1 Instructions for Using the Maintenance Department s Computerized Maintenance Management System Request Line For Audio/Visual Equipment Repairs 1. Work Order Request Line Module. After you log in the system will display the menu below. (See Figure 1.) a. Equipment Repair Custodial - allows you to submit work orders for repair of audio-visual equipment. b. Site Specific Work Order Status Report - allows you to review work orders already in the system for your site. (See Figure 6.) c. Change Password - allows you to change your password. 2 Figure 2 School/Site. If your location is not correct, or if you have moved to a different location, send an using the maintenance help link on the Log In web page. Request Type. Select Audio Visual. Serial Number. Insert the serial number for the equipment in need of repair. If you don t find a serial number, enter the word unknown. Requestor automatically displays your name. Contact Info automatically displays your name. If questions and information about this work order should be directed to someone else, please enter that person s name here. Priority defaults to the Routine Work Order Priority Number - 7. The requestor may change the priority based on the need and the severity of the situation. For audio/visual equipment you should only need 7 or 8. Your items will be picked up according to the site s pickup/delivery schedule. Requested Completion Date is based on the priority. You can modify this date, but the return of your equipment will depend on your site s pickup/delivery schedule and how long it takes for the equipment to be repaired. Description allows you to enter a short description of the audio/visual equipment that needs repair. Be sure to tell us about the type of equipment. We want the serial number, but it does not tell us what kind of equipment it is. Describe the problem you are having with the equipment but please be brief and clear. 3 The Clear button clears any information entered so you can start over. The Submit button saves the information into the system. The newly produced work order is made available for review in a report format. (See Figure 3.) You may print this report by selecting Print this page which appears at the bottom of the screen. Work orders are also made available for review in a summary report format. The summary report is site specific and is titled Work Order Status Report. 4 Figure 4 - Work Order Status Report Work Order Status If a work order is rejected or voided, a reason should be shown in the Description block of the report. A list of status codes is shown below. 1. Submitted (S) A work order has been submitted by the customer but has not been reviewed and processed by the Maintenance Department s Customer Service Section. The work order will not become active until the status is changed to incomplete. 2. Incomplete (I) Work Orders. A work order that has been distributed to a technician but has not been completed. 3. Material Delay (M) the work order is delayed because the necessary materials are on order.. 4. Void (V) A work order that has been voided or cancelled. 5 5. Rejected (R) If a work order is rejected, the reason should be displayed in the description box. 6. Complete (C) The work has been completed and the labor and/or materials used information has been entered. 7. Preventive (PM) Preventive Maintenance work. Survey The work order survey is hyperlinked to the description when the work order is completed. This opens a feedback form in a new window.
