This event is proudly brought to you by the Riverside County Regional Park and Open-Space District in collaboration with Riverside County Parks Foundation, Healthy Jurupa Valley, the City of Jurupa Valley, Jurupa Area Recreation and Park District, Jurupa Unified School District, Reach Out, the City of Riverside, Riverside Community Services Foundation, Riverside County Health Coalition, and the Riverside University Health System-Public Health. The 4 th Annual Healthy Living Extravaganza will be taking place on Saturday, April 28 th 2018 from 10 AM 3 PM at Ranch Jurupa Regional Park located at 4800 Crestmore Rd in Jurupa Valley, California. This year we will be joining forces with the Riverside County Superhero 5 K Run/ Walk which will commence at 8 AM the morning of the Healthy Living Extravaganza. Participants of the Healthy Living Extravaganza are encouraged to come early and participate in the Run/ Walk. Event participants will get the opportunity to explore and take advantage of free health screenings, nutrition and meal planning, opportunity drawings, kid s zone, food zone, entertainment and more! This event is beneficial for all ages and income levels, as we encourage all to come and bring the whole family to learn about available services throughout Riverside County. Resources are brought to one central location where Riverside County residents are able to gain invaluable information on how to live a healthier and more active life.

Company banner placed at volunteer station. We accept donations too! Donations can range from $25 and up. All Donors will receive text acknowledgement on our event website. Donations can range from all of the following: monetary gifts, gift cards, exercise/sports equipment, media marketing donations, miscellaneous services, etc. *Sponsorships will be valued at a cost that will be determined based on cash and/or in-kind contributions.

8 A D D I T I O N A L I N F O R M A T I O N Donations to the Healthy Living Extravaganza are tax deductible! Application Deadlines: The deadline for your Sponsorship Commitment is January 3, To complete your sponsorship contract, you must sign the terms and conditions and return your sponsorship registration and payment to the address listed below. Early-bird Sponsorship Contracts: If you submit a contract before November 30, 2017, and choose a sponsorship package at or above $2,500 you will receive an a la carte item for free. Terms of Cancellation: There will be no refunds issued after point of sale, and the full balance of the sponsorship invoice must be paid on or before February 28, All early bird contracts must pay 50% of their full sponsorship on or before January 3, 2018 and remaining balance on or before February 28, Any unpaid balance will be due at time of cancellation. Instructions for Logo Submission: Submit your company logo in high resolution JPG and EPS format. The logo will be used for print and website purposes.
