Introduction COMMERCIAL INTERNATIONAL BANK, EGYPT (CIB) aim to establish a new Electro Mechanical Consultants / Contractors registry for the years 2015 & 2016 for the banks construction & renovation projects and repair works. CIB is seeking well known and efficient Consultants whom are able and qualified to produce a high quality construction services that meet CIB expectations. Hereby, CIB is inviting local Electro-Mechanical consulting firms to submit their profiles and fill in this qualification form to include their experience in the following consulting fields : 1- Preparation of Electro-Mechanical Tender Documents including but not limited to preparation of the detailed design drawings, technical specification, Bill of quantities, preparation of Project vendor list etc. 2- Tenders evaluation. 3- Construction supervision for all Electro-Mechanical tasks which include : Electric Power works which includes Cabling, Lighting, sockets, transformers, generators, earthing works. Light Current systems which includes CCTV, Access control, sound system, Data system, fire alarm system, T.V systems & Telephone systems. HVAC Works. Firefighting & Plumbing. 4- Implementation of Quality Control systems. 5- Implementation of Health & Safety procedures. It is expected that the Pre-Qualifications forms will be complete and sufficient. However, CIB reserves the right to obtain subsequent clarifying statements of information from the participating Contractors

4 2. Instructions to Applicants 1. The invited Consultants should completely fill all the requested information included in the Prequalification form. 2. The data Provided by the Consultants should be listed in the same order as requested in the prequalification form. 3. The Prequalification form should be stamped and signed by a legally authorized representative of the Consultancy Firm. 4. Consultants should ensure that all the information provided is correct, readable with full clarification (if needed). 5. Attachments could be added to any page where more space is required. 6. Invited consultants should support their provided information by copies of the relevant documents and references e.g. letter of awards, certificates etc. 7. Applicants should deliver a sealed envelope marked CIB Electro Mechanical CONSULTANTS Prequalification form no later than to the following address : Commercial International Bank (CIB) Tender & Contracting Department 17 Tiba St., Behind El-Seid Club, Mohandseen, Cairo.

Consultant's Statement to be written on Consultant' s letter head It is understood and agreed upon that the information submitted is to be used by Commercial International Bank (CIB) in determining, -according to their sole judgment and discretion- the qualifications of prospective Consultants to perform works as described above. In consideration of being permitted to submit his qualifications as a prospective consultant for review, the undersigned waives any claim against CIB that might arise with respect to decision as to prospective consultant's qualifications. It is understood that the decision of the CIB with respect to the qualifications of any prospective consultant is final and not subject to appeal of any kind. A prospective consultant will not be considered qualified by the CIB unless he possesses high reputation, ability, experience, and Project funding guarantees sufficient in the judgment by the CIB to render it probable that he can satisfactorily execute the works, should it be awarded to him, and meet his obligations therein incurred. The signing by the undersigned of his attached form in duplicate guarantees as evidenced by the sworn affidavit required herein, the truth and accuracy of all statements and of all answers to the interrogatories hereinafter made.
