position involves case management and supervision of contracts for children accessing Program Unit Funding (PUF), who have been approved for Specialized Service Funding through the Government of Alberta Family Supports for Children with Disabilities (FSCD) program. For children accessing PUF, Edmonton Catholic School District is an in-home specialized service provider. This position will be responsible for ensuring that the timelines identified in the contracts are maintained, ensuring that all hours identified are fulfilled for the child/family. The incumbent organizes and facilitates meetings between staff and the family including service plan meetings. The job maintains detailed and up-to-date notes on the child, collects and analyzes data and participates in research activities. This position will liaison between the multi-disciplinary team (MDT), which may consist of Speech-Language Pathologist, Occupational Therapist, Physical Therapist, Behavioural Specialist, and the child and family. This position will also be the primary contact for the child s FSCD Case Manager, Government of Alberta. The job collaborates with the respective therapeutic specialist in the functional supervision of therapeutic assistants. The job is part of the Early Learning management team. This is a permanent, 12 month, full-time, Out of Scope, Level 7 position with a salary range of $78, $97, to commence as soon as possible. Primary Duties and Responsibilities: Identify the timelines of the child s contract and adhere to those timelines by ensuring service plans are developed, planning and facilitating meetings, paperwork and billing is completed; Work in partnership with the MDT to develop a system of tracking and monitoring therapeutic progress that the child is making within the home; Ensure the goals identified in the service plan reflect the priorities of the family and can be carried out within the home environment; Support the family, MDT, and therapeutic assistant in identifying goals that have been met and changes that may need to happen; Collaborate with MDT members to manage the delivery of service plans by therapeutic assistants; Ensure therapeutic specialist provides accurate documentation and progress reports; Participate in and represent the district as an in-home service provider of specialized services at community meetings. Qualifications: Master s degree in Social Work, Education or related field, plus five years directly related experience; Knowledge of the Alberta Family Supports for Children with Disabilities program; Knowledge of Individual Program Plan development principles, Early Learning, Child Development and Inclusion principles and practices; Knowledge of Case Management and Coordination principles and practices; Knowledge of program planning and project management techniques Excellent organizational, communication, and interpersonal skills; Ability to work with a wide variety of clients and needs; Strong teamwork and leadership skills with the ability to work independently and as part of a team. Address issues/concerns raised relating to therapeutic assistants job performance and ability to carry out the service plan. Contribute to the performance evaluation process and provide feedback to the therapeutic assistant. Identify professional development needs and coordinate delivery 3. Liaison activities Act as the primary contact for the program and district staff Report on issues/concerns that were identified by the funder, district staff or stakeholders Discuss issues/concerns with the unit s management team Participate in and represent the district as an in-home service provider of specialized services at community meetings. 4. Administrative activities Develop, maintain and review program policies and procedures Ensure the therapeutic assistant provides accurate billing/invoicing to the billing clerk. Maintain accurate financial records with the assistance of the business manager. Ensure that the child s main record and documentation is held in a secure location and accessible electronically. Complete the scheduling of the therapeutic assistant and the MDT team members. Develop and maintain a data collection tool and participate in research. 5. Maintain technical knowledge and associated credentials through a range of professional development activities PROBLEM SOLVING: Describe difficult or challenging situations the position is typically expected to solve; the degree of originality of the solutions; assistance available; and any problems or decisions that must be referred to a supervisor for resolution or approval This job will requires the ability to support the therapeutic assistant, the MDT members and the child/family while adhering to the guidelines of the contract. The work involves a variety of skills in mediation, conflict resolution, creative thinking, problem solving, cultural awareness and advocacy. The ability to resolve challenges within the home setting and staff scheduling will be important. Ensuring alignment between specialized services at home and at school is essential. ORGANIZATIONAL DATA: Identify any measurable areas upon which the job has either direct or indirect impact. Measures that may be included are: number of staff supervised, annual operating budget, number and size of projects, clients served or any other significant values. Include examples that indicate the size of the job. Attach a complete Organization Chart that includes supervisor, peers, direct reports and staff. The Family Support for Children with Disabilities (FSCD) Program provides a wide range of family-centred supports and services. Services are meant to help strengthen families ability to promote their child s healthy development and encourage their child s participation in activities at home and in the community. The job collaborates with the therapeutic specialist in supervision and practice and documentation of therapeutic assistants. This position will involve working within the home setting and will require flexibility as there may be a requirement to work outside of core hours.
